A hipotensão pós-exercício caracteriza-se pela redução da pressão arterial após uma sessão de exercício. Temos estudado este fenômeno há 10 anos, de modo que esta tese visou caracterizá-lo, avaliando sua magnitude, duração, fatores de influência e mecanismos. Foram analisados os dados de 9 estudos publicados e 3 em andamento. Uma única sessão de exercício aeróbio em cicloergômetro com diferentes massas musculares, durações e intensidades reduziu a pressão arterial pós-exercício em condições laboratoriais, mas não ambulatoriais. As respostas individuais, no entanto, foram muito variáveis e dependeram de fatores como: o nível inicial da pressão arterial, a pressão arterial diastólica de repouso, a aptidão aeróbia, a intensidade do exercício e sua duração. Quanto ao mecanismo hipotensor, 50% dos normotensos apresentaram redução do débito cardíaco e 50% da resistência vascular periférica. O fator determinante deste mecanismo relacionou-se ao efeito do exercício sobre a pré e pós-cargas cardíacas. A hipotensão pós-exercício associou-se também à redução da atividade nervosa simpática muscular e ao aumento da modulação simpatovagal cardíaca. Após o exercício resistido, observamos hipotensão pós-exercício em normotensos, sendo a queda maior após o exercício menos intenso, e associada à redução do débito cardíaco. Nos hipertensos, a hipotensão pós-exercício resistido também ocorreu e se manteve em condições ambulatoriais. Esta análise permitiu levantar aspectos a serem estudados, como a maior avaliação de hipertensos e a análise de situações que modifiquem o efeito do exercício na pré ou pós-cargas cardíacas

Post-exercise hypotension is characterized by a reduction of blood pressure after a bout of exercise. We are studing this phenomenon for 10 years, and so this thesis aimed to characterize this phenomenon, evaluating its magnitude, duration, factors of influence and mechanisms. Nine published studies and 3 actual ones were analyzed. A single bout of aerobic cycle ergometer exercise, performed with different muscle masses, durations and intensities, produced post-exercise hypotension in the laboratory, but not during daily activities. However, individual responses varied a lot, and this variation was due to pre-exercise blood pressure leveis, rest diastolic blood pressure, aerobic fitness, exercise intensity, and exercise duration. Considering the hemodynamic mechanism, 50% of the normotensives presented a reduction in cardiac output, and 50% in peripheral vascular resistance. This variation was due to the effects of exercise on pre and post cardiac loads. Postexercise hypotesion was also associated to a decrease in muscle sympathetic nerve activity, and an increase in cardiac sympathovagal modulation. After resistance exercise, normotensives presented post-exercise hypotension, which was greater after low intensity exercise, and was due to a reduction in cardiac output. In hypertensives, post-resistance exercise hypotension also occured, and it was sustained under daily living activities.This analysis elicited aspects to be investigated, like the expansion of evaluations in hypertensives, and the introduction of situations that might influence exercise-induced changes in pre and post cardiac loads.
