As cirurgias primárias interferem em graus variados sobre o crescimento e desenvolvimento do complexo maxilar de indivíduos com fissura labiopalatina (FLP). O objetivo do estudo foi avaliar a influência da palatoplastia em dois estágios 2Stg (1º estágio: Queiloplastia + Asa Nasal + Palato duro com retalho de vômer; 2º estágio: Palato mole) no desenvolvimento maxilar de crianças com fissura labiopalatina unilateral (FLPU), e sem intervenção ortodôntica prévia. O grupo foi comparado a um grupo controle de crianças submetidas a palatoplastia em um estágio 1Stg, com fechamento simultâneo do palato duro e mole. Ambos os protocolos se referem ao protocolo atual e antigo do Hospital de Reabilitação de Anomalias Craniofaciais (HRAC) USP. A influência da técnica, expertise do cirurgião plástico operante e cronologia de fechamento do lábio, palato duro e palato mole foram verificados e comparados. 349 modelos de gesso de pacientes em fase da dentadura decídua completa ou mista precoce (2Stg: 169; 1Stg: 180) foram avaliados e categorizados o grau de severidade da discrepância maxilo mandibular por meio do Índice Oclusal dos 5 anos por três ortodontistas. O grau de reprodutibilidade intra avaliadores foi Muito bom (0.81 a 0.98) e inter avaliadores satisfatório (0.56 a 0.83). O índice oclusal médio apresentado em 2Stg e 1Stg foi respectivamente 2,77 e 3,03. Houve significância na distribuição de índice oclusal G5. O grupo de interesse apresentou mais casos de índice G1 (10,65%) e menos casos G5 (1,18%), enquanto o GC apresentou 11,11% de índice G5. Os índices agrupados apresentaram maior percentual de resultados bom a excelente (1+2 = 36,68%) e menor percentual de índices pobres com provável necessidade de cirurgia ortognática (4+5 = 25,44%), enquanto o grupo 1Stg apresentou (1+2 = 31,67%; 4+5 = 30,56%). A variável cirurgião parece influenciar nos resultados dos índices oclusais. Não houve diferença entre índices oclusais dos cirurgiões que operaram nos dois grupos. Apesar da diferença estatística no índice 5 entre os grupos, não é possível afirmar que um protocolo seja melhor que o outro. Sugere-se não haver diferença entre eles e sim na experiência do cirurgião ino prognóstico do crescimento maxilar. Estudos futuros com follow-up mais longo devem ser realizados afim de acompanhar o crescimento dessas crianças

Primary surgeries interfere, to a variable extent, on the maxillary growth and development in patients with cleft lip and palate (CLP). The aim of the present study was to evaluate the influence of two-stage palatoplasty 2Stg (1st stage: lip repair + nasal ala + hard palate with vomer flap at 3-6m; 2nd stage: soft palate at 12-18m) on the maxilla development of children with unilateral cleft lip and palate (UCLP), without previous orthodontic intervention. The 2Stg group was compared to a control group of children submitted to one-stage palatoplasty (1Stg), with simultaneous repair of hard and soft palate (12-18m). Both protocols refer to the current (2Stg) and prior (1Stg) protocol of the Hospital for Rehabilitation of Craniofacial Anomalies (HRAC/USP). The influence of the technique, expertise of the plastic surgeon and timing of lip, hard palate and soft palate closure were verified and compared. A total of 349 plaster models of patients in complete deciduous or mixed dentition phase (2Stg: 169; 1Stg: 180) were categorized by three orthodontists as to severity of maxillomandibular discrepancy using the 5-year old occlusal index. The intraexaminer reliability was very good (0.81 to 0.98) and interexaminer reliability varied from satisfactory to very good (0.56 0.83). The mean occlusal index of 2Stg and 1Stg groups was 2.77 and 3.03, respectively. The variability of the mean index between surgeons varied from 2.38 to 3.2 (2Stg) and 2.91 to 3.2 (1Stg). There was significance in the distribution of G5 index (p=0.002) in which the study group (2Stg) presented more cases of index 1 (10.65%) and fewer cases of index 5 (1.18%), while the control group (1Stg) presented 11.11% of index 5. The grouped index showed a higher percentage of good to excellent results (indexes 1 + 2 = 36.68%) and lower percentage of poor index with probable need for orthognathic surgery (indexes 4 + 5 = 25.44%) compared to the 1Stg group (1 + 2 = 31.67%; 4 + 5 = 30.56%), though not statistically different. Individuals who underwent 2Stg palatoplasty had lower percentage of severe maxillomandibular discrepancy (index 5) compared to 1Stg palatoplasty, however, it is not possible to state that the 2Stg protocol is better than the 1Stg protocol, considering that there was no statistical difference between the mean occlusal indexes. The percentage distribution of occlusal indexes showed considerable variation between surgeons. Surgeons operating in both groups had similar mean occlusal indexes in both groups. Future studies with longer follow-up should be carried out to monitor the growth of children in the present sample
