Os carcinomas espinocelulares de orofaringe e laringe podem ser tratados clinicamente (quimioterapia associada à radioterapia) ou cirurgicamente (cirurgia e, eventualmente, radioterapia) como opção terapêutica inicial, com intenção curativa. Ambas as modalidades apresentam diferenças significativas em vários aspectos clínicos e terapêuticos, mas a diferença em termos de custos ainda não foi avaliada em uma instituição brasileira. O objetivo deste estudo é analisar os custos médicos diretos em ambas as modalidades em um estudo retrospectivo, no período entre 2014 e 2017, no Instituto do Câncer do Estado de São Paulo (ICESP), juntamente com fatores sociodemográficos e desfechos clínicos. Os resultados foram semelhantes em termos de custos para o tratamento de ambas as neoplasias: na coorte Orofaringe: Clínica U$ 32.460,54 (DP U$ 15.612,57) e Cirúrgica U$ 31.119,99 (DP U$ 9.720,69), grupos (p 0,396) e, na coorte Laríngea, U$ 32.259,65 (DP U$16.841,10) para o grupo clínico e U$34.385,87 (DP U$17.705,56) para o grupo cirúrgico (p 0,215). Os resultados clínicos foram semelhantes para CEC de orofaringe (p 0,087), mas melhores resultados para CEC de laringe em termos de taxas de sobrevivência (p <0,001). Este estudo pode orientar protocolos públicos de tratamento para tumores complexos de Cabeça e Pescoço, mas são necessárias mais avaliações para melhores conclusões

Oropharyngeal and Laryngeal Squamous Cell Carcinomas may both be treated clinically (chemotherapy associated with radiation therapy) or surgically (surgery and, eventually, radiation therapy) as initial therapeutic option, with curative intention. Both modalities have significant differences in several clinic and therapeutic aspects, but the difference in terms of costs has not still been evaluated in a Brazilian institution. The aim of this study is to analyze direct medical costs in both modalities in a retrospective study, in the period between 2014 and 2017, at Sao Paulo State Cancer Institution (ICESP), along with socio-demographic factors and clinical outcomes. The results were similar in terms of costs for the treatment of both neoplasms: in Oropharynx cohort: Clinic U$32,460.54 (SD U$15,612.57) and Surgical U$31,119.99 (SD U$9,720.69), groups (p 0,396) and, in Laryngeal cohort, U$32,259.65 (SD U$16,841.10) for the clinical group and U$34,385.87 (SD U$17,705.56) for the surgical group (p 0,215). Clinical outcomes were similar for oropharyngeal SCCs (p 0.087), but better outcomes for laryngeal SCCs in terms of survival rates (p <0.001). This study may guide public treatment protocols for complex Head and Neck tumors, but it is needed further evaluations for better conclusions
