Abstract in English
New onset postoperative atrial fibrillation (POAF) of cardiac surgery is associated with an increase in patients complications, length of hospital stay and mortality, therefore, POAF could influence quality of life (QoL). The aims of this study were to evaluate the impact on the quality of life of POAF after coronary artery bypass grafting (CABG), as well as to identify predictive clinical, echocardiographic and perioperative factors of POAF; to evaluate outcomes related to POAF, such as length of hospital stay, in-hospital mortality and one-year survival. As for the methods, pacients undergoing isolated CABG in a tertiary cardiology center, from July 2016 to July 2017, divided into two groups, with and without POAF. This study was performed with information obtained from a database of the institution, thus, a retrospective analysis of a database that was built from a prospective cohort. Quality of life assesment was made using the Quality of Life in Cardiovascular Surgery (QLCS) questionnaire, applied 01 month, 06 and 12 months after CABG. The results show a total of 418 patients, 341 (81.6%) without POAF and 77 (18.4%) with POAF. Patients with POAF were older than those without, mean age 62.16 (SD ± 9.06) and 66.04 (SD ± 7.35) years in the groups without and with POAF, respectively (P = 0.001), age was an independent risk factor for POAF (OR 1.061; 95% CI: 1.026 - 1.097; P = 0.001). Peripheral arterial disease (PAD) was also an independent risk factor for POAF (OR 3.50; 95% CI: 1.236 - 9.931; P = 0.018). There was an increase in QoL questionnaire scores in both groups over time, but with no statistical difference in QoL between the two groups, mean QLCS scores at 01, 06 and 12 months in the group without POAF 17.64 (± 3.77), 18.91 (± 3.36), 19.57 (± 3.28) and with POAF 17.65 (± 4.16), 18.24 (± 3.45), 19.11 (± 3 , 81) (P interaction = 0.208, P group = 0.440, P time <0.001). Total length of stay was longer in POAF group, with a mean difference of 3 days (P = 0.001). There were no statistical significant difference between in-hospital and one-year mortality (P = 0.625; P = 0.758). We concluded that age and PAD were independent risk factors for POAF. POAF is associated with greater length of hospital stay. This study showed no difference in QL between patients with and without POAF after CABG, during the follow-up of 01 month, 6 and 12 months, thus the two groups behave similarly, with improvement in quality of life over time.
