Este estudo analisou a validade da estimulação ultra-sônica de baixa intensidade na reparação tecidual em úlceras vasculares através de estudos anatomo-patológicos com biópsias colhidas da borda e fundo das úlceras, antes e depois da estimulação ultra-sônica. Trabalhos experimentais utilizando a aplicação do ultra-som terapêutico têm sido extensivamente usados desde 1955, para uma variedade de condições traumáticas, músculo-esqueléticas, articulares, incluindo osteoartrites, periartrites, bursites e tenosinovites. Recentemente, aplicações adicionais como a aceleração da reparação tecidual e cicatrização das feridas têm sido relatadas na literatura. Com base nos estudos descritos anteriormente e em estudos da aplicação do ultra-som de baixa intensidade para a reparação tecidual de pacientes portadores de úlceras vasculares, iniciou-se a realização desse trabalho no setor de curativos do Posto de Saúde de Franca. Nessa pesquisa participaram 28 pacientes voluntários encaminhados com diagnóstico de úlceras flebopáticas. Estes pacientes inicialmente foram submetidos ao tratamento de ultra-som pulsado de baixa intensidade, com um protocolo previamente estabelecido. A freqüência utilizada foi de 1 KHz e intensidade 25mW/cm2. Para comprovação da ação reparadora do ultra-som foram feitos dois estudos: estudo microscópico e estudo macroscópico. O estudo microscópico consiste da análise histopatológica dos tecidos excisados do leito da úlcera através da técnica "punch". A excisão foi feita por um médico dermatologista com o consentimento dos pacientes antes, durante e depois da estimulação ultra-sônica. Para a aquisição das fotomicrografias foi utilizada uma câmera Olympus DP10, acoplada a um microscópio Olympus DX60. O estudo macroscópico foi feito com o uso de técnicas de processamento de imagens. Para isto foi utilizado um sistema de aquisição e processamento de imagens (software) desenvolvido no Departamento de Engenharia Elétrica, no Laboratório de Microprocessadores da EESC-USP, que analisou características da evolução das úlceras fornecendo como resultados gráficos dessas mesmas características, ao longo do tempo de tratamento. As imagens foram anteriormente capturadas a cada duas semanas e armazenadas. Foram estabelecidos protocolos de aplicação do ultra-som de 4, 8 e 12 semanas de acordo com a área das úlceras. Os resultados obtidos da análise macroscópica revelaram a redução da área das úlceras. Esses achados coincidiram com as características observadas no processo de reparação tecidual através do sistema de monitoramento com o uso de visão computacional. O exame histopatológico de casos correspondentes mostrou em alguns casos a neoformação de vasos e o aumento de fibroblastos. Os resultados obtidos neste trabalho comprovam que o tratamento com o ultra-som pulsado de baixa intensidade agiu significativamente na redução das úlceras, promovendo a aceleração da reparação tecidual e a cicatrização das mesmas.

This study analysed the validity of low intensity, ultrasound stimulation in the tissue healing among vascular ulcers through anatomic pathological studies with biopsies collected from the edge and middle of the ulcers, before and after the ultrasound stimulation. Experimental work utilizing therapeutic ultrasound applications have been used since 1955 for a variety of trauma, muscular-skeletal and joint conditions including osteoarthritis, periarthritis, bursitis and fenosinovites. Recently results of additional applications such as the acceleration of tissue repair and healing in wounds has been reported in the literature available. Based on the aforementioned studies, and on further studies of low intensity, ultrasound applications for tissue repair among patients suffering from vascular ulcers, the present work was initiated within the wound dressirig sector located in the Public Health Center of the City of Franca, SP, Brazil. Twenty-eight volunteer patients diagnosed with phlebopathic ulcers participated in this research. To prove the healing action of ultrasound, two studies were made: a rnicroscopic study and a macroscopic study. The microscopic study consisted of a histopathological analysis of tissues cut from the base of the ulcer applying the punch technique. The cut was made by a dermatologist with the patients' consent, before, during and after the ultrasound stimulation. An Olyrnpus DP10 camera coupled to an Olympus DX60 rnicroscope was used in taking the photomicrographs. The macroscopic study was made using image processing techniques. This was carried out using a system of capturing and processing images (software) developed by the Department of Electrical Engineering's Microprocessor Laboratory at EESC-USP (University of São Paulo), which analysed the characteristics of the evolution of the ulcers supplying graphs of these characteristics during the whole treatment. The images were taken every two weeks and the stored away. Registers of the ultrasound applications were established at 4, 8 and 12 weeks according to the area of the ulcers. The results obtained from the macroscopic analysis revealed a reduction in the area o f the ulcers. They coincided with the observed characteristics within the process of the observed tissue repair through the Monitoring System which uses a computational view point. The histopathological exam showed in some cases a neo-formation of veins and the increase of fibroblasts. The results obtained in this work proved that the treatment with ultrasound pulsed at low intensity acted significantly on the reduction of the ulcers, promoting the acceleration of tissue repair and healing.
