Introdução - No atual contexto da epidemia da Aids no Brasil, em que se destaca o advento da terapia antiretroviral de alta atividade (TARV), o surgimento da síndrome da lipodistrofia associada ao HIV/Aids aponta para a necessidade de estudos que avaliem efeitos de intervenções nutricionais nesta população. Objetivo - Avaliar a eficácia de um programa de aconselhamento nutricional na prevenção de alterações morfológicas e metabólicas em pacientes vivendo com HIV/Aids, em uso de terapia antiretroviral de alta atividade. Métodos - Estudo de intervenção, do tipo ensaio clínico aleatorizado. Foi conduzido por um período de 12 meses e abrangeu 53 indivíduos adultos vivendo com HIV/Aids de ambos os sexos, em uso de TARV, alocados nos grupos intervenção e controle. O aconselhamento nutricional baseou-se na promoção de padrão alimentar saudável. Foram avaliadas variáveis referentes à antropometria, parâmetros bioquímicos, pressão arterial e consumo alimentar; coletadas em 4 momentos de investigação. Os dados foram descritos segundo a mediana e percentis, analisados segundo a técnica lntention-to-treat, Anova for Repeted Measures para a evolução das variáveis quantitativas, Profile Test para comparar os valores entre os tempos, teste de Tukey para a comparação entre os grupos e a avaliação das áreas sob a curva incrementai (AUIC). O nível de significância adotado para os testes estatísticos foi de 5%. Resultados - Os participantes de intervenção e controle não se diferenciaram no início do estudo em relação às características estudadas. Considerando a comparação entre os tempos, observou-se alterações em ambos os grupos para as variáveis: dobra cutânea subescapular (intervenção: T1=14,0 e T4=15,6; controle: T1=13,6 e T4=16,1; p<0,001), colesterol LDL (intervenção: T1=99,0 e T2=93,0; controle: T1=108,0 e T2=102,5; p=0,047), glicemia de jejum (intervenção: T1=86,0 e T2=76,0; controle:T1=85,50 e T2=76,00; p=0,001), pressão arterial diastólíca (íntervenção:T1=80,0 e T2=70,0; controle: T1= 77,5 e T2=70,0; p=0,04) e consumo de fibras (intervenção: T1=13,6, T4=24,5 e controle T1=17,03 e T4=22,5; p<0,001). Considerando a AUIC, houve diferença entre os grupos no percentual de gorduras da dieta (AUIC intervenção: -1 ,79; AUIC controle = 2,82; p=0,023). Conclusão: A eficácia da intervenção nutricional proposta limitou-se a melhoras no padrão alimentar dos indivíduos estudados, caracterizada pela adequação do consumo de fibras dietéticas e redução no consumo percentual de lipídeos.

lntroduction - Current Brazilian context of Aids epidemic, including advent of highly active antiretroviral therapy (HAART) and consequent lipodystrophy syndrome related to HIV/Aids, emphasizes the lack of investigation on effects of nutritional interventions for. this population. Objective - Evaluate efficacy of a nutritional counseling program on prevention of morphologic and metabolic alterations in patients living with HIV/Aids, under highly active antiretroviral therapy. Methods - lt was conducted a randomized clinical trial, for 12 months, involving 53 adults, both genders, under HAART, allocated at intervention and control groups. Nutritional counseling was based on promotion of healthy eating patterns. Anthropometrical, biochemical, blood pressure and food intake variables were assessed for 4 period investigation. Data was described according to median and percents. lt was performed lntention-to-treat analysis, Anova for Repeted Measures for evolution of quantitative variables, Profile Test to compare values between times, Tukey Test for comparing groups and evaluation of Area Under Incremental Curve (AUIC), at 5% significance statistic level. Results - lntervention and control participants were equals at baseline, according to assessed characteristics. Considering comparison between investigation times, observed alterations for both groups were related to variables: sub scapular skin fold (mm) (intervention: T1=14,0 and T4=15,6; control: T1=13,6 and T4=16,1; p<0,001), LDL-cholesterol (mg/dl) (intervention: T1=99,0 and T2=93,0; control: T1=108,0 and T2=102,5; p=0,047), fasting glucose (mg/dl) (intervention: T1 =86,0 and T2=76,0; control: T1 =85,50 and T2=76,00; p=0,001), diastolic blood pressure (mmHg) (intervention: T1 =80,0 and T2=70,0; control: T1 = 77,5 and T2=70,0; p=0,04) and dietetic fiber intake (g) (intervention: T1 =13,6 and T4=24,5; control T1=17,03 and T4=22,5; p<0,001). Considering AUIC, there was significant difference between groups for dietetic fat percent (%) (AUIC intervention: - 1 ,79; AUIC control = 2,82; p=0,023). Conclusion: Nutritional intervention efficacy was limited to improvements at individuals eating patterns, characterized by dietetic fiber adequate and fat percent reduction.
