Introdução: A forma como a APS deve se organizar e atuar diante de singularidades existentes nos territórios rurais ou remotos tem levantado discussões. A literatura internacional vem se dedicando a analisar os fatores que influenciam a cobertura, qualidade e capacidade de resposta dos serviços de APS nas localidades que apresentam reduzido número de pessoas, dispersas em grandes distâncias e que apresentam dificuldades de acesso aos centros urbanos. A Amazônia, em que pese sua heterogeneidade geográfica e populacional, é um dos cenários onde estas condições são evidenciadas em uma parte significativa do seu território; entretanto, ainda prevalecem lacunas nas políticas e pesquisas na área da saúde dedicadas a esse tema. Objetivo: Analisar como os serviços de APS se organizam e se estruturam diante das necessidades e singularidades amazônicas. Métodos: Revisão de Escopo que incluiu estudos que focaram na APS na Amazônia, realizados entre 2000 e 2019; um Estudo de Caso na localidade Cuieiras situada na área rural de Manaus, com dados obtidos a partir de um Inquérito Domiciliar, Observação Participante e Entrevistas Semiestruturadas com gestores e profissionais. Resultados: A Revisão de Escopo incluiu 25 estudos. Destes, 11 apresentaram resultados promissores quanto ao funcionamento da APS na região amazônica, outros 14 apresentaram desafios e dificuldades nesse sentido. Algumas estratégias de APS implementadas na Amazônia mostraram impactos positivos na região, principalmente quando envolveram ações que, por meio de inovações, buscaram se adequar às singularidades locais. O mal preparo dos profissionais para atuar em áreas rurais e a abordagem restrita, com ênfase nos aspectos biomédicos foram as questões que estiveram mais relacionadas aos desafios e dificuldades, implicando em impactos negativos. Os achados do Estudo de Caso revelaram a relevância da inclusão de modelos de unidades adaptadas para região amazônica na PNAB, os quais significaram apoio institucional e recursos para a atuação em áreas rurais e remotas. Em contrapartida, a análise demonstrou limites para o desenvolvimento de ações sustentáveis e qualificadas na localidade estudada, os quais se relacionaram com a invisibilidade das singularidades presentes no contexto rural para o país, em especial o rural amazônico, implicando em problemas na forma como gestores e profissionais significam o lugar e no predomínio de uma atuação focada em um padrão urbano que considera os aspectos físicos próprios do contexto amazônico como obstáculos para a atuação dos serviços de saúde. A análise revelou ainda que atuação dos serviços de APS tem como pano de fundo relações de poder desequilibradas. Tais relações, existentes desde as estruturas mais amplas, invisibilizam e excluem as singularidades amazônicas. Esse desequilíbrio influencia as relações entre profissionais de saúde e as pessoas atendidas no local, o que pode ser identificado na limitada aproximação dos profissionais com o lugar e na adoção de práticas hegemônicas pelos profissionais de saúde, situações que tendem a reforçar as fragilidades e constranger a participação das pessoas na construção dos serviços. Conclusões: O conhecimento sobre o que tem sido ofertado em termos de serviços de APS para Amazônia revelou possibilidades inovadoras de atuação de serviços de saúde para áreas fora das localidades urbanas. No entanto, também evidenciou desafios, entre eles o desconhecimento sobre o que seria ideal para os serviços de APS nessa região, o número reduzido de estudos sobre esse tema e desequilíbrios nas relações de poder que impactam a prestação de serviços na Amazônia rural. Os achados destacaram que além do desafio de alcançar as pessoas diante das especificidades do território amazônico, os serviços de saúde são responsáveis por contribuir para a transformação das condições de vida e romper com fatores geradores de vulnerabilidades. Nesse sentido, há a necessidade de pesquisas que foquem em formas de construção de serviços de saúde em cooperação com pessoas assistidas que resultem no empoderamento das pessoas e na construção de serviços de saúde mais sustentáveis e adaptados nas regiões amazônicas.

Introduction: How PHC should organise itself and act given singularities in rural or remote territories have raised discussions. The international literature has analysed the factors that influence the coverage, quality, and responsiveness of PHC services in locations with few people dispersed over large distances and difficulties in accessing urban centres. Despite its geographic and population heterogeneity, the Amazon is one of the scenarios where these conditions are evident in a significant part of its territory. However, gaps in health policies and research dedicated to this topic still prevail. Goal: To analyse how PHC services are organised and structured in light of Amazonian needs and singularities. Methods: Scope Review that included studies focused on PHC in the Amazon, carried out between 2000 and 2019; a Case Study in Cuieiras, located in the rural area of Manaus, using data obtained from a Household Survey, Participant Observation and Semi-structured Interviews with managers and professionals. Results: The Scope Review included 25 studies. Of these, 11 presented promising results regarding the functioning of PHC in the Amazon region, another 14 presented challenges and difficulties in this regard. Some PHC strategies implemented in the Amazon showed positive impacts in the region, especially when they involved actions that, through innovations, sought to adapt to local singularities. The poor preparation of professionals to work in rural areas and the restricted approach, with an emphasis on biomedical aspects, were the issues that were most related to challenges and difficulties, resulting in negative impacts; findings of the Case Study revealed the relevance of including models of units adapted to the Amazon region in the PNAB, which meant institutional support and resources for working in rural and remote areas. On the other hand, the analysis showed limits for the development of sustainable and qualified actions in the studied location, which were related to the invisibility of the existing singularities in the rural context of the country, especially the rural Amazon, resulting in problems in the way managers and professionals perceive the place, as well as in the predominance of actions focused on an urban pattern that considers the physical aspects of the Amazon context as obstacles to the delivery of health services. The analysis also revealed that the delivery of PHC services has unbalanced power relations as a backdrop. Such relationships, which exist from the broader structures, make the Amazon singularities invisible and excluded. This imbalance influences the relationships between health professionals and local patients, which can be identified in the limited approximation of professionals to the place and the adoption of hegemonic practices by health professionals, situations that tend to reinforce weaknesses and constrain the participation of people in the decisions regarding the services. Conclusions: The knowledge about what has been offered concerning PHC services for the Amazon region revealed innovative possibilities for delivering services at this level of care in areas outside urban areas. However, it also showed challenges, among them the lack of knowledge about what would be ideal for PHC services in this region, the reduced number of studies on this topic and imbalances in power relations that impact the provision of services in rural Amazon. The findings emphasized that beyond the challenge of reaching people in the face of the specificities of the Amazon territory, health services are responsible for contributing to the transformation of living conditions and breaking with factors that generate vulnerabilities. In this sense, there is a need for research that focuses on ways of building health services in cooperation with people assisted results in the empowerment of people and the construction of more sustainable and adapted health services in the Amazonian regions.
