Objetivo. Avaliar os efeitos à saúde decorrentes da exposição a níveis pouco elevados de mercúrio metálico, e contribuir para a facilitação do diagnóstico da intoxicação crônica. Métodos. Foi realizado um estudo transversal em um grupo de 180 trabalhadores expostos a mercúrio metálico e de 180 não expostos em uma indústria de lâmpadas elétricas, respeitando-se a homogeneidade entre os grupos em relação às variáveis sexo, idade escolaridade e funções desenvolvidas. Foram feitas avaliações clínico-neurológicas, dosagem de mercúrio urinário e aplicada uma bateria de testes neuropsicológicos. Resultados. O tempo médio de exposição ao mercúrio foi de 3,93 anos, sendo que o tempo mínimo foi de 2 (dois) meses e o máximo de 20 (vinte) anos. As avaliações ambientais de mercúrio no ar apresentadas pela empresa, para o período estudado, mostraram valores que variaram de 0,000 a 0,083 mg Hg/m3 de ar. A média de mercúrio urinário dos expostos foi de 31,11 e dos não expostos 2,04 ug/l de urina, sendo que o nível máximo de mercúrio encontrado foi de 129,0 ug/l. Os sinais e sintomas referidos pelos trabalhadores que apresentaram significância estatística foram: Síndrome Gastrointestinal: sangramento oral, sialorréia, má digestão, gosto metálico na boca, náuseas, gengivite, ulceração oral, diarréia, amolecimento dos dentes; Síndrome Neurológica: cãibras, parestesia, tremores, sonolência, alteração de grafia, abalos e fraqueza muscular; Síndrome do Eretismo Psíquico: nervosismo, irritabilidade, dificuldade de memória, ansiedade, tristeza, depressão, redução da atenção, agressividade, insegurança e medo. Ao exame fisico foram detectadas as seguintes alterações cujos resultados foram estatisticamente significativos: hiperemia de orofaringe, depósitos gengivais e de palato, ulceração oral, linha azul na borda gengival, tremores, alteração de grafia, de sensibilidade térmica e hiperreflexia. Não houve diferença estatisticamente significativa entre os dois grupos com relação a pressão arterial. Quanto aos resultados dos testes neuropsicológicos foram encontradas diferenças estatisticamente significativas entre os grupos em relação aos seguintes testes: aptidão mecânica de Léon Walther - TAM (itens discos, contas e pontilhagem); bateria de atenção concentrada de Toulouse Pierón e Wechsler Adult Intelligence Scale- WAIS (item símbolos numéricos) indicando a presença de déficits na rapidez de movimentos, destreza manual e coordenação motora; na atenção concentrada; na eficiência cognitiva e velocidade perceptiva motora. Não foi encontrada diferença entre os dois grupos no teste Wechsler Memory Scale, quanto a memória (itens informação e orientação; controle mental; memória lógica; reprodução visual e aprendizagem por associação); porém, a presença de tremor foi altamente significativa no grupo de trabalhadores expostos. Conclusões. Com base nos dados encontrados na investigação consideramos que existe associação entre a exposição a concentrações pouco elevadas de vapores de mercúrio e as alterações patológicas encontradas nos trabalhadores expostos. As alterações encontradas contribuem para a facilitação do diagnóstico da intoxicação.

Objective. To evaluate the toxic effects of low-level exposure to metallic mercury and to contribute to the chronic intoxication diagnosis. Methods. A cross sectional study has been carried out in 180 exposed workers to metallic mercury and 180 non exposed of a electric light bulbs industry in wich were maintened the homogeneity between the groups relating to the variables: sex, age, level of education and employment. The clinicals effects, the level of urinary mercury and a neuropsychological test battery of exposure to metallic mercury were investigated. Results. The average time of mercury exposure was 3,93 years. The minimum time was 2 (two) month and the maximum was 20 (twenty) years. The levei of mercury in air was evaluated by the industry and the variation of the values ranged from 0,000 to 0,083 mg/Hg/m3. The level average of mercury found in the urine of exposed workers was 31,11 ug/l and the of non exposed was 2,04 ug/l. Among the exposed workers the highest level of urinary mercury was 129,0 ug/l. The signs and symptoms mentioned by the two groups of workers that were statistically significant were: Gastrointestinal Syndrome: metallic taste in the mouth, sickness, gingivitis, ulcerations of the oral mucosa, diarrhoea, loose teeth; Nervous Syndrome: muscle fatigue, cramps, tremor, sleepiness, spasms, change of handwriting; Psychic Erethism Syndrome: nervousness, irritability, decrease in memory, anxiety, melancholy, depression, decrease in attention, aggressive, insecurity and fear. In the clinical picture were finding the signs statistically significant: oral cavity irritation, faringitis, oral mucosa ulcerations, blue line of the gums, tremor, handwriting alteration, thermic sensibility alteration and hyperreflexia. The differences were not statistically significant between the groups about arterial hypertension. The psychologycal investigation showed alterations estatistically significant between the groups in the tests: ability mechanical test of Léon Walther- TAM (disk, reckoning and dot); battery about attention concentrated of Toulouse Pierón and Wechsler Adult Intelligence Scale- WAIS (digit symbols), slowing the presence of deficit in the motor speed, manual dexterity and motor coordenation; attention and concentration; cognitive efficiency and perceptual motor speed. It was not found statistically significant differences between the two groups in the memory test Wechsler Memory Scale, (information, orientation, mental control, logic memory, visual, reprodution association learning), however, the tremor was statistically significant in the exposed workers group. Conclusions. Comparing the exposed workers with the non exposed, the investigation showed pathological findings probably caused by mercury exposure. The alterations found provide an easy intoxication diagnosis.
