De um total de 836 casos de varíola ocorridos no ano de 1969 e de 932 ocorridos em 1970, descreve-se a investigação epidemiológica de 245 casos contidos em seis surtos ocorridos no Rio Grande do Sul, Brasil. Este trabalho demonstra a importância da investigação epidemiológica da varíola como elemento indispensável para o desenvolvimento de um sistema de vigilância epidemilógica. A utilização de tal método proporcionou uma melhoria considerável na qualidade da informação e permitiu a identificação de um maior número de casos. O aumento pode ser demonstrado pelo número médio de casos encontrados para cada caso notificado. Vinte e cinco para um em 1969 e 17,5 para um em 1970. A qualidade da informação foi apurada pela confirmação ou não do diagnóstico inicial do notificante, tanto clínica como laboratorialmente. A investigação epidemiológica também auxiliou na erradicação de varíola, descobrindo casos em áreas já vacinadas, demonstrando a origem dos surtos e revelando problemas criados pelo fluxo bidirecional através de fronteiras. Esta filosofia de trabalho é recomendada para outras doenças como parte de um programa de desenvolvimento gradativo de atividades de vigilância epidemiológica.

In 1969/70, 1768 cases of smallpox were reported in Rio Grande do Sul, Brasil. The epidemiological description of 245 of them which pertained to six outbreaks is presented. The paper shows clearly the importance of small pox epidemiological investigation as an indispensabel tool for the development of an epidemiological surveillance system. Utilization of this method caused a major impact in information quality and promoted an increase in the number of cases identified. This increase can be shown by the mean number of cases discovered for each reported case: 25:1 in 1969 and 17,5:1 in 1970. The assessment of information quality was made by checking clinical and laboratory confirmation against the initial diagnosis reported. Epidemiological investigation played an important role in smallpox erradication, identifying new cases in areas already covered by mass vaccination, detecting outbreaks' source of infection and revealing problems generated by two ways population flow through international border. Such working principies can be applied to epidemiological surveillance related to other diseases.
