Os hipertensos resistentes estão especialmente em risco de evoluir com lesões de órgão-alvo e eventos cardiovasculares. Dentre as possibilidades de avaliação destes indivíduos, uma metodologia ecocardiográfica avançada chamada trabalho miocárdico (myocardial work-MW) visa analisar a alça pressão deformação do ventrículo esquerdo. Nosso estudo objetivou descrever o comportamento dos índices do MW no grupo de hipertensos resistentes (HR), hipertensos controlados (HC) e normotensos (N). Foi realizado monitorização ambulatorial da pressão arterial (MAPA) de 71 pacientes, que foram categorizados em três grupos após consulta clínica: HR (em uso de quatro ou mais anti-hipertensivos, independentemente do controle da pressão); HC (hipertensos em uso de até duas medicações e MAPA dentro da meta); N (MAPA normal sem uso de medicações). A análise ecocardiográfica dos indivíduos foi realizada utilizando aparelhos Vivid E95 GE, sendo a pressão arterial aferida no momento do exame, para posterior cálculo dos índices do MW. O grupo HR apresentou menor eficiência miocárdica global (GWE, Média = 95%; p = 0,005) e maior trabalho miocárdico desperdiçado (GWW, Média = 114 mmHg%; p = 0,011). Houve correlação inversa entre a massa ventricular esquerda 3D e do estresse sistólico de parede com o GWE. Entre o grupo HC e os N não houve diferença entre as variáveis do MW.

Resistant hypertensive patients are especially at risk of disease progression, target organ injuries and cardiovascular events. Among the possibilities of evaluating those individuals, an advanced echocardiographic methodology called myocardial work (myocardial work-MW) aims to analyze a pressure loop deformation of the left ventricle. Our study aimed to describe the behavior of MW indices in patients belonging to the resistant hypertensive group (HR), as well as in controlled hypertensive (HC) and normotensive (N) patients. Ambulatory blood pressure monitoring (ABPM) was performed on 71 patients, that were categorized into three groups after clinical consultation: HR (individuals using four or more anti-hypertensive drugs, independently of pressure control); HC (hypertensive patients using up to two medications and with ABPM within the target); N (normal ABPM, without the use of medications). An echocardiographic analysis of the individuals was performed using Vivid E95 GE equipment, and blood pressure was measured at the time of the examination for the calculation of MW indices. HR patients have shown a decrease on myocardial efficiency (GWE, Mean = 95%; p = 0.005) and greater wasted myocardial work (GWW, Mean = 114 mmHg%; p = 0.011. There was an inverse correlation between 3D left ventricular mass and systolic wall stress with GWE. There were no differences between HC and N patients.
