Introdução. A tuberculose vem acometendo cada vez mais os idosos em relação aos não idosos. Objetivo. Verificar diferenças na tuberculose pulmonar com baciloscopia positiva que acomete o idoso em comparação com o adulto jovem. Metodologia. Selecionados 80 idosos com 60 anos de idade ou mais, doentes de tuberculose pulmonar com baciloscopia de escarro positiva para o bacilo álcool ácido resistente, residentes e tratados no Município de São Paulo-SP em 2001. Estes foram pareados com 80 adultos entre 20 e 39 anos de idade, da mesma unidade de saúde e mesmo sexo, procurando-se também parear escolaridade, ocupação e residência. Comparou-se a evolução do tratamento por até 12 meses. Resultados. Os idosos tiveram freqüências maiores em: tempo de demora entre o início dos sintomas ao início do tratamento, reações medicamentosas, pacientes tratados por mais de seis meses, número de óbitos, doenças associadas e mudanças de medicamentos. Porém tiveram menos sintomas e menor número de abandonos de tratamento. Conclusões. Os idosos, devido às dificuldades no diagnóstico, demoras no início do tratamento e doenças associadas, têm maiores riscos de letalidade e de complicações durante o tratamento, principalmente entre os mais velhos. Preconiza-se uma atuação mais atenta e capacitada dos profissionais, bem como políticas públicas que contemplem este segmento etário.

Introduction. Tuberculosis has been increasingly affecting elderly compared to non-elderly populations. Objective. To assess differences in positive microscopy pulmonary tuberculosis affecting elderly compared to young adults. Methods. The study comprised 80 elderly subjects aged 60 years and more who had pulmonary tuberculosis with positive sputum microscopy and acid-fast bacilli detected and were living and treated in the municipality of São Paulo, Brazil, in 2001. These subjects were paired with 80 adults aged between 20 and 39 years, of the same sex and treated in the same health care center, and also matched by schooling, occupation, and place of residence. Treatment progression was compared for up to 12 months. Results. Elderly group showed to have greater delay in starting treatment after symptom onset, more drug reactions, more patients treated for more than six months, more deaths, more associated diseases and more drug treatment changes. However, they had fewer symptoms and fewer treatment dropouts. Conclusions. Elderly, especially those of more advanced age, have higher risk of fatality and complications during treatment due to difficult diagnosis, delay in starting treatment and associated diseases. It is recommended that health care providers deliver more thorough and qualified attention to these populations as well as developing public policies targeted to this age group.
