Introdução: O cuidado pré-concepcional tem impactos positivos nos indicadores de saúde materna e infantil pelo fato de possibilitar que mulheres e casais vivenciem uma gravidez de forma mais saudável. Entretanto, é pouco ofertado nos serviços de atenção primária, sendo o baixo nível de conhecimento dos trabalhadores de saúde sobre o tema um dos obstáculos para sua efetiva implementação. Objetivo: Avaliar o efeito de uma intervenção educativa no conhecimento, nas atitudes e na realização de ações relacionadas ao cuidado pré-concepcional e compreender a forma como o cuidado pré- concepcional é e pode ser ofertado por trabalhadores de saúde da atenção primária. Método: Trata-se de uma pesquisa de métodos mistos, com abordagem quanti e qualitativa. A abordagem quantitativa constituiu-se de um ensaio comunitário randomizado conduzido em oito unidades básicas de saúde da zona leste da cidade de São Paulo (quatro unidades compuseram o grupo intervenção e quatro o grupo controle). A intervenção constitui-se de uma ação educativa destinada aos médicos e enfermeiros das equipes de saúde da família, com duração de uma hora, que abordou os principais tópicos da saúde pré-concepcional. Os trabalhadores preencheram instrumentos estruturados em três momentos: antes da intervenção, um mês e três meses após a intervenção. Além disso, mulheres em idade fértil, usuárias das unidades participantes, foram aleatoriamente selecionadas para responder a respeito de ações de cuidado pré-concepcional que porventura tenham recebido/participado naquela unidade; por fim, os farmacêuticos de cada unidade responderam quantos comprimidos e/ou frascos de ácido fólico foram dispensados pela farmácia local. A abordagem qualitativa constituiu-se de grupos focais com os trabalhadores do grupo intervenção. Foi conduzida uma investigação narrativa que envolveu os relatos sobre as experiências vividas. Resultados: A intervenção educativa aumentou o nível de conhecimento sobre o cuidado pré-concepcional entre os trabalhadores da saúde, mas não teve efeito em suas atitudes nem na realização de ações relacionadas ao cuidado pré-concepcional, com exceção do aumento do questionamento da intenção de engravidar, evidenciado nos relatos das mulheres usuárias das UBS. A oferta do cuidado pré-concepcional parece ser realizada de maneira indireta e com fragilidades, como a falta de agenda destinada a esse cuidado, existência de protocolos inadequados para sua oferta, pouca participação das mulheres em grupos educativos, vergonha de algumas mulheres em questionar sobre como preparar-se para uma gravidez, receio de julgamento por parte dos trabalhadores de saúde, dentre outras. As ações do cuidado pré-concepcional ofertadas são basicamente de cunho biológico, desconsiderado os aspectos psicológicos e sociais do cuidado pré-concepcional. Os trabalhadores de saúde identificaram o momento de coleta do exame preventivo de câncer de colo do útero, os grupos educativos e a participação dos agentes comunitários de saúde como estratégias para a promoção da saúde pré-concepcional na atenção primária à saúde. Conclusão: Embora o conhecimento dos trabalhadores de saúde e a educação permanente em saúde sejam requisitos fundamentais para a oferta do cuidado pré-concepcional, outros elementos mostraram-se mais importantes para sua implementação, como a necessidade de políticas públicas e programas específicos para fomentar a oferta do cuidado pré-concepcional na atenção primária à saúde, bem como a divulgação desse cuidado para a população geral.

Introduction: The preconceptional care has positive impacts on maternal and child health indicators because it allows women and couples to experience pregnancy in a healthier way. However, it is rarely offered in primary care services, and the low level of knowledge of health workers on the topic is one of the obstacles to achieve the effective implementation. Objective: To evaluate the impact of an educational intervention on knowledge, attitudes, and actions related to preconception care and to understand the way preconception care is and can be offered by primary care health workers. Method: It is a research of mixed methods, with a quantitative and qualitative approach. The quantitative approach consisted of a randomized community trial conducted in eight basic health units in the east side of the city of São Paulo (four units comprised the intervention group and four units the control group). The intervention consists of an educational action aimed at doctors and nurses in family health teams, lasting one hour, which addressed the main topics of preconception health. The workers filled out structured instruments in three moments: before the intervention, one month and three months after the intervention. In addition, women of childbearing age, users of the participating units, were randomly selected to respond to preconception care actions that they may have received / participated in in that unit; finally, the pharmacists in each unit answered how many pills and / or bottles of folic acid were dispensed by the local pharmacy. The qualitative approach consisted of focus groups with the workers in the intervention group. A narrative investigation was conducted that involved the reports about the lived experiences. Result: The educational intervention increased the level of knowledge about the preconception care of health workers, but it had no effect on their attitudes and in carrying out actions related to preconception care, except by the increased questioning about pregnancy according to UBS women users' reports. The provision of preconception care is carried out indirectly and presents some weaknesses, such as the lack of disclosure about preconception care, lack of a scheduled towards it, inadequate protocols for its provision, limited participation by women in educational groups, some women are ashamed to question how to prepare for pregnancy, fear of health workers and health workers judgment and 'disagreement with their patients' reproductive intent. However, health workers identified the moment to collect the cervical cancer preventive exam, the activities of educational groups, and the CHA participation in the care disclosure as initiatives to promote preconception health in primary health care. Conclusion: Knowledge is a fundamental condition to provide preconception care, but other elements are essential for this action to occur, such as the need for public policies and specific programs to foster the provision of preconception care in primary health care, as well as the care dissemination to population.
