INTRODUÇÃO: O câncer colorretal (CCR) é a segunda causa de mortalidade por câncer em países desenvolvidos e com incidência crescente em países em desenvolvimento como o Brasil. O fígado é o órgão acometido em até 75% dos pacientes que tem doença metastática. Metade dos pacientes com CCR apresentarão metástase hepática durante a vida, sendo que 20 a 40% restrita ao fígado. Os avanços no manejo perioperatório, quimioterapia e de estratégias e técnica cirúrgica fizeram com que muitos pacientes antes tidos como inoperáveis fossem submetidos às ressecções hepáticas com ganho de sobrevida e chances de cura. OBJETIVOS: Avaliar os resultados do tratamento cirúrgico e fatores prognósticos de pacientes com quatro ou mais metástases hepáticas de adenocarcinoma colorretal (MHCCR). MÉTODO: Coorte retrospectiva de pacientes com diagnóstico de MHCCR com quatro ou mais metástases submetidos a ressecção hepática com intuito curativo entre janeiro de 2000 e julho de 2019. RESULTADOS: Foram realizadas 1230 ressecções hepáticas das quais 617 em pacientes com diagnóstico de MHCCR. Destes, 145 pacientes apresentavam quatro ou mais metástases e foram submetidos ao tratamento cirúrgico. O tempo de seguimento médio foi de 36,2 meses. A sobrevida global média foi de 77 meses e a sobrevida média livre de doença foi de 30,3 meses. A probabilidade de sobrevida global e livre de doença em 5 anos foi de 40,6% e 18,1%, respectivamente. O número de metástases não impactou a sobrevida global. O maior número de metástases implicou em maior risco de recidiva, impactando negativamente na sobrevida livre de doença. A presença de linfonodos comprometidos na doença primária, CEA > 20ng/ml, ressecção R1, uso da ablação por radiofrequência associada à ressecção e maior número de metástases foram fatores independentes de piora da sobrevida livre de doença. CONCLUSÕES: O número de metástases hepáticas não alterou a sobrevida global, porém teve impacto negativo na sobrevida livre de doença. A maior probabilidade de recidiva da doença ocorreu quando presentes os seguintes fatores prognósticos: CEA > 20ng/ml, ablação por radiofrequência, N+ do tumor primário e ressecção cirúrgica do tipo R1. A ressecção de múltiplas metástases hepáticas pode propiciar sobrevida a longo prazo (5 anos) em 40% dos pacientes apesar de apenas metade destes estar livre de doença

INTRODUCTION: Colorectal cancer (CRC) is the second cause of death in developed countries and its incidence is rising in developing countries like Brazil. The liver is the site of distant metastases in up to 75% patients with stage IV disease. Half of the patients with CRC will present with liver metastases during their lives and aproximately 20- 40 % will be restricted to the liver. The evolution of anesthesia, perioperative care, systemic chemotherapy and surgical techniques/strategies led to an increase of surgical resectability in patients with multiple liver metastases. These patients were denied surgery in the recent past and now have a real chance of survival benefit and even cure. AIM: Evaluate the surgical treatment outcomes and prognostic factors of patients with multiple (four or more) liver metastases from CRC. METHODS: Retrospective study of patients with diagnosis of four or more liver metastases from CRC submitted to curative intent resection. RESULTS: From January 2000 to July 2019, out of a total of 1230 liver operations, 617 patients with CRC liver metastases were submitted to resection. Of these, 145 patients had four or more liver metastases and underwent liver resection. Mean follow-up was 36.2 months. Mean overall survival (OS) was 77 months and mean recurrence free survival (RFS) was 30.3 months. The 5 years OS and RFS were 40.6% and 18.1%, respectively. The number of liver metastases did not have impact on OS. The number of liver metastases, positive primary lymph nodes, CEA > 20ng/ml, R1 resection and associated radiofrequency ablation were independent negative factors associated with poorer RFS. CONCLUSIONS: The number of liver metastases did not have impact on OS. Factors significantly associated with recurrence were: CEA > 20ng/ml, radiofrequency ablation, positive primary lymph nodes and R1 resection. Liver surgery for patients with multiple liver metastases can lead to long term survival (5 years) in 40% of the patients despite only half of them will be free of disease
