A Doença Falciforme (DF) é uma doença hereditária comum no mundo. No Brasil aproximadamente 3000 crianças nascem com a doença a cada ano. A úlcera de perna é uma das complicações da Doença Falciforme. A proposta deste estudo foi realizar uma análise das variáveis, genótipo, tratamento atual de hidroxiureia (HU), histórico de uso de cigarro, histórico de consumo de álcool, escolaridade, trabalha atualmente, gênero, idade, cor da pele, estado civil, renda mensal, seguro de saúde, SNPs relacionados às úlceras de perna, terapia transfusional crônica atual, hemocentro, eventos clínicos, resultados laboratoriais, número de dias perdidos trabalho/escola por dor no último mês e suas relações com a presença de úlcera de perna em pessoas com Doença Falciforme maiores e menores de 18 anos e entre os diferentes genótipos. Este estudo visa buscar um melhor entendimento acerca da úlcera de perna nesse grupo de pacientes. Tratase de um estudo analítico observacional transversal, desenvolvido através do banco de dados do Estudo Multicêntrico da Doença Falciforme no Brasil REDSIII entre 2013 e 2017, com dados de quatro hemocentros públicos brasileiros: Fundação Hemominas (Minas Gerais), Fundação Hemope (Pernambuco) e Fundação Hemorio (Rio de Janeiro) e o ITACI Instituto de Tratamento do Câncer Infantil (São Paulo). O estudo foi desenvolvido em duas etapas: 1ª etapa - análise descritiva (frequência das variáveis qualitativas, média e mediana das idades) e análise comparativa (úlcera de perna x cada uma dessas variáveis); 2ª etapa - avaliação e comparação dos Single Nucleotide Polymorphisms (SNPs) de pacientes com e sem úlcera de perna. Analisamos os dados dos 2.779 pacientes inscritos na coorte de Doença Falciforme, do estudo REDSIII, comparando os que relataram história de úlcera de perna com os que nunca tiveram essa complicação em suas vidas. A prevalência de úlcera de perna em pacientes maiores de 18 anos com anemia falciforme foi de 21% (genótipos SS e Sb0- talassemia) vs. 4,5% em pacientes de outros genótipos. Nos pacientes menores de 18 anos essa prevalência foi de 0,3% entre pacientes com AF e não tivemos nenhum paciente nessa faixa etária, pertencente ao grupo dos outros genótipos com histórico de úlcera. A análise multivariada mostrou que quanto mais velho maior a chance de ter histórico de úlcera de perna (OR= 1,074 95%IC 1,059; 1,090, p < 0,001), o risco de histórico de úlcera é maior em indivíduos do sexo masculino (OR=1,760 95%IC 1,268; 2,450, p <0,001), morar no Rio de Janeiro é um fator protetor em relação a BH (OR= 0,409 95%IC 0,256; 0,646, p <0,001), pacientes brancos e pardos apresentam menores chances de ter úlcera de perna em relação aos pretos (brancos OR=0,199 95%IC 0,071; 0,472, p<0,001; pardos OR=0,588 95%IC 0,414; 0,835, p=0,002). Estar em regime de transfusão crônica, bem como apresentar bilirrubina indireta acima do percentil 75 e hemoglobina abaixo do percentil 25 são fatores de risco para histórico de úlcera de perna (transfusão crônica OR= 1,819 95%IC 1,310; 2,532, p > 0,001; bilirrubina indireta >75% OR=1,456 95%IC 1,008; 2,093, p<0,001; hemoglobina <25% OR=2,170 95%IC 1,520; 3,110, p<0,001). As análises genéticas sugeriram que o risco de úlcera de perna em pacientes com o SNP rs11800462, presente no cromossomo 1, cujo gene TNFRSF25 tem função de apoptose e homeostase linfocitária, é aumentado em 1,699 (95%IC 1,229; 2,349) em relação aos pacientes que não tem esse SNP. A UP apresentou maior prevalência em pacientes do sexo masculino, conforme o aumento da idade, em pacientes com marcador de hemólise presente, em regime de transfusão crônica, e uma menor prevalência em pacientes brancos e pardos quando comparado a pacientes pretos. Os níveis aumentados de bilirrubina e diminuídos da hemoglobina entre os pacientes com UP corroboram a hipótese que a hemólise está correlacionada com a ocorrência de UP. Estas características podem nos ajudar a identificar pacientes mais susceptíveis à UP e a implementar condutas preventivas nestes. Quanto à análise genética, um maior número de amostras assim comoo melhor entendimento desses SNPs, também podem colaborar para a identificação de pacientes susceptíveis à UP assim como a tratamentos futuros

