Introdução. A dor aguda no peito é uma queixa frequente nos serviços de urgência. A correta caracterização desse evento em curto espaço de tempo é de suma importância para o tratamento adequado de pacientes com Infarto Agudo do Miocárdio (IAM) que se apresentam nos serviços de urgência. As classificações de risco são instrumentos para garantir que os pacientes com maior prioridade tenham atendimento mais rápido. Objetivo. Investigar se a adoção do Sistema Manchester de Classificação de Risco (SMCR em um hospital escola de atenção secundária influenciou o tempo para tratamento farmacológico dos pacientes com IAM no serviço de emergência e, se os diferentes níveis de prioridades determinados pelo SMCR estavam relacionados com esse tempo. Método. Estudo observacional analítico, tipo antes e depois, de análise secundária de dados de 251 pacientes que deram entrada no Pronto Atendimento (PA) do Hospital Universitário da Universidade de São Paulo (HU-USP) com diagnóstico médico de IAM com ou sem elevação do segmento ST, antes (n=132) e depois (n=119) da implementação do SMCR. O SMCR era aplicado apenas no período das 7 às 19 horas, mas todos os pacientes do período pós SMCR foram incluídos nas análises. Foram analisados os intervalos de tempo entre a abertura de ficha de PA e a administração do fármaco específico para o tratamento de IAM (ácido acetilsalicílico (AAS), heparina, clopidogrel e estreptoquinase). Resultados. As amostras de antes e de depois da implementação do SMCR foram consideradas semelhantes. A média do intervalo de tempo entre a abertura de ficha e a primeira administração de AAS registrada em prontuário no período anterior à implementação do SMCR foi de 444,4 1055,1 minutos e de 335,6 563,8 minutos no período posterior ao SMCR, porém sem diferença estatisticamente significante (p=0,636). Quando considerado o intervalo de tempo entre a abertura da ficha e o primeiro registro da administração de qualquer medicamento específico para IAM (AAS, clopidogrel, heparina ou estreptoquinase) foi observada ligeira diminuição dos valores, sem significância estatística (350,8 1020,5 minutos x 261,2 361,5 minutos; p=0,148), enquanto que houve aumento no tempo para início da administração do trombolítico, do período pré SMCR para o pós SMCR (54,5 46,4 minutos x 78,5 45,4 minutos; p=0,047). Os pacientes submetidos ao SMCR (n=53) receberam prioridade adequada em 28,30% dos casos. A adequada alocação foi associada à queda no intervalo de tempo para a administração do tratamento específico (p = 0,003). Conclusões. Entre os pacientes que tiveram o SMCR aplicado, aqueles com priorização adequada tiveram acesso ao tratamento farmacológico mais precocemente. No entanto, não foi observada mudança no tempo para tratamento farmacológico depois da adoção do SMCR e houve aumento do tempo para o tratamento trombolítico no período em que o protocolo era aplicado. Essas conclusões precisam ser tomadas com cautela devido ao pequeno tamanho da amostra.

Background/introduction: Acute chest pain is a frequent complaint in urgent care services. The prompt and correct characterization of this event is crucial to the appropriate treatment of patients with acute myocardial Infarction (AMI). Risk classification systems can potentially improve patient outcomes by prioritizing care based on conditions severity. Objective: The aim of this work was to investigate whether the implementation of the Manchester Triage System (MTS) improved the timing of pharmacological treatment of patients with AMI and if MTS level influenced those times. Methods: An observational, before-and-after study of secondary data analysis of 251 patients admitted to the Emergency Department of the University Hospital of the University of São Paulo (HU-USP) with a medical diagnosis of AMI with or without ST-segment elevation, before (n=132) and after (n= 119) the implementation of MTS. The MTS was only applied from 7am to 7pm, but all patients in the post-MTS period were included in the analyzes. Analyzed data included the duration between patient admission and treatment of AMI (e.g., acetylsalicylic acid, heparin, clopidogrel or streptokinase). Results: Samples before and after the implementation of MTS were found to be similar. Implementation of the MTS had no statistically significant impact on the mean time interval between patient administration and the first administration of acetylsalicylic acid recorded. Although mean time intervals were reduced from 444.4 1055,1 min to 335.6 563,8 min when MTS was implemented, it was found to be not statistically significant (p = 0.636). When considering the time interval between the patient admission and the first record on any drug used to treat AMI (e.g., acetylsalicylic acid, clopidogrel, heparin, or streptokinase), mean time intervals were again reduced from 350,8 1020,5 min to 261,2 361,5 min without statistical significance (p = 0.148), while there was an increase in the time to start of thrombolytic administration (54,5 46,4 minutes x 78,5 45,4 minutes, p = 0.047). Patients admitted to MTS (n= 53) received adequate priority in 28,30% of the cases. Adequate allocation was associated with a decrease in the time interval for administration of the specific treatment (p = 0.003). Conclusions: Among the patients who had the Manchester protocol applied, those with adequate prioritization had access to the specific treatment earlier. However, no evidence of changing in time was observed for the pharmacological treatment for AMI after the implementation of MTS. Furthermore, there was an increase in the time for thrombolytic treatment administration in the period in which the protocol was applied. Although promising, these results need to be taken with caution due to the small sample size.
