A doença arterial coronariana (DAC) é a principal causa de morte no Brasil. Dentre os fatores de risco, a hiperhomocisteinemia representa um fator isolado que pode interagir com polimorfismos genéticos potencializando seu risco. No presente estudo avaliou-se em três grupos distintos a associação entre hiperhomocisteinemia e os polimorfismos C677T MTHFR e G1691 A no fator V sobre o desenvolvimento de DAC de início precoce. A homocisteína plasmática estava significantemente aumentada nos pacientes jovens quando comparada os controles (7,3 μM vs 4,2μM P <0,0001). Não variando em função dos genótipos C677T MTHFR (P = 0,27). A freqüência do polimorfismo C677T MTHFR foi similar nos controles e pacientes com DAC precoce. O homozigoto 1691M para o fator V estava presente em 5% dos pacientes. Em conclusão aos nossos dados a homocisteína plasmática está associada com o desenvolvimento de DAC, mas o efeito dos polimorfismos é aparentemente limitado.

The coronary arterial disease (CAD) is the main cause of death in Brazil. Among the risk factors the hyperhomocysteinemia represents an isolated factor, wich can interact with genetic polymorphism potencialing its risk. In the present study we evaluated in three distinct groups the association between hyperhomocysteinemia and polymorphisms C677T MTHFR and G1691A in factor V on the development of DAC of precocious beginning. The homocysteine plasmatic was significantly magnified in the young patients when compared with the controls (7,3 μM versus 4,2 μM P< 0.0001), not varying in function of genotype C677T MTHFR ( P = 0,27). The frequency of polymorphism C677T MTHFR was similar in the controls and patients with precocious CAD. The homozygote 1691M for factor V was present in 5% of the patients witch infarction precocious. IN CONCLUSION TO OUR DATA the homocysteine plasmatic is associated with the CAD development, but the effect of polymorphisms apparently is limited.