Sickle cell disease is a frequent hereditary disease worldwide. In Brazil, approximately, 3000 children are born with the disease each year. Leg ulcers (LU) are one of the complications of sickle cell disease. The purpose of this study was to perform an analysis of the variables genotype, current hydroxyurea treatment, history of cigarette use, history of alcohol consumption, education, currently works, gender, age, skin color, marital status, monthly income, health insurance, genetic history, current chronic transfusion therapy, blood center, clinical events, laboratory results, number of days lost work/ school due to pain in the last month and its relationship with the presence of leg ulcers in people with sickle cell disease older and younger than 18 and between different genotypes. This study aims to improve the understanding about leg ulcers in this group of patients. This is a cross-sectional observational analytical study, developed through the database of the Multicenter Study of Sickle Cell Disease in Brazil REDSIII during 2017 and 2017, with data from four Brazilian public blood centers: Fundação Hemominas (Minas Gerais), Fundação Hemope(Pernambuco) and Fundação Hemorio (Rio de Janeiro) and ITACI Childhood Cancer Treatment Institute (São Paulo). The study was developed in two stages: 1st stage - descriptive analysis (frequency of qualitative variables, mean and median of ages) and comparative analysis (leg ulcer vs. each of these variables); 2nd stage - evaluation and comparison of Single Nucleotide Polymorphisms (SNPs) from patients with and without leg ulcers. We analyzed the data of 2,779 patients enrolled in the Sickle Cell Disease cohort in the REDSIII study, comparing those who reported a history of leg ulcers with those who never had this complication in their lives. The prevalence of leg ulcers in patients older than 18 years with sickle cell anemia was 21% (SS and Sb0- thalassemia genotypes) vs. 4.5% in patients of other genotypes. In patients under 18 years old, this prevalence was 0.3% among patients with SCA and we did not have any patients in this age group, belonging to the group of other genotypes with a history of ulcers. Multivariate analysis showed that the older the patient the greater the chance of having a history of leg ulcers (OR= 1.074 95%CI 1.059; 1.090, p < 0.001), the risk of a history of ulcers is higher among males (OR=1.760 95%CI 1.268; 2.450, p <0.001), living in Rio de Janeiro is a protective factor in relation to BH (OR= 0.409 95%CI 0.256; 0.646, p <0.001), white and mixed patients are less likely to have leg ulcers than blacks (white OR=0.199 95%CI 0.071; 0.472, p<0.001; mixed race OR=0.588 95%CI 0.414; 0.835, p=0.002). Being in chronic transfusion, as well as having pain, indirect bilirubin above the 75th percentile and hemoglobin below the 25th percentile are risk factors for a history of leg ulcer (chronic transfusion OR= 1.819 95%CI 1.310; 2.532, p > 0.001; indirect bilirubin> 75% OR=1.456 95%CI 1.008; 2.093, p<0.001; hemoglobin <25% OR=2,170 95%CI 1,520; 3,110, p<0,001). Genetic analyzes suggested that the risk of leg ulceration in patients with the SNP rs11800462, present on chromosome 1, whose TNFRSF25 gene has the function of apoptosis and lymphocytic homeostasis, is increased by 1.699 (95%CI 1,229; 2,349) in relation to patients who do not have this SNP.LU showed a higher prevalence in male patients, as age increases, in patients with the presence of hemolysis marker, in chronic transfusion regimen, and a lower prevalence in white and mixed patients when compared to black patients. The increased bilirubin level among participants with leg ulcers and decreased hemoglobin levels supports the hypothesis that hemolysis is correlated with leg ulcer pathogenesis. These characteristics can help us to identify patients most susceptible to LU and to implement preventive measures in them. As for genetic analysis, a larger number of samples as well as a better understanding of these SNPs can also contribute to the identification of susceptible patients and future treatments
