Os limites fundamentais da resolução espacial e do contraste que se consegue nas imagens radiológicas são devidos à natureza quântica da radiação X. Pequenos detalhes de baixo contraste nas estruturas anatômicas podem ser revelados somente pelo aumento da fluência de fótons absorvidos por aquelas estruturas. Este é um procedimento aceitável nas aplicações industriais, mas, proporciona um risco ao paciente nas aplicações clínicas dos raios X. Nós desenvolvemos um procedimento analítico para encontrar o melhor compromisso entre as condições diagnósticas de alta absorbância no paciente, para a qual poucos fótons levam informação aos detentores, e de baixa absorbância no paciente, para a qual as flutuações quânticas no grande número de fótons que alcançam os detentores mascaram a informação. A análise é estendida ao método da dupla energia para obtenção de imagens diagnósticas melhoradas digitalmente. Para esta finalidade, a escolha das energias dos fótons precisa ser otimizada, como também o tempo destinado à exposição em cada energia. A otimização do tempo da exposição é um novo conceito experimental. Os procedimentos analíticos são desenvolvidos para encontrar as soluções numéricas, utilizando condições gerais relativas aos espectros de raios X, aos filtros característicos adicionais, e à sensibilidade do detentor. A utilização destes novos métodos simultaneamente com o diagnóstico deve reduzir a dose absorvida enquanto mantém a qualidade, não somente das imagens diagnósticas convencionais utilizando o sistema de detecção tela-filme, mas também, das imagens melhoradas pelo computador na tomografia feita por dupla energia. A dose absorvida deverá ser reduzida na obtenção das imagens por subtração digital, como aquelas devidas às diferenças temporais ou aos agentes de contraste incorporados pelo paciente. The fundamental limits to the spatial resolution and to the contrast attainable in radiological images are owing to the quantum nature of X-radiation. Smalllow-contrast details in anatomic structures may be revealed only by increasing the fluence of photons absorbed by those structures, an acceptable procedure in industrial applications but which incurs a proportional risk to the patient in clinical applications of X- rays. We develop an analytical procedure to find the optimum compromise between the diagnostic conditions of high patient absorbance, for which few photons remain to carry information to the detectors, and low patient absorbance, for which the quantum fluctuations in the large numbers of photons reaching the detectors obscure the information. The analysis is extended to the dual-energy method of obtaining digitally enhanced diagnostic images for which both photon energies must be optmized, as well as the optimum counting time alotted to each, a new experimental concept. The analytical procedures are developed for obtaining numerical solutions using general X-ray spectra, external filter characteristics, and detector sensitivity. Use of these new methods to computer control exposures should reduce the absorbed dose while maintaining the quality, not only of conventional film-screen diagnostic images, but of computer enhanced images obtained in dual-energy tomography, in contrast agent and temporal digital subtraction radiology and in the future clinical applications of syncrotron X-radiation as well
