A mucosite oral (MO) é uma lesão frequente em pacientes oncológicos e que pode ser causada tanto por radioterapia em região de cabeça e pescoço ou por certos quimioterápicos. Podendo variar no grau clínico de sua manifestação, a MO quando causa ulcerações gera severo quando álgico, risco de infecção local e sistêmica e aumento de custo no tratamento, principalmente quando causada por radiação ionizante. Não há um consenso terapêutico quanto ao manejo da MO e, nas últimas décadas, há um destaque para a fotobiomodulação (PBM), com laser de baixa intensidade, nos comprimentos de onda vermelho e infravermelho. Esta é uma opção não medicamentosa capaz de diminuir o desconforto e acelerar a cicatrização das feridas. Os mecanismos de ação da PBM na evolução do reparo da MO não são esclarecidos. Visando elucidar a atuação da PBM na evolução da maturidade epitelial em úlceras de MO induzidas por radiação ionizante, o presente estudo avaliou o efeito da PBM com comprimento de onda (λ) vermelho, de 660 nanometros (nm) e infravermelho, de 780 nm em feridas de MO radioinduzidas, na língua de ratos, oito e vinte dias após irradiação, em tempo único, com 20 Gy, de fonte de raios gama. Avaliou-se, na área epitelial das lesões, a área percentual correspondente a marcação positiva para as proteínas citoqueratinas 10 (CK10) e 14 (CK14), por meio de técnica de imunohistoquímica (IHQ), após 8 e 20 dias da indução das lesões, e comparou-se com um grupo controle não tratado. A CK10 é uma proteína relacionada a células epiteliais de maior grau de maturidade e diferenciação, sendo capazes de colaborar com a resposta de defesa inata tecidual, e foi significativamente mais expressa no grupo tratado com PBM de 660 nm. A CK14, que é característica de células epiteliais de menor maturidade, menos diferenciadas, importante para o reparo e resistência mecânica epitelial e, normalmente, devendo estar restrita à camada basal de epitélios saudáveis, não apresentou diferenças quantitativas entre os grupos avaliados. No entanto, seu padrão de distribuição no epitélio das amostras do grupo controle no vigésimo dia, ainda se apresentava alterado, ao passo que nos grupos tratados com PBM, a CK14 já aparecia restrita à camada basal do epitélio, como esperado em epitélios saudáveis. O estudo permitiu concluir que a PBM foi capaz de melhorar indicadores de maturidade epitelial da ferida de MO reparada, o que potencialmente se traduz em um epitélio funcionalmente mais competente para um mesmo tempo clínico do grupo controle, na qual as lesões seguiram seu curso natural de cicatrização.

Oral mucositis (OM) is a frequent lesion in cancer patients and can be caused either by radiotherapy in the head and neck region or by certain chemotherapy agents. It may vary in the clinical degree of its manifestation and, when OM causes ulcerations, it generates severe pain, risk of local and systemic infection and increased treatment cost, especially when caused by ionizing radiation. There is no therapeutic consensus regarding the management of OM and, in recent decades, there has been an emphasis on photobiomodulation (PBM), with low-level laser, in red and infrared wavelengths. This is a non-drug option capable of decreasing discomfort and accelerating wound healing. The mechanisms of action of PBM in the evolution of OM repair are unclear. Aiming to elucidate the role of PBM in the evolution of epithelial maturity in OM ulcers induced by ionizing radiation, the present study evaluated the effect of PBM with red (λ) wavelength, of 660 nanometers (nm) and infrared, of 780 nm in radio-induced OM wounds on the tongue of rats, eight and twenty days after irradiation, in a single time, with 20 Gy, from a gamma ray source. The percentage area corresponding to positive staining for cytokeratin 10 (CK10) and 14 (CK14) proteins was evaluated in the epithelial area of the lesions, using an immunohistochemical technique (IHC), 8 and 20 days after the induction of lesions, and compared with an untreated control group. CK10 is a protein related to epithelial cells with a higher degree of maturity and differentiation, being able to collaborate with the innate tissue defense response, and was significantly more expressed in the group treated with 660 nm PBM. CK14, which is characteristic of less mature, less differentiated epithelial cells, important for epithelial repair and mechanical strength and, normally, should be restricted to the basal layer of healthy epithelia, did not show quantitative differences between the groups evaluated. However, its distribution pattern in the epithelium of the samples from the control group on the twentieth day was still altered, whereas in the groups treated with PBM, CK14 was already restricted to the basal layer of the epithelium, as expected in healthy epithelia. The study allowed us to conclude that PBM was able to improve indicators of epithelial maturity of the repaired OM wound, which potentially translates into a functionally more competent epithelium for the same clinical time of the control group, in which the lesions followed their natural course of healing.
