A Radioterapia é uma das diversas formas de terapia antineoplásica, cujo agente terapêutico é a radiação ionizante, que tem como alvo específico a molécula de DNA, sendo seu maior objetivo a destruição das células neoplásicas. Apesar do grande sucesso da radioterapia, ainda assim é possível o aparecimento precoce e/ou tardio de danos estruturais em células adjacentes. Tendo em vista a possibilidade de complicações auditivas em pacientes submetidos a tratamento radioterápico, este estudo teve por objetivo investigar o efeito causal-radiação no aparelho auditivo de pacientes portadores de câncer de cabeça e pescoço. Além de descrever a incidência de alterações auditivas, para estabelecer rotinas no monitoramento da audição dos pacientes. Este trabalho consiste no estudo prospectivo, descritivo, exploratório e in loco, de possíveis alterações audiológicas de pacientes submetidos a radioterapia para tratamento de câncer de cabeça e pescoço do serviço de oncologia do Hospital Regional de Araguaína (HRA), em parceria com a Clínica de Otorrinolaringologia (COL) privada, especializada em otorrinolaringologia. A partir das análises realizadas, observou-se que a maioria das alterações audiológicas acometem mais indivíduos homens e na faixa etária entre 50 e 70 anos de idade. Observou-se que a radioterapia 2D convencional foi a mais utilizada nos pacientes. Sendo que 100% destes apresentaram radiodermite associada a mucosite após a terapia. Quanto à perda auditiva, observa-se que ao longo do tempo os pacientes tiveram uma perda auditiva de até 70%, sendo o grau moderado o mais predominante, tendo uma frequência de 80% em ambas as orelhas. Quanto ao tipo de curva timpanométrica, observou-se que a curva do tipo A foi sempre a mais predominante após todos os 3 exames, sendo que as curvas do tipo As, B e C tiveram uma frequência aumentada até a realização do exame 3. A ausência dos reflexos acústicos aumentou em até 60% dos pacientes até o terceiro exame. As emissões otoacústicas evocadas foram na maioria das vezes ausentes nos indivíduos analisados, porém quando observada foi mais presente na orelha direita. A frequência das perdas auditivas foi sempre maior nos intervalos de frequências entre 2khz a 8khz, acontecendo sempre em mais de 50% das vezes. Quanto ao limiar de detecção de fala, após o exame 3, foram mais frequentes no intervalo entre 25 e 65, e em ambas orelhas. Após avaliação da intensidade dos potenciais evocados, a intensidade de 120dB SPL/NPS foi a mais predominante, ocorrendo em 100% das vezes após o exame 3. Na maioria das vezes a diferença interaural estava dentro dos limites de normalidade. Após o exame 1, houveram sinais sugestivos de perda auditiva, que ocorreram em uma frequência menor que 20% dos pacientes, afetando na maioria das vezes ambas as orelhas, portanto conclui-se que a maioria dos pacientes não apresentaram alterações audiológicas severas.

Radiotherapy is one of several forms of antineoplastic therapy, the therapeutic agent of which is ionizing radiation, which specifically targets the DNA molecule, with the greatest objective being the destruction of neoplastic cells. Despite the great success of radiotherapy, it is still possible the early and / or late appearance of structural damage in adjacent cells. In view of the possibility of hearing complications in patients undergoing radiotherapy treatment, this study aimed to investigate the causal-radiation effect on the hearing system of patients with head and neck cancer. In addition to describing the incidence of hearing disorders, to establish routines in monitoring patients' hearing. This work consists of a prospective, descriptive, exploratory and on-site study of possible audiological changes in patients undergoing radiation therapy for the treatment of head and neck cancer at the oncology service of the Hospital Regional de Araguaína (HRA), in partnership with the Clínica de Araguaína. Private Otorhinolaryngology (COL), specialized in Otorhinolaryngology. From the analyzes carried out, it was observed that the majority of audiological alterations affect more male individuals and in the age group between 50 and 70 years of age. It was observed that conventional 2D radiotherapy was the most used in patients. 100% of them presented radiodermatitis associated with mucositis after therapy. As for hearing loss, it is observed that over time, patients had a hearing loss of up to 70%, with the moderate degree being the most prevalent, with a frequency of 80% in both ears. As for the type of tympanometric curve, it was observed that the type A curve was always the most prevalent after all 3 exams, with the As, B and C type curves having an increased frequency until the exam 3. An absence of acoustic reflexes increased in up to 60% of patients until the third exam. Evoked otoacoustic emissions were mostly absent in the individuals analyzed, but when observed it was more present in the right ear. The frequency of hearing loss was always higher in the frequency ranges between 2khz to 8khz, always happening more than 50% of the time. As for the speech detection threshold, after exam 3, they were more frequent in the range between 25 and 65, and in both ears. After assessing the intensity of the evoked potentials, the intensity of 120dB SPL / NPS was the most prevalent, occurring 100% of the time after the exam 3. Most of the time the interaural difference was within normal limits. After examination 1, there were signs suggestive of hearing loss, which occurred less than 20% of patients, most often affecting both ears, therefore, it is concluded that the majority of the patients did not present severe audiological alterations.
