A obesidade constitui um problema crescente de saúde pública em nível mundial, resultando em incremento das taxas de morbimortalidade de doenças crônicas não transmissíveis. Considerando-se a ausência de revisões sistemáticas de estudos de intervenção para perda de peso corporal baseadas em promoção de alimentação saudável e atividade física, a presente pesquisa teve como objetivo realizar uma revisão sistemática para analisar quantidade e qualidade das evidências existentes quanto à efetividade e à adesão de pacientes a estratégias de redução de peso baseadas em aconselhamento nutricional e/ou prática de atividade física em grupos populacionais compostos por indivíduos adultos de diferentes países. A busca sistemática de literatura foi realizada nas bases de dados eletrônicas Scielo, Scholar Google, PubMed e Lilacs, analisando-se estudos publicados em português ou inglês no período de 2010 a 2016, sendo incluídos somente trabalhos baseados em estudos de intervenção randomizados, estudos clínicos ou revisões sistemáticas e metanálises. A revisão sistemática resultou em 24 estudos, que foram analisados em termos de efetividade das intervenções para perda de peso e nível de adesão dos pacientes. Verificou-se que, em geral, as intervenções apresentaram efetividade moderada em termos de perda de peso dos pacientes em comparação com grupos de controle, especialmente em vista do alto nível de desistência de pacientes no caso de intervenções sem apoio de terapia de grupo. A maioria dos estudos identificados na revisão de literatura apresentou alta qualidade das evidências. Em conclusão, observou-se necessidade de organização de intervenções de perda de peso pautadas em equipe multidisciplinar de profissionais de saúde, incluindo-se apoio social por meio de terapia de grupo para aumento da adesão e da efetividade das ações de promoção de alimentação saudável e prática de atividade física.

Obesity is an increasing problem in public health worldwide, resulting in growing morbimortality rates associated with noncommunicable chronic diseases. Considering the absence of systematic review of literature regarding interventions for weight loss based on the promotion of healthy diet and physical activity, the objective of the present research was to perform a systematic literature review to analyze quantity and quality of evidences referring to effectiveness and adherence of patients to strategies of weight loss based on nutritional counselling and/or physical activity in populational groups comprised by adult individuals in diverse countries. The literature search for systematic review was conducted in the electronic databases Scielo, Scholar Google, PubMed and Lilacs, including studies published in Portuguese or English from 2010 until 2016, only based on randomized interventions, clinical trials or systematic reviews and meta-analyses. The systematic review obtained 24 studies, which were analyzed in relation to effectiveness of the interventions for weight loss and level of adherence of patients. In general, interventions presented moderate effectiveness in patients' weight loss in comparison to control groups, especially due to high level of desistance of patients in cases of interventions without group therapy support. The majority of studies identified in the review presented high quality of evidences. In conclusion, there is need for organization of weight loss interventions based on multidisciplinary team of health professionals, including social support through group therapy to increase the adherence and the effectiveness of actions for promotion of healthy diet and physical activity.
