INTRODUÇÃO: Comunicação é o processo humano que visa trocar informações e estabelecer uma relação entre os participantes. No contexto do cuidado em saúde, a comunicação efetiva está relacionada a desfechos positivos para médicos e pacientes, e como tal, é parte do processo de formação de novos médicos. OBJETIVO: Este estudo objetiva analisar a influência das habilidades de comunicação na empatia percebida pelo paciente, satisfação com o cuidado e confiança no profissional sob o ponto de vista do paciente. METÓDOS: Estudo transversal de modelo misto (qualitativo e quantitativo) realizado com residentes do segundo ano de Clínica Médica e seus pacientes em um ambulatório de avaliação pré-operatória. Suas consultas foram gravadas em vídeo. Ao término, pacientes preencheram questionários para avaliar sua percepção da empatia do médico (Escala de Jefferson da Percepção de Empatia Médica pelo Doente) e satisfação com o cuidado (Net Promoter Score NPS e avaliação global). Ao final, participaram de uma entrevista de roteiro semiestruturado para investigar satisfação com o cuidado e confiança no profissional. Posteriormente, os pesquisadores avaliaram as habilidades de comunicação verbais (Sistema de Comportamentos de Comunicação Médica) e não verbais (definidas por Heintzman et al. e Caris-Verhallen et al.) por meio dos vídeos das consultas. As medidas das habilidades de comunicação foram comparadas entre os pacientes mais satisfeitos e os menos satisfeitos pela classificação do NPS. As entrevistas foram analisadas por meio de análise temática. RESULTADOS: Sessenta pacientes (mediana = 61,5 [52,5 - 70,0] anos; 60,0% mulheres) e 16 residentes (mediana = 26,0 [25,0 27,0] anos; 43,8% mulheres) participaram do estudo. As medidas de empatia do médico percebida pelo paciente (mediana = 32,0 [29,8 - 35,0]) e satisfação do cuidado medida pelo NPS (mediana = 10 [9 - 10]) e por avaliação global (mediana = 10 [10 - 10]) apresentaram efeito teto. A classificação de NPS demonstrou 3,3% de detratores, 11,7% de neutros e 85,0% de promotores. Não houve associação entre essas medidas e sexo do paciente, do médico, escolaridade do paciente ou liberação para o procedimento. Encontrou-se correlação significativa fraca entre empatia e avaliação global e fortes entre empatia e NPS e avaliação global e NPS. O grupo de pacientes mais satisfeitos utilizaram significativamente mais a categoria Habilidades Afetivas; o grupo menos satisfeito utilizou mais a habilidade verbal Perguntas de Conteúdo. Não houve diferença entre nenhuma outra habilidade verbal ou qualquer habilidade não verbal. Identificou-se que as ações do médico, os efeitos no paciente e o contexto do cuidado são fatores que interferem na relação entre habilidades de comunicação e satisfação do paciente com o cuidado e confiança do paciente no profissional. CONCLUSÃO: Habilidades de comunicação influenciam a satisfação do paciente com o cuidado e confiança do paciente no profissional. Percebeu-se que a satisfação com o cuidado foi influenciada pela expressão de afetividade e por perguntas feitas pelo paciente e não sofreu influência das habilidades não verbais ou da comunicação do médico. Além disso, o contexto em que ocorre o cuidado, bem como as ações do médico e suas consequências no paciente também permitem compreender os fatores determinantes para a construção de uma boa relação terapêutica

INTRODUCTION: Communication is the human process that aims to exchange information and to establish a relationship between participants. In the healthcare setting, the effective use of communication relates to positive outcomes for physicians and patients and, as such, it is part of the medical training of new physicians. OBJECTIVE: This study aims to analyse the influence of communication skills on patients perception of physician empathy, patients satisfaction with care and patients trust in physician. METHODS: A cross-sectional study using a mixed-methods model (qualitative and quantitative) with second-year Internal Medicine residents and their patients, at a preoperative risk assessment in an outpatient ambulatory setting. Consultations were videorecorded, and patients completed surveys to assess their perception of their physicians empathy (Jefferson Scale of Patient Perceptions of Physician Empathy) and satisfaction with care (Net Promoter Score NPS and global rating). Finally, patients were interviewed using a semi-structured script to investigate satisfaction with care and trust in physician. Afterwards, researchers assessed verbal (Medical Communications Behavior System) and non-verbal (defined by Heintzman et al. and Caris-Verhallen et al.) communication skills through consultation video recordings. Communication skills measurements were compared between more satisfied and less satisfied patients as classified by NPS. Interviews were analysed using thematic analysis. RESULTS: Sixty patients (median = 61.5 [52.5 - 70.0] years old; 60.0% female) and 16 residents (median = 26.0 [25.0 27.0] years old; 43.8% female) participated in the study. Measurements of patient perceived physicians empathy (median = 32.0 [29.8 - 35.0]) and satisfaction with care measured by NPS (median = 10 [9 - 10]) and by global rating (median = 10 [10 - 10]) presented with ceiling effect. NPS classification showed with 3.3% promoters, 11.7% passives and 85.0% detractors. There was no association between these measurements and patient and physician gender, patient educational level, or clearance for procedure. We found a weak significant correlation between empathy and global rating and strong correlations between empathy and NPS, and global rating and NPS. More satisfied patients used significantly more Patient Affective Behaviors; less satisfied patients use more Patients Content Questions. There was no significant difference in any other verbal or in any non-verbal communication skill. Furthermore, we identified that physicians actions, effects on patients and healthcare context are factors that interfere in the relationship between communication skills and patients satisfaction with care and patient trust in the physician. CONCLUSION: Communication skills influence patient satisfaction with care and patient trust in the physician. We identified that satisfaction with care was influenced by the expression of affection and by patient questioning. It was not influenced by patient nonverbal skills or physicians communication. Additionally, the context in which healthcare happens as well as a physicians actions and their consequences on patients also allow us to understand better the determining factors to building a good therapeutic relationship
