Introdução: Evidências internacionais e nacionais apontam que são elevados os percentuais de insatisfação com o peso corporal entre adolescentes (quase 50%). A insatisfação com o peso corporal tem sido associada à adoção de comportamentos de risco à saúde, os quais iniciam ou são reforçados na adolescência, com profundo impacto à saúde futura. Assim, constitui objeto deste estudo a insatisfação dos adolescentes com o peso corporal. Objetivo geral: Avaliar a associação entre insatisfação com o peso corporal e comportamentos de risco à saúde em adolescentes brasileiros. Objetivos específicos: 1º) Sintetizar as melhores evidências científicas disponíveis para determinar a associação entre insatisfação com o peso corporal em adolescentes e comportamentos alimentares não saudáveis e/ou falta de atividade física e, analisar quais as dimensões mais comumente utilizadas para se avaliar insatisfação com o peso corporal; 2º) Avaliar a associação entre as duas formas de insatisfação com o peso corporal (insatisfação com baixo peso e com excesso de peso) e comportamentos de risco à saúde em adolescentes brasileiros, estratificado por sexos; 3º) Avaliar a associação entre as duas formas de insatisfação com o peso corporal (insatisfação com baixo peso e com excesso de peso) e a ocorrência de múltiplos comportamentos de risco à saúde em adolescentes brasileiros, estratificado por sexos. Método: Revisão sistemática, para o primeiro objetivo, e análise da base de dados do Estudo de Riscos Cardiovasculares em Adolescentes (ERICA), para o segundo e o terceiro objetivos. Estudo seccional multicêntrico nacional de base escolar, aprovado pelo Conselho Nacional de Ética em Pesquisa, por Comitês de Ética Local e por Secretarias de Educação. Este estudo avaliou 71.740 adolescentes de ambos os sexos, de 12 a 17 anos, de escolas públicas e privadas. A variável dependente foi insatisfação com o peso corporal (satisfação, insatisfação com baixo peso e insatisfação com excesso de peso), a independente principal foram os comportamentos de risco à saúde (tabagismo, uso de bebidas alcoólicas, comportamentos alimentares não saudáveis e inatividade física). As variáveis independentes utilizadas como ajuste foram sexo, idade, estado nutricional e transtornos mentais comuns do adolescente, além de escolaridade da mãe e tipo de escola do adolescente. A análise dos dados foi processada no software Stata versão 15.0, por meio de análise descritiva, bivariada e regressão logística multinomial simples e múltipla, estratificada pelo sexo do adolescente. O nível de significância de todos os testes foi de 5%. Resultados: 1º) Total de 11 artigos preencheram os critérios de inclusão. Os estudos incluídos avaliaram a insatisfação com o peso corporal utilizando diferentes dimensões: cinco, componente de satisfação da dimensão atitudinal; dois, componente de comportamento da dimensão atitudinal; dois, dimensão perceptiva e dois, a dimensão não era clara. Quanto aos comportamentos: dois avaliaram apenas comportamentos alimentares pouco saudáveis, seis avaliaram a inatividade física e três avaliaram ambos os comportamentos. Apenas três estudos encontraram associação entre insatisfação com o peso corporal e comportamentos alimentares pouco saudáveis, três com falta de atividade física e um não realizou teste estatístico para associação entre insatisfação com o peso corporal e comportamentos alimentares pouco saudáveis e/ou falta de atividade física; 2º) 45,0% dos adolescentes (36,4% masculino e 53,8% feminino) relataram insatisfação com o peso corporal, em especial (30,3%) a forma insatisfação com excesso de peso (21,5% masculino e 39,3% feminino). No modelo final da regressão múltipla, os hábitos alimentares foram associados às duas formas de insatisfação corporal entre adolescentes do sexo masculino, enquanto o uso de álcool e hábitos alimentares associaram-se às duas formas de insatisfação corporal entre adolescentes do sexo feminino; e 3º) 22,5% dos adolescentes (16,8% masculino e 28,3% feminino) relataram adoção de dois ou mais comportamentos de risco à saúde. Entre os adolescentes do sexo masculino, a adoção de dois ou mais comportamentos de risco à saúde foi associada à insatisfação com o excesso de peso. Entre as adolescentes do sexo feminino, a adoção de dois ou mais comportamentos de risco à saúde foi associada a ambas as formas de insatisfação com o peso corporal (com baixo peso e excesso de peso). Conclusões: Os achados da presente investigação destacam a relevância da temática insatisfação com o peso corporal em adolescentes brasileiros, uma vez que quase metade da população apresentava alguma forma de insatisfação (insatisfação com baixo peso e com excesso de peso), além de constatar associação com comportamentos de risco à saúde. Espera-se que os resultados desta tese subsidiem a elaboração/revisão de políticas públicas de saúde direcionadas aos adolescentes que incorporem a discussão da temática insatisfação com o peso corporal, bem como sua associação com comportamentos de risco à saúde.

