Analysis of the influence of prosthetic rehabilitation of the ocular globe in anophthalmic patients: a prospective longitudinal clinical study.

Abstract in English
The aim of this longitudinal and prospective study was to identify the factors that influence the anophthalmic patients to seek for treatment, evaluate the quality of life and perceived stress before and after the rehabilitation with ocular prosthesis and correlate with the sociodemographic characteristics, cause of eye loss and type of surgery. The clinical and sociodemographic data were collected by an identification record and anamnese. A semistructured questionnaire in accord to Planned Behavior Theory was applied to identify the patient's motivation to seek treatment, and the interviews were recorded and transcribed to data analyses, guided by the theme content. The quality of life and stress were evaluated by the SF-36 and PSS-10 questionnaires, respectively on Baseline, and after the 03 and 06 months of prosthesis insertion. The adaptation difficulties were evaluated by a questionnaire after 07 days, 03 and 06 months. The comparison of the SF-36 and PSS-10 scores were made by Friedman's test; the clinical characteristics were evaluated by Cochran's Q test and Friedman's test; the correlation between variables were made by Pearson's Correlation test (α = 0,05; statistical software SPSS 12.0). The sociodemographic results showed a predominance of adult individuals (53,84%), male (69,23%), married (42,3%), low-superior socioeconomic income (65,38%), with eye loss by trauma (46,15%), eviscerated (50%) and the anophthalmic period of three months by five years (38,46%). The semistructured interview indicates that social and economic factors highly influenced the individuals to seek for treatment. The quality of life showed an improvement in "pain" domain at 03 months and in "General Health State" at 06 months; stress didn't show significant difference throughout the study. The clinical evaluation showed presence of discharge and eyelash crusting throughout 06 months and presence of pain at 07 days, with regression at 03 and 06 months. Had weak correlation between sociodemographic characteristics and "Physical Limitation" in Baseline (0,423) and "General Health State" in 03 months (0,494); cause of anophthalmia and "Physical Limitation" at 06 months (-0,471) and type of surgery and "General Health State" at Baseline (0,432). Its's concluded that the patient's motivation as to seek prosthetic treatment seems to be influenced by the public attend quality and disponibility, economics issues, indications and information about treatment by the others or by professionals; the ocular prosthesis showed a positive influence in two domains of quality of life, but without significant alteration in stress; the prosthesis promoted an income in tissues health, but with the continuous presence of discharge and crusting, which were related as the frequent difficulties between patients; individuals with better socioeconomic conditions showed better general health state and less physical limitation and persons with trauma eye loss submitted to enucleation showed better physical limitation and general health state, respectively.
