Objetivo: Comparar os índices de sucesso de enxertos alveolares associados aos reposicionamentos cirúrgicos de pré-maxila realizados com osso medular autógeno da crista ilíaca ou proteína morfogenética óssea (rhBMP-2). Material e métodos: Estudo transversal que avaliou uma amostra de radiografias periapicais e oclusais de 28 indivíduos com fissura de lábio e palato completa bilateral (idade média de 12 anos e 11 meses), submetidos consecutivamente ao reposicionamento cirúrgico da pré-maxila com enxerto alveolar divididos igualmente em 2 grupos conforme o material empregado no preenchimento do defeito ósseo: osso da crista ilíaca anterior (GI) ou rhBMP-2 (GBMP). As cirurgias foram realizadas por 3 cirurgiões bucomaxilofaciais, em um único centro de reabilitação. As radiografias foram feitas após 12 meses de pós-operatório. O resultado das cirurgias foi caracterizado em sucesso ou insucesso por 3 avaliadores cegos e independentes que se basearam nos critérios adotados pelas escalas de Bergland modificada e SWAG (The Americleft Standardized Way to Assess Grafts). O teste Kappa verificou as concordâncias intra e inter-avaliadores e entre as escalas. A comparação entre GI e GBMP foi realizada por meio do teste de Fisher (p<0,05). Resultados: Não houve significância entre as idades dos indivíduos do GI e GBMP (p=0,471). As escalas mostraram 96,4% de concordância (K=0,85). O GBMP e GI apresentaram taxas de sucesso semelhantes com as duas escalas. Contudo, apenas a escala de Bergland modificada verificou influência da idade no resultado das cirurgias Conclusão: Os enxertos alveolares associados aos reposicionamentos cirúrgicos de pré-maxila realizados com rhBMP-2 apresentaram resultados semelhantes àqueles realizados com osso esponjoso da crista ilíaca anterior. Independente do material utilizado, as cirurgias realizadas tardiamente tiveram uma prevalência 8 vezes maior de insucesso do que as operadas no tempo ideal.

Objectives: To compare success rates of alveolar grafts associated with surgical premaxillary repositioning performed with autogenous cancellous bone from the anterior iliac crest or bone morphogenetic protein (rhBMP-2). Material and methods: This cross-sectional study evaluated a sample of periapical and occlusal radiographs of 28 individuals with bilateral complete cleft lip and palate (mean age 12 years 11 months), consecutively submitted to surgical premaxillary repositioning with alveolar graft. The individuals were divided into 2 groups according to the material used to fill the bone defect: anterior iliac crest bone (IG) or rhBMP-2 (BMPG), totaling 14 patients per group. Surgeries were performed by 3 maxillofacial surgeons, at a single rehabilitation center. Radiographs were taken 12 months after surgery. Surgical outcomes were characterized as success or failure by 3 independent and blinded examiners based on the modified Bergland and SWAG (The Americleft Standardized Way to Assess Grafts) scales. Intra and inter-examiner and between scales reliabilities were verified by the Kappa test. Comparison between GI and GBMP was performed using Fisher's test (p<0.05). Results: There was no significance between the ages of individuals in the IG and BMPG (p=0.471). The scales showed 96.4% agreement (K=0.85). The BMPG and IG had similar success rates with the two scales. However, only the modified Bergland scale verified the influence of age on the surgical outcome. Conclusion: Alveolar grafts associated with surgical premaxillary repositioning performed with rhBMP-2 showed similar outcomes to those performed with cancellous bone from the anterior iliac crest. Regardless of the material used, surgeries performed late had an 8 times higher prevalence of failure than those operated at the ideal time.
