Esta tese de doutorado visa contribuir com um registro crítico de uma fração do tempo político (2012-2019) da presença da Agenda 2030 e dos Objetivos do Desenvolvimento Sustentável (ODS) na agenda política e técnica da América Latina e do Caribe (ALC), que vai da Rio + 20 (2012) - ano no qual se inicia oficialmente o processo de definições da Agenda - às sessões do Fórum Político de Alto Nível do Desenvolvimento Sustentável (HLPF), condutor oficial do processo, em 2019, ano em que se encerra o primeiro ciclo de avaliações de todos os ODS pelo HLPF, razão pela qual foi escolhido como o limite final da periodização. Seu foco principal, neste contexto, é a saúde. Valendo-se da metodologia da análise de conteúdo, examina os principais documentos resultantes do conjunto de eventos e manifestações relacionados ao tema que ocorreram no período, no mundo e na região, reunindo governos de países, organizações multilaterais, organizações da sociedade civil, academia e setor privado, destacando os principais elementos comuns e os aspectos divergentes ao longo do processo, assim como entre os posicionamentos dos atores políticos envolvidos. O trabalho desdobra-se logicamente em cinco macroprocessos que guardam elementos comuns e divergentes, assim como influenciam-se mutuamente: 1) A Agenda 2030 e seus ODS no plano global; 2) A saúde na Agenda 2030 e seus ODS no plano global; 3) A Agenda 2030 e seus ODS na ALC; 4) A saúde na Agenda 2030 e seus ODS na ALC; e 5) A agenda 2030 e a saúde no Brasil, neste caso num balanço de menor intensidade. A análise apresenta a situação política, econômica e social da ALC, na qual se desdobra o processo da Agenda 2030 e seus ODS, bem como a situação de saúde e dos sistemas de saúde na região, palcos da implementação do ODS 3 e das metas dos demais ODS relacionados com a saúde. Examina também a cooperação Sul-Sul, pelo papel proeminente que lhe é atribuída no processo. Transcorrido o primeiro terço do tempo (2015-2019) para a implementação da Agenda até 2030, examina os sucessos alcançados, as dificuldades existentes e os desafios que estão presentes, tanto no plano global, quanto regional, para o alcance das metas dos ODS, concluindo que o contexto político é adverso neste momento do processo devido às barreiras que as forças conservadoras levantam à solidariedade internacional, aos direitos humanos e às alianças em prol do desenvolvimento que permearam, pelo menos retoricamente, as relações internacionais ao redor do ano 2000. Entre as principais barreiras encontram-se a recrudescência do neoliberalismo extremado na economia, as imensas desigualdades e inequidades entre países e no interior dos mesmos, a fragilização do multilateralismo e a emergência do unilateralismo xenófobo, entre outros elementos políticos. A tese conclui propondo que as governanças nacionais e global precisam ser transformadas com vistas à implementação de planos de desenvolvimento coerentes com a equidade e a inclusão social, num contexto de desenvolvimento econômico que preserve o meio ambiente e os recursos naturais ameaçados. Ademais, entende que é na redefinição do papel do Estado como provedor de direitos fundamentais como a saúde e outros ODS e no planejamento e ação intersetorial sob a égide do poder público, nos planos locais, nacionais e global, que se encontra a chave para o desenvolvimento sustentável e a consecução dos Objetivos do Desenvolvimento Sustentável da Agenda 2030.

This doctorate thesis intends to contribute to a critical record of a fraction of the political time (2012-2019) of the presence of the 2030 Agenda and of the Sustainable Development Goals (SDG) in the political and technical agendas of Latin America and the Caribbean (LAC), spanning from Rio +20 (2012), the year in which the process of definition of the Agenda was officially kicked off, to the sessions of the High-Level Political Forum of Sustainable Development (HLPF), the official conductor of the process, in 2019, when the first assessment cycle of all SDGs by the HLPF was wrapped up, which is why it was chosen as the final deadline for the periodization. In this context, its main focus is health. Using the content analysis methodology, it examines the main documents resulting from the ensemble of events and manifestations related to the theme that took place in that period, regionally and globally, bringing together country governments, multilateral organizations, civil society organizations, the academia and the private sector, highlighting the main common elements and the divergent aspects throughout the process, as well as between the positions of the political actors involved. The work is logically articulated into five macro processes that contain common and divergent elements that influence each other: 1) The 2030 Agenda and its SDGs on a global scale; 2) Health in the 2030 Agenda and its SDGs on a global scale; 3) The 2030 Agenda and its SDGs in LAC; 4) Health in the 2030 Agenda and its SDGs in LAC; and 5) The 2030 Agenda and health in Brazil, in this case in a less intense approach. The analysis presents the political, economic and social situation in LAC, in which the 2030 Agenda process unfolds with its SDGs, as well as the situation of health and health systems in the region, where the SDGs 3 and the goals of the remaining SDGs related to health are implemented. It also examines South-South cooperation, due to the prominent role it plays in the process. After the first third of the time (2015-2019) required to implement the Agenda by 2030, it examines the successes that have been achieved, the existing difficulties and the challenges, both globally and regionally, for the achievement of the SDG goals. It concludes that the political context is adverse at this moment of the process, due to the barriers that conservative forces have raised against international solidarity, human rights, and development-promoting alliances, which permeated, at least rhetorically, international relations around the year 2000. Some of the main barriers are the return of extreme neoliberalism in economy, the huge inequality between countries and within the countries themselves, the fragilization of multilateralism, and the emergence of xenophobic unilateralism, among other political elements. The thesis concludes proposing that national and global governances must be transformed in order to implement development plans that are coherent with equity and social inclusion, in a context of economic development that preserves the environment and natural resources under threat. In addition, it also understands that the key for sustainable development and the achievement of the Sustainable Development Goals of the 2030 Agenda lies in the redefinition of the role of the State as provider of fundamental rights such as health and other SDGs and the planning and intersectoral action under the State's leadership, locally, nationally, and globally.
