Introdução: As fissuras labiopalatinas são malformações estabelecidas precocemente nos períodos embrionário e fetal, e ocorrem concomitante ao processo evolutivo do sistema nervoso central. Estudos recentes têm discutido consistentemente a condição gerada pela malformação da face na influência dos processos evolutivos cerebrais. Pesquisas científicas, nas últimas décadas, sobre as anormalidades cerebrais estruturais nessa população, reforçam as evidências quanto à diferenciação funcional nas funções cognitivas, indicando maior vulnerabilidade para as dificuldades na aprendizagem. Objetivo: identificar o perfil neuropsicológico de funções cognitivas percepto-motoras e executivas como fenótipos de crianças com fissura lábiopalatina, numa relação clínico-anátomo-funcional. Método: realizados os procedimentos éticos, o estudo foi organizado em duas etapas: Estudo 1, composto por 40 participantes, que realizaram a avaliação neuropsicológica e o exame de ressonância magnética, compondo o G1, com 20 participantes com Fissura de Lábio e Palato não sindrômica (FLPNS), sendo 11 femininos, com média de idade de 12,6 anos, inscritos num hospital de reabilitação em anomalias craniofaciais, e o G2, sem FLPNS, sendo 10 de cada sexo, com média de idade de 12,6, equivalentes em média de idade e características sociodemográficas ao G1. Estudo 2, composto por 24 participantes, distribuídos em dois subgrupos, compondo o G1.1, com 12 participantes com FLPNS, sendo 7 masculinos, com média de idade de 13 anos; e o G2.1, também 12 participantes sem FLPNS, pareados em idade, gênero e nível socioeconômico ao G1.1. Os instrumentos e recursos utilizados foram as Matrizes Progressivas Coloridas de Raven, Escala Especial e Geral; Escala Wechsler de Inteligência para Criança e Adolescentes (WISC IV), Wisconsin Card Sorting Test, Figuras Complexas de Rey e os Exames de Ressonância Magnética Nuclear. Resultado: No Estudo 1, para os componentes cognitivos avaliados, foram encontrados prejuízos nos recursos intelectuais (p<0,001) e cognitivos (p=0,020), relacionados à capacidade de integrar diferentes habilidades na execução de tarefas, com significância estatística nos domínios de compreensão verbal, memória operacional, velocidade de processamento, organização perceptual. Também foram constatados prejuízos nos subdomínios das funções executivas de atenção sustentada, memória operacional e planejamento cognitivo, porém, com preservação dos subdomínios de controle inibitório e flexibilidade cognitiva (p=0,623 e p=0,644). No Estudo 2, quanto às análises morfológicas, foram encontradas alterações, na espessura cortical menor no G1.1, regiões no hemisfério esquerdo, como a Supramarginal (r=0,036), Parietal superior (r=0,036) e Parietal inferior(r=0,036). No hemisfério direito, nas regiões Occipital lateral (r=0,032), Temporal médio (r=0,032), Lobo temporal (r=0,036), pares orbitais (r=0,036), supramarginal (r=0,036), Temporal superior (r=0,036), Parietal inferior (r=0,042), Frontal médio rostral (r=0,049). Quanto ao volume, foi encontrado na região do núcleo accumbens (r=0,037) e Lobo occipital (r= 0,049). As correlações demonstraram associações de estruturas e funções que integram as redes associativas parieto-temporo-occipital, frontoparietal e Córtex pré-frontal, indicando prejuízos com significância estatística. Conclusão: o estudo forneceu elementos para caracterizar o perfil anátomo-clínico e funcional das crianças e adolescentes com FLPNS que participaram do estudo, bem como, proporcionou ampliação do conhecimento das particularidades do desenvolvimento do fenótipo da FLPNS, como subsídio para, cada vez mais, refutar o prejuízo cognitivo dessa população como consequência secundária, porém, o constructo ainda está em processo de construção e comprovação para novas proposições.

Introduction: clefts of the lip and palate are malformations early established in the embryonic and fetal periods, and concurrently with the evolutionary process of the central nervous system. Nowadays studies, have consistently discussed the condition generated by facial malformation on the influence of brain evolutionary processes. Scientific research in the last decades on structural brain abnormalities in this population reinforces the evidence regarding functional differentiation in cognitive functions, indicating a greater vulnerability to learning difficulties. Objective: to identify the neuropsychological profile of cognitive and perceptual-motor functions as the phenotypes of children with isolated clefts of the lip and palate, in a clinical-anatomo-functional relationship. Method: the ethical procedures were performed, the study was organized in two stages: Study 1, composed of 40 participants, who underwent neuropsychological evaluation and magnetic resonance imaging, composing the G1, with 20 participants with isolated clefts of the lip and palate (ICLP), of which 11 were female, with an average age of 12.6 years old, enrolled in a rehabilitation hospital in craniofacial anomalies, and G2, without ICLP, being 10 of each sex, with an average age of 12.6 years old, equivalent in average age and sociodemographic characteristics to G1. Study 2, composed of 24 participants, divided into two subgroups, comprising G1.1, with 12 participants with ICLP, being 7 males, with an average age of 13 years old; and G2.1, also being 12 participants without ICLP, of the same age, gender and socioeconomic status to G1.1. The instruments and resources used were Raven's Progressive Matrices, Special and General Scale; Wechsler Scale of Intelligence for Children and Adolescents (WISC IV), Wisconsin Card Sorting Test, Rey Complex Figures and Nuclear Magnetic Resonance Examinations. Results: in Study 1, impairments in intellectual (p <0.001) and cognitive (r = 0.020) resources were found related to the ability to integrate different skills in the execution of tasks, with statistical significance in the domains of comprehension verbal, operational memory, processing speed, perceptual organization. We also observed impairments in the subdomains of the executive functions of sustained attention, working memory and cognitive planning, however, with preservation of the subdomains of inhibitory control and cognitive flexibility (r = 0.623 and r= 0.644). In study 2, morphological analyzes showed alterations in the lower cortical thickness in G1.1, regions in the left hemisphere, such as Supramarginal (r = 0.036), Upper Parietal (r = 0.036) and Lower Parietal (r = 0.036). In the right hemisphere, in the lateral occipital regions (r = 0.032), mean temporal (r = 0.032), temporal lobes (r = 0.036), orbital pairs (r=0.036), supramarginal (r= 0.036), Lower parietal (r = 0.042), Mean rostral frontal (r=0.049). As for volume, it was found in the region of the nucleus accumbens (r = 0.037) and occipital Lobes (r = 0.049). The correlations showed associations of structures and functions that integrate the parietal-temporal-occipital associative networks, frontoparietal and prefrontal cortex, indicating damages with statistical significance. Conclusion: the study provided elements to characterize the anatomo-clinical and functional profile of children and adolescents with ICLP who participated in the study, as well as, provided an increase in the knowledge of the peculiarities of the development of ICLP phenotype, as a subsidy for refuting the cognitive impairment of this population as a secondary consequence, however, the construct is still in process of construction and confirmation for new propositions.
