Introdução: As Doenças Inflamatórias Intestinais (DII), Doença de Crohn (DC) e a Retocolite Ulcerativa (RCU), são desordens de caráter crônico, recidivante, elevada morbidade e impacto para o Sistema Único de Saúde. A despeito de tratamento adequado, alguns pacientes persistem com sintomas que nem sempre são justificados por atividade inflamatória. Nestes casos, o diagnóstico de Síndrome do Intestino Irritável (SII) é frequente, principalmente quando os resultados de calprotectina fecal são inferiores a 50 ?g/g. A SII é ainda considerada uma doença funcional com prevalência estimada de até 40% em pacientes com DII e a avaliação clínica exclusiva através dos critérios de ROMA IV é insuficiente para diagnosticar a sobreposição de DII-SII. Em 2015 dois auto-anticorpos, anti-CdtB e anti-vinculina, foram validados como marcadores da SII pós-infecciosa, porém ainda não há evidência na literatura do seu papel diagnóstico em pacientes com DII. Objetivo: O objetivo principal do presente trabalho é avaliar a aplicabilidade clínica e o desempenho dos anticorpos anti-CdtB e anti-vinculina para diagnóstico de Síndrome do Intestino Irritável em pacientes com Doenças Inflamatórias Intestinais e persistência de diarreia. O objetivo secundário é observar a coexistência de supercrescimento bacteriano nos pacientes com SII. Pacientes e Métodos: Os pacientes foram divididos em 4 grupos: (i) DII em atividade endoscópica e diarreia persistente (mais de 3 evacuações ao dia), (ii) DII em remissão endoscópica - DII-SII, (iii) SII com predomínio de diarreia; (iv) grupo controle. Os critérios de inclusão foram idade superior a 18 anos, presença de diarreia definida como mais de 3 evacuações ao dia e concordância em participar no estudo. Critérios de exclusão: a) outras doenças intestinais; b) antecedente de cirurgia abdominal; c) neoplasia atual ou prévia; d) uso de corticoterapia nos 6 meses anteriores à inclusão no estudo; e) infecção pelo HIV. Foram coletadas amostras de sangue no momento da colonoscopia e a determinação dos anticorpos anti-CdtB e anti-vinculina foi realizada por ensaio imunoenzimático indireto. Resultados: Foram incluídos 160 pacientes assim distribuídos: grupo i (n= 44); grupo ii (n= 25); grupo iii (n= 45); grupo iv (n= 46). A média de idade foi de 39,5 anos nos pacientes com DII em atividade, 46,5 anos no grupo com DII-SII e 39,5 anos nos pacientes com SII com predomínio de diarreia e 56,6 anos no grupo controle. Cinquenta e três por cento (37/69) dos pacientes com DII apresentavam diagnóstico de Doença de Crohn. o valor médio do anti-CdtB aferido por densidade óptica foi de 0,71±0,48 no grupo i, 0,71±0,43 no grupo ii, 0,70±0,44 no grupo iii e 0,85±0,72 nos controles. Não houve diferença significante entre os grupos (p=0,95). Os pacientes com DII-SII apresentaram tendência a níveis mais elevados de anti-vinculina (1,68±1,04) quando comparados aos pacientes com DII em atividade (1,50±0,81), SII-D (1,50±0,94) e controles (1,63±0,9), sem significância estatística (p=0,76). Considerando o ponto de corte descrito na literatura de 1,56 para o anti-CdtB e 1,6 para anti-vinculina, a frequência de positividade foi, respectivamente: DII em atividade n= 3 (6,8%) e n= 21 (27,7%); DII em remissão n= 0 e n= 12 (48%); SII-D n=3 (6,6%) e n= 19 (42,2%); controles n= 5 (10,8%) e n= 23 (50%). Conclusões: Não foram encontradas diferenças estatisticamente significantes entre os níveis séricos e a frequência de anti-CdtB e anti-vinculina entre os grupos de DII em atividade, DII-SII, SII-D e controles. Entretanto, houve tendência de níveis mais elevados de anti-vinculina no grupo DII-SII

Background: Inflammatory bowel diseases (IBD), Crohn's Disease and Ulcerative Colitis, are chronic conditions associated with high morbidity and healthcare costs. Despite adequate treatment, a subgroup of IBD patients exhibits persistent gastrointestinal symptoms that are not always related to mucosal damage. In this setting, there is frequent association with Irritable Bowel Syndrome (IBS), especially when fecal calprotectin level is under 50 ug/g. IBS is still considered a functional disease with an estimate prevalence of 40% in IBD patients. ROME IV criteria have limited diagnostic accuracy to diagnose superimposed conditions. In 2015, two autoantibodies, anti-CdtB and anti-vinculin, were validated as post-infectious IBS markers, however there is limited evidence of its role in IBD population. Objetives: The primary aim of this study was to assess the clinical applicability of anti-CdtB and anti-vinculin antibodies to diagnose superimposed Irritable Bowel Syndrome in Inflammatory Bowel Disease patients and persistent diarrhea. The secondary aim was to investigate the prevalence of small intestine bacterial overgrowth in IBS patients. Methods: Patients were enrolled into 4 groups: (i) active IBD patients with chronic diarrhea (more than 3 bowel movements/day); (ii) quiescent IBD and chronic diarrhea IBD-IBS (defined by quiescent mucosal inflammation SES-CD <= 2 or Mayo score <= 1); (iii) IBS predominant diarrhea patients; (iv) healthy controls. Inclusion criteria were age over 18 years old and consent to participate in the study. Exclusion criteria were: other intestinal diseases, previous abdominal surgery or neoplasia, corticosteroid use less than 6 months prior to inclusion, HIV infection. All blood samples were collected at the time of colonoscopy and serum levels of anti-CdtB/anti-vinculin antibodies were determined by enzyme-linked immunosorbent assay. Results: 160 subjects were included and assigned into group i (n=44); group ii (n=25); group iii (n=45); group iv (n=46). The mean age of active IBD patients was 39.5 years, whereas 46.5 years in the overlapping IBD-IBS group, and 39.5 in the IBS-D group; 53.6% (37/69) of total IBD patients had Crohn`s disease. The mean value of the optical density for anti-CdtB was 0.71±0.48 in active IBD, 0.71±0.43 in quiescent IBD, 0.70±0.44 in IBS-D group and 0.85±0.72 in controls. There were no statistical differences observed between groups (p=0.95). IBD-IBS subjects had a trend to have higher levels of anti-vinculin (1.68±1.04) when compared to active IBD (1.50±0.81), IBS-D (1.50±0.94) and controls (1.63±0.9), without significance differences (p=0.76). Considering the cut-off point of 1.56 for anti-CdtB and 1.60 for anti-vinculin reported in literature, the frequency of seropositive cases were, respectively: active IBD n= 3 (6.8%) and n= 21 (27.7%); quiescent IBD n=0 and n= 12 (48%); IBS-D n=3 (6.6%) and n=19 (42.2%); controls n= 5 (10.8%) and n=23 (50%). Conclusions: We did not find distinct levels of either anti-CdtB or anti-vinculin among active or quiescent IBD, IBS-D and healthy controls. Trend for higher anti-vinculin antibodies titers was observed in IBD-IBS
