Objetivando compreender o modo como foram tratadas as demandas reprodutivas das pessoas vivendo com HIV/Aids em serviços de saúde especializados no Município de São Paulo, desenvolveu-se estudo qualitativo utilizando-se da observação participante como estratégia metodológica. Foram observadas e registradas em diário de campo atividades rotineiras tais como: consultas com diferentes profissionais, atividades grupais, discussão entre as equipes e discussões de casos, além do registro de depoimentos dos profissionais e usuários a respeito do tema. Para analise, adotou-se a perspectiva teórica do processo de trabalho em saúde e necessidades de saúde e dos Direitos Reprodutivos. De modo geral, os trabalhadores expressaram a compreensão de que as demandas reprodutivas eram das mulheres e ligadas ao controle da transmissão materno infantil do HIV. Questões como contracepção, reprodução, infertilidade, por exemplo, não foram reconhecidas como necessidades e/ou demandas dos usuários. Foi observado que os trabalhadores precisaram revisar as práticas assistenciais diante da manifestação pelo desejo da gravidez. As conclusões indicam a necessidade de evidenciar as diferentes racionalidades presentes no espaço assistencial e que vão se configurando, ora pela afirmação de um pólo (o do controle) ora pelas decisões não explicitadas pelos usuários e consubstanciada no "aparecimento da gravidez". Ainda parece necessário o reconhecimento da autonomia das pessoas vivendo com HIV/Aids quanto às decisões reprodutivas para que essas possam ocorrer em condições de menor risco. Para ir ao encontro dessa finalidade é necessário compreender às diferentes racionalidades que envolvem o contexto do controle da epidemia do HIV/Aids e o viver com HIV/Aids.

To understand the way in which professionals within specialized health services in the municipality of Sao Paulo have responded towards the reproductive demands of people living with HIV/Aids is the objective of this qualitative study. Observant participation was the main methodological strategy employed. Routine activities were observed and registered in a field diary and these included: consultations with different professionals, group activities, discussions among working teams, and case discussions, as well as statements made by professionals and clients to the researcher with respect to this theme. Theoretical perspectives contemplating health as medical work, health demands and Reproductive Rights were employed in the analysis. In general, health workers believed that demands with respect to reproductive health were formulated exclusively by female clients and were related to maternal HIV transmission. lssues concerning contraception, reproduction and infertility, for example, were not recognized as patients' necessities and/or demands. Confronted with a patient's explicit desire to become pregnant, for example, these health workers had to revise their practices with respect to assistance. This study indicates the need to explicit the different rationalities which make themselves present within the process of health care and which are constituted, on one hand, by the assertion of control, and, on the other hand, by the unexplicited reproductive decisions of clients, consubstantiated by the "unexpected advent/emergence of pregnancy". Furthermore, it is necessary to recognize the autonomy of people living with HIV/Aids with respect to reproductive decisions so that these may occur within circumstances involving fewer risks. In order for this to occur, it is necessary to understand the different rationalities present within the contexts in which reproductive decisions are made as well as those in which the norms involving the control of the HIV/Aids epidemic are defined and put into practice.
