O governo municipal de Fortaleza (2005-2008), empossado em janeiro de 2005, reorientou a política pública de saúde na cidade e desenvolveu um novo modelo de atenção à saúde bucal. Neste estudo analisou-se como foi o financiamento dessas ações pelas três esferas de governo, o curso histórico da implantação das políticas públicas municipais de saúde bucal, a implementação do novo modelo de atenção e, ainda, o processo de gestão que levou à tomada de decisão de incluir equipes de saúde bucal na estratégia saúde da família. Trata-se de estudo analítico-crítico, incluindo construção retrospectiva, dentro da lógica geral do método histórico. O trabalho toma o município de Fortaleza como caso e, para empreender a análise, vale-se da literatura sobre o assunto e entrevistas com atores-chave. As entrevistas foram semi-estruturadas e realizadas com os principais atores que no período da segunda metade do século XX e começo do século XXI estiveram envolvidos com a gestão da saúde na cidade. A literatura e as entrevistas permitiram remontar o curso histórico da política de saúde bucal em Fortaleza. Constata-se a insuficiência do financiamento para as ações de saúde bucal requeridas para a população do município, notadamente com a pífia participação da esfera estadual. Destaca-se uma modelo como um recipiente das possibilidades dos tomadores de decisão e aponta-se o marco conceitual da Saúde Bucal Coletiva como sustentáculo do modelo em desenvolvimento, contudo com as lacunas e insuficiências de enraizamento popular, o que implica tensões e requer protagonismo político dos setores interessados na consolidação desse novo modelo da atenção em saúde bucal.

The new Fortaleza City Hall which started in 2005 redesigned public health in the city and launched a new model of public politics with focused attention on oral health. The present study analyzed (1) how these actions were sponsored by the three layers of the government, (2) how these public politics of oral health took place in the course of history of public health, (3) the implementation of the new model of assistance and (4) the decision making process of including public health staff in health family strategy. Through previous literature review available in the area, the study was characterized as critical analytical, including retrospective reconstruction within the general logic of the historical method. The study selected the city of Fortaleza for case study and besides literature review, semi-guided interviews were carried with the main agents involved in public health in the city from the middle of the XX century to the beginning of the XXI century. The review of the literature in tandem with the interviews enabled to resemble the historic course of oral public health in the city. Results seem to indicate the an insufficiency of sponsorship for public oral health. In addition to that, a new model is highlighted as a catalyst of new possibilities for the decision makers, and the conceptual mark of oral health is pointed as a new basis for the model under development. However, the gaps and insufficiency which are popular grounded imply tension and requires political focus from the sectors involved in the consolidation of the new model of attention to public oral health.