Introduction: International and national evidence shows high percentage of body weight dissatisfaction among adolescents (almost 50%). Body weight dissatisfaction has been associated with the adoption of health-risk behaviors, which begin or are reinforced in adolescence, with profound impact on future health. Thus, the object of this study is the adolescents' body weight dissatisfaction. Objective: To evaluate the association between body weight dissatisfaction and health risk behaviors among Brazilian adolescents. Specific objectives: 1) Synthesize the best available scientific evidence to determine the association between body weight dissatisfaction in adolescents and unhealthy eating behaviors and/or lack of physical activity, and to analyze the most commonly used dimensions to assess body weight dissatisfaction; 2) To evaluate the association between the two forms of body weight dissatisfaction (dissatisfaction with underweight and overweight) and health-risk behaviors in Brazilian adolescents, according to gender; 3) To evaluate the association between the two forms of body weight dissatisfaction (dissatisfaction with underweight and overweight) and the occurrence of multiple health-risk behaviors in Brazilian adolescents, according to gender. Method: Systematic review, for the first objective, and analysis of the dataset of the study of Cardiovascular Risks in Adolescents (ERICA), for the second and the third objectives. This is a cross-sectional multicenter national school-based study, approved by the National ethics committee, Local ethics committee and Education Departments ethics committee. This study evaluated 71,740 adolescents of both sexes, from 12 to 17 years old, from public and private schools. The dependent variable was body weight dissatisfaction (satisfaction, dissatisfaction with being underweight and dissatisfaction with being overweight), and the main independent variables were health-risk behaviors (smoking, alcohol use, unhealthy eating behaviors and physical inactivity). The independent variables used as adjustment were sex, age, weight status and common mental disorders of the adolescent, as well as the mothers schooling and school type of the adolescent. The data analysis was processed in Stata software version 15.0, using descriptive, bivariate, simple and multiple multinomial logistic regression, according to gender of the adolescent. The significance level of all tests was 5%. Results: 1º) A total of 11 articles met the inclusion criteria. The included studies assessed body weight dissatisfaction using different dimensions: five, satisfaction component of the attitudinal dimension; two, behavior component of the attitudinal dimension; two, perceptual dimension and two, the dimension was unclear. The behaviors: two assessed only unhealthy eating behaviors, six assessed lack of physical activity and three assessed both behaviors. Only three studies found an association between body weight dissatisfaction and unhealthy eating behaviors, three with lack of physical activity, and one did not perform a statistical test for an association between body weight dissatisfaction and the unhealthy eating behaviors and/or lack of physical activity; 2º) 45.0% of the adolescents (36.4% male and 53.8% female) reported body weight dissatisfaction, especially (30.3%) the form of dissatisfaction with being overweight (21.5% male and 39.3% female). In the final model of the multiple regression, eatings habits were associated with the two forms of body weight dissatisfaction among male adolescents, while alcohol use and eating habits were associated with the two forms of body weight dissatisfaction among female adolescents; 3º) 22.5% (16.8% male and 28.3% female) of adolescents have adopted two or more health-risk behaviors. Among male adolescents the adoption of two or more health-risk behaviors was associated with body weight dissatisfaction with being overweight. Among female adolescents the adoption of two or more health-risk behaviors were associated with both forms of body weight dissatisfaction (with being underweight and with being overweight). Conclusions: The findings of the present study highlight the relevance of the issue of body weight dissatisfaction in Brazilian adolescents, since almost half of the population had some form of body weight dissatisfaction (dissatisfaction with being underweight and with being overweight), besides was showed association with body weight dissatisfaction and health-risk behaviors. Through the results of this thesis we expect to support the elaboration/review of adolescents public health policies that incorporate the discussion of the issue body weight dissatisfaction, as well as its association with health-risk behaviors.
