O Hospital de Reabilitação de Anomalias Craniofaciais da Universidade de São Paulo (HRAC USP) é referência para cirurgia de alta complexidade em pacientes com deformidades craniofaciais, sendo o hospital com maior volume de pacientes submetidos à queiloplastia e palatoplastia no país. Com o início da pandemia de COVID-19, a rotina do serviço mudou. Esta situação, gerou questões sobre como equilibrar a transmissão viral, visando proteger tanto pacientes quanto as equipes de profissionais e consequentemente diminuir os impactos negativos nos serviços prestados por este hospital. O objetivo deste estudo retrospectivo e observacional foi identificar as consequências desta pandemia no serviço ambulatorial e cirúrgico do HRAC. A amostra foi obtida das tabelas públicas ofertadas pelo site do HRAC com tabulação de dados numéricos de procedimentos cirúrgicos e ambulatoriais realizados nos pacientes dos programas de fissuras labiopalatinas, malformações craniofaciais e saúde auditiva. Foram também avaliados índices anuais de produção acadêmica, quanto ao número de estudantes matriculados em pós-graduação, residências médicas e áreas multidisciplinares em saúde. Encontrou-se queda no número de prontuários abertos, consultas realizadas, cirurgias e internações durante o período da pandemia da COVID-19. Houve também redução em serviços complementares como diagnóstico por imagem, exames complementares e redução do uso de implantes. Ocorreu, porém, uma manutenção do número de alunos matriculados bem como de publicações e produção científicas. Estes dados nos orientam como devem ser desenvolvidas políticas públicas visando melhorar o atendimento e a qualidade de vida dos pacientes atendidos em serviços públicos em situações de agravos de saúde pública

The Hospital for Research and Rehabilitation of Cleft Lip and Palate HPRLLP/USP, highlighted as Center of Excellence in Care and world reference by the World Health Organization is the leading hospital in palatoplasty and cheiloplasty in Brazil. With the onset of the COVID-19 pandemic this scenario was challenged. As other reference centers, the hospital was faced with challenges regarding the balance of virus transmission and maintenance of its service while also protecting patients and healthcare workers from this new disease. The objective of this retrospective and observational study was to identify the consequences of the pandemic in the surgical and outpatient services of HPRLLP/USP. The samples were obtained from public data published on HPRLLP/USP website by sorting numeric data of procedures in patients on the cleft lip and palate, craniofacial deformities e hearing impairment programs in the years ranging from 2016 to 2021. The academic data on the number of articles published in this same period of time was also in the scope of this study. During the pandemic, a decrease in new medical records was found, as well as medical, dental consultations, hospital admissions and surgeries. There was also a decrease in the use of complementary services as medical imaging, laboratory studies and general implants. A steady number of scientific publishing and postgraduate enrollment was found. A better understanding of healthcare limitations and restrictions during pandemics are necessary to develop public health actions that may improve health and quality of life of the patients during healthcare threatening crisis.
