Introdução: O transplante de células de tronco hematopoiéticas (TCTH) é o único tratamento curativo atualmente disponível para a doença falciforme (DF) no Brasil. A Portaria do Ministério da Saúde (MS) do Brasil (2018) estabelece a inclusão do TCTH no Protocolo Clínico e Diretrizes Terapêuticas da DF, e determina que o SUS irá financiar este tratamento para pacientes com determinadas indicações. Objetivos: Essa tese é composta pela compilação de dois artigos com o objetivo geral de identificar e caracterizar os candidatos ao transplante em uma coorte de pacientes com DF no Brasil. Os objetivos específicos de cada artigo são: Artigo 1) Identificar, caracterizar e comparar os pacientes com 0 e 1 indicação de TCTH conforme as indicações da Portaria do MS e identificar quantos candidatos ao TCTH tinham irmãos sem DF. Artigo 2) aplicar um escore de gravidade para os participantes e avaliar se o escore se relaciona com as indicações de transplante. Métodos: Este é um estudo de corte transversal dos dados de recrutamento de uma coorte de 2793 pacientes, o Estudo Longitudinal Multicêntrico da doença falciforme no BrasilREDS-III. Artigo 1) O estudo comparou características clínicas e laboratoriais e de tratamento dos pacientes com 0 e 1 indicação para o TCTH conforme o MS, e identificou quantos tinham pelo menos um irmão sem doença falciforme (potencial doador para o transplante). Artigo 2) O estudo aplicou um escore de gravidade que foi desenvolvido com um Modelo bayesiano para predizer o risco de morte em 5 anos para um paciente com DF. O escore é dado pela Disease severity score calculator. O estudo comparou os escores dos participantes com e sem indicação para o transplante, e analisou a acurácia do escore em diferenciar esses dois grupos, através da curva ROC (Receiver Operating Characteristic). Resultados: Artigo 1) Dos 2063 pacientes do grupo anemia falciforme, 152 das 974 crianças (16%) e 279 dos 1090 adultos (26%) tinham pelo menos uma indicação para o TCTH. As indicações mais comuns foram: acidente vascular encefálico (n= 239); necrose avascular (n=96); priapismo (n=82), doença cerebrovascular (n=55); > 2 crises vasoclusivas no último ano (n=38); presença de aloanticorpos em pacientes sob programa de transfusão crônica (n=18) e > 2 episódios de síndrome torácica aguda no último ano (n=11). Das 152 crianças e 279 adultos candidatos ao transplante, 77 (50%) e 204 (70%), respectivamente, tinham pelo menos um irmão sem doença falciforme que poderia ser doador para o transplante. Artigo 2) As medianas dos escores foram significativamente mais altas no grupo com indicação de transplante em comparação com os sem indicação, para todas as idades. Quando o estudo foi estratificado entre adultos e crianças, os escores foram maiores em crianças sem indicação para o transplante, e não houve diferença entre os escores dos dois grupos de adultos. Quando visualizamos a distribuição dos escores nos box plots, o estudo não encontrou diferença entre os pacientes com e sem indicação para o transplante, de qualquer idade. A curva ROC não mostrou um escore que fosse um ponto de corte entre candidatos e não candidatos ao TCTH. Conclusões: em uma grande coorte de pacientes com doença falciforme, 16% das crianças e 26% dos adultos têm pelo menos uma indicação para o TCTH. O escore de gravidade não demonstrou acurácia em diferenciar os candidatos e não candidatos ao transplante. Este estudo mostrou dados importantes para estimar custos e infraestrutura que seriam necessários para implementar o transplante em um sistema público de saúde. Mostrou ainda que as indicações de transplante podem não refletir a gravidade da doença e o risco de morte dos pacientes.

Introduction: Hematopoietic stem cell transplantation (HSCT) is the only currently available curative treatment for sickle cell disease (SCD) in Brazil. The Brazilian Ministry of Health (MoH) Regulation (2018) establishes the inclusion of HSCT in the National therapeutic guidelines of SCD, and states that the Public Health System will pay for the procedure for patients under certain criteria. Objectives: This Thesis is a combination of two articles with the objective of identify and characterize candidates for transplant in a cohort of SCD patients in Brazil. The specific objectives are: Article 1) Identify, characterize and compare patients with 0 and 1 indications for HSCT according to MoH Regulation and identify how many candidates have full siblings without SCD. Article 2) To apply a severity score and evaluate if there is a correlation with transplant indications. Methods: This is a cross-sectional study of recruitment data of 2793 patients of the REDS-III Brazil SCD cohort (Recipient Epidemiology and Donor Evaluation Study-III). Article 1) The study compared clinical and laboratory characteristics of patients with 0 and 1 indications for HSCT according to MoH, identified how many had at least one sibling without SCD (potential donor for transplant) Article 2) A Bayesian network modeling was created to predict the risk of death in 5 years in a patient with SCD). This study applied this score through the Disease severity score calculator. It compared scores of patients with and without indications for HSCT, and analyzed the accuracy of the score in distinguish both groups of patients, with a ROC curve (Receiver Operating Characteristic). Results: Article 1) Of 2063 patients of the group sickle cell anemia, 152 of 974 children (16%) and 279 of 1090 adults (26%) had at least one indication for transplant. The most common indications were: stroke (n=239); avascular necrosis (n=96); priapism (n=82); cerebrovascular disease (n=55); > 2 vasocclusive episodes in the last year (n=38); presence of 18 alloantibodies in a patient under chronic transfusion program (n=18) and > 2 episodes of acute chest syndrome in the last year (n=11). Of 152 children and 279 adults who had at least one indication, 77 (50%) and 204 (70%), respectively, had at least one full sibling without SCD who could serve as a donor for transplant. Article 2) Median scores were significantly higher in the group with at least one indication, when compared to those without indication, for all ages. When stratified in age groups, scores were higher in the group of children without any indication, and there was not difference of scores in the two groups of adults. There was not an obvious difference between groups with 0 and 1 indication for transplant as shown in box plots, for all ages. ROC curve did not demonstrate an obvious cutoff of score that could distinguish candidates and non-candidates for transplant. Conclusions: in a large cohort of SCD patients, 16% of children and 26% of adults have at least one indication for HSCT. The severity score did not show accuracy to distinguish candidates from non-candidates for transplant. This study revealed important data to estimate costs and infrastructure that would be necessary to implement HSCT in a public health system. The study also showed that indications for transplant could not replicate the severity of the disease and the risk of death.
