Introdução. A notificação de eventos adversos para o gerenciamento de risco é um indicador para avaliar a segurança do paciente nos estabelecimentos de saúde, no entanto, à subnotificação e a notificação de baixa qualidade prejudicam a avaliação do risco/benefício da utilização de medicamentos. Objetivo. Identificar os problemas relacionados a notificação dos eventos adversos a medicamentos (EAM) e propor estratégias de prevenção destes eventos para promoção da segurança do paciente. Método. Foi realizado um estudo multi-método em que diferentes tipos de métodos de pesquisa foram utilizados para explorar o fenômeno de pesquisa "notificação de evento adverso". O método observacional foi realizado no estudo I do qual uma aplicação de análise detalhada de monitorização intensiva do farmacêutico com uso de instrumentos clínicos na identificação de eventos adversos. Enquanto, a revisão sistemática foi utilizada nos estudos II, III e IV que tiveram um escopo internacional contemplando fatores e estratégias na prevenção de eventos adversos. Já o relato de caso foi utilizado no estudo V que relata a importância do ensino da farmacovigilância na grade de formação do profissional para capacita-lo no gerenciamento de risco. Resultado. Estudo I: O uso do medicamento anticolinérgico com carga anticolinérgica cognitiva, superior ou igual a três, em idosos não alterou o número de EA, mas aumentou o tempo de internação com relação aos pacientes em uso de medicamentos com carga anticolinérgica cognitiva inferior a três. Por isto foi observado que a implantação de um rastreador de medicamentos com carga anticolinérgica maior ou igual a três pode ser uma estratégia de prevenir EAM em idosos. Estudo II: As sete principais categorias que sensibilizam as notificações de EAM nos profissionais de saúde são o Conhecimento de EAM, Incentivo, Métodos de notificação, Intervenção Educativas, Gerenciamento de Risco, Material Educativo e Responsabilidade do Profissional. É provável que múltiplas categorias associadas em uma educação continuada possam causar uma maior motivação em notificação dos EAM. Estudo III: As atuações efetuadas pelo farmacêutico, os tipos de intervenções para prevenção de eventos adversos observados foram: 1) reconciliação medicamentosa; 2) acompanhamento farmacoterapêutico; 3) revisão da farmacoterapia; 4) avaliação de medicamentos potencialmente inapropriados; 5) discussão com a equipe de saúde; 6) educação do paciente sobre o uso correto de medicamentos; 7) orientação do cuidador; 8) orientação na alta; e 9) acompanhamento após a alta. Estas intervenções múltiplas, juntamente com a utilização de instrumentos clínicos, melhoram o impacto do farmacêutico na saúde de idosos hospitalizados. Estudo IV: As bases de dados online tradicionais em farmacovigilância são as que demonstram, com mais frequência, a contribuição do paciente na notificação de EAM, apesar dos números de relatos estarem somados com os realizados pelos profissionais da saúde. Aplicativos para celular e a mineração de dados nas mídias sociais para identificar suspeitas de EAM relatadas pelos pacientes também foram estratégias identificadas. Estudo V: A adição da disciplina de farmacovigilância ao currículo de graduação é uma estratégia de educação inovadora para cursos da área de saúde com a finalidade de facilitar o treinamento de futuros profissionais de saúde na aquisição de competência em relação aos medicamentos pós-comercialização. A aprendizagem experimental e simulações devem ser implementadas para desenvolver essas competências em relação à farmacovigilância. Conclusão. Por fim, acredita-se que o conhecimento, a atitude e a habilidade só podem ser modificados pela formação direcionada aos eventos adversos dos medicamentos pós comercialização, assim espera-se amenizar as causas de subnotificação tornando os futuros profissionais de saúde mais capacitados para desenvolver orientações, ações educativas para orientar e conhecer as condutas necessárias para promover a segurança do paciente.

Introduction. The reporting of adverse events for risk management is an indicator for assessing patient safety in healthcare facilities; however, due to underreporting and low quality reporting, the risk / benefit assessment of the use of medicines is hindered. Objective. Identify problems related to reporting of adverse drug events (ADEs) and propose strategies to prevent these events to promote patient safety. Method. A multi-method study was carried out in which different types of research methods were used to explore the "adverse event notification" research phenomenon. The observational method was carried out in study I, of which an application of detailed analysis of intensive monitoring by the pharmacist using clinical instruments to identify adverse events was performed. Meanwhile, a systematic review was used in studies II, III and IV, which had an international scope covering factors and strategies in the prevention of adverse events. In contrast, the case report was used in study V, which reports the importance of teaching pharmacovigilance in the professional training grid to enable the professional in risk management. Result. Study I: The use of anticholinergic medication with a cognitive anticholinergic load greater than or equal to three in the elderly did not change the number of AEs, but increased the length of hospital stay by 1,5 times in relation to patients using medications with a cognitive anticholinergic load of less than three. For this reason, it was observed that the implementation of a drug tracker with an anticholinergic load greater than or equal to three may be a strategy to prevent ADEs in the elderly. Study II: The seven main categories that sensitize ADEs notifications in health professionals are ADEs Knowledge, Incentive, Notification Methods, Educational Intervention, Risk Management, Educational Material, and Professional Responsibility. It is likely that multiple associated categories in continuing education may cause greater motivation in reporting ADEs. Study III: The actions performed by the pharmacist, being the types of interventions for preventing adverse events observed were: 1) medication reconciliation; 2) pharmacotherapeutic follow-up; 3) review of pharmacotherapy; 4) evaluation of potentially inappropriate medications; 5) discussion with the health team; 6) education of the patient on the correct use of medicines; 7) caregiver guidance; 8) orientation at discharge; and 9) follow-up after discharge. These multiple interventions, together with the use of clinical instruments, improve the pharmacist's impact on the health of hospitalized elderly people. Study IV: The traditional online databases on pharmacovigilance are those that most frequently demonstrate the patient's contribution to the notification an ADE, despite the number of reports being added to those made by health professionals. Mobile applications and data mining on social media to identify suspected ADEs reported by patients were also identified strategies. Study V: The addition of the pharmacovigilance discipline to the undergraduate curriculum is an innovative education strategy for courses in the health field with the purpose of facilitating the training of future health professionals in the acquisition of competence in relation to post-marketing medicines. Experimental learning and simulations must be implemented to develop these competencies in relation to pharmacovigilance. Conclusion. Finally, it is believed that knowledge, attitude, and skill can only be modified by training directed to adverse events of post-marketing drugs; thus, it is expected to alleviate the causes of underreporting, making future health professionals more qualified to develop guidelines, educational actions to guide, and to learn about the necessary behaviors to promote patient safety.
