Fundoplicatura laparoscópica a Nissen (FLN) é indicada no tratamento do refluxo gastroesofágico (RGE) e no esôfago de Barrett (EB), pois permite um controle adequado do refluxo ácido e não ácido. Dismotilidade esofágica (DE) pode ser encontrada em indivíduos com RGE e EB. O objetivo deste estudo foi avaliar a DE, a presença de disfagia, o adequado controle do RGE e a extensão do esôfago de Barrett antes e após a FLN em pacientes previamente diagnosticados com EB e DE. Vinte e dois pacientes, dezesseis homens, com idade de 55,14 (± 15,52) anos foram diagnosticados préoperatoriamente com EB e DE através de endoscopia digestiva alta (EDA) com biópsias e manometria esofágica convencional (MEC) e submetidos a fundoplicatura laparoscópica a Nissen, foram incluídos neste estudo. No pós-operatório, os pacientes foram acompanhados com exame clínico, endoscópico, manométrico e pH-metrico por um período mínimo de 12 meses após o procedimento cirúrgico. O seguimento pós-operatório foi de 26.2 (± 9.27) meses e os pacientes não apresentaram sintomas de refluxo gastroesofágico (RGE) ou disfagia. Nenhuma morte ocorreu durante o estudo. A EDA mostrou que o comprimento médio do EB foi de 4,09 cm (± 2,05) no pré-operatório e de 3,77 cm (± 2,19) na avaliação pós-operatória (p 0,042). A manometria esofágica convencional MEC demonstrou que a pressão de repouso do esfíncter esofágico inferior (EIE) pré-operatória mediana (Q1; Q3) foi de 9,15 mmHg (5,78; 11,93) e 13,2 mmHg (11,0; 16,28) no pós-operatório (p = 0,006). A mediana (Q1; Q3) da amplitude préoperatória da contração do corpo esofágico foi de 47,85 mmHg (35,65; 60,43) e 57,50 mmHg (34,10; 80,45) no pós-operatório (p = 0,408). No pósoperatório, a pH-metria evidenciou que a média do percentual de tempo total de refluxo (%TTR) foi 0,38 (±0,66) e a média do escore de DeMeester (DMS) foi 2,00 (±3,32). A cirurgia antirefluxo laparoscópica a Nissen (FLN) pode ser indicada para pacientes com EB e DE, pois aumentou a pressão de repouso do EIE, reduziu a exposição do esôfago distal ao conteúdo gástrico, diminuiu a extensão do EB e foi capaz de melhorar a amplitude de contração do corpo esofágico. As alterações no peristaltismo esofágico não foram modificadas após a cirurgia e os pacientes não evoluíram com sintomas de RGE nem disfagia no pós-operatório

Laparoscopic Nissen fundoplication (LNF) is indicated for the treatment of gastroesophageal reflux (GER) and Barrett's esophagus (BE), as it allows adequate control of acid and non-acid reflux. Esophageal dysmotility (ED) can be found in individuals with GER and BE. The aim of this study was to evaluate ED, the presence of dysphagia, adequate GER control and the extent of Barrett's esophagus before and after LNF in patients previously diagnosed with BE and ED. Twenty-two patients, sixteen men, aged 55.14 (± 15.52) years were diagnosed preoperatively with BE and ED through upper gastrointestinal (GI) endoscopy with biopsies and conventional esophageal manometry (CEM) and submitted to LNF were included in this study. In the postoperative period, they were followed up with clinical, endoscopic, manometric and pH-metry tests for a minimum period of 12 months after the surgical procedure. The postoperative follow-up was 26.2 (± 9.27) months, and the patients did not present symptoms of gastroesophageal reflux (GER) or dysphagia. No deaths occurred during the study. The upper (GI) endoscopy showed that the average length of the BE was 4.09 cm (± 2.05) in the preoperative period and 3.77 cm (± 2.19) in the postoperative evaluation (p 0.042). CEM demonstrated that the median preoperative lower esophageal sphincter resting pressure (LESRP) (Q1; Q3) was 9.15 mmHg (5.78; 11.93) and 13.2 mmHg (11.0; 16, 28) in the postoperative period (p = 0.006). The median (Q1; Q3) of the preoperative range of contraction of the esophageal body was 47.85 mmHg (35.65; 60.43) and 57.50 mmHg (34.10; 80.45) in the postoperative period (p = 0.408). In the postoperative period, the pH metry showed that the average of the percentage of total reflux time (% TRT) was 0.38 (± 0.66) and the average of the DeMeester score (DMS) was 2.00 (± 3.32). Laparoscopic Nissen anti-reflux surgery (LNF) may be indicated for patients with BE and ED, as it increased LESRP, reduced distal esophagus exposure to gastric content, decreased the extent of BE and was able to improve the esophageal body contraction amplitude. Changes in esophageal peristalsis were not modified after surgery and the patients did not present symptoms of GER or dysphagia in the postoperative period
