Objetivos: Diversos estudos avaliaram a significância prognóstica da invasão perineural (IPN) no câncer de boca e de laringe; entretanto, os resultados são inconclusivos. Portanto, conduzimos uma metanálise com o objetivo de identificar o valor prognóstico de IPN no câncer de boca e de laringe, separadamente. Métodos: Foi realizada uma revisão da literatura, com busca nas bases MedLine via Pubmed, Scielo, Lilacs, Cochrane e Websco. RevMan 5.4 foi utilizado para a análise estatística. Resultados: Um total de 19 estudos de câncer de laringe e um total de 56 estudos de câncer de boca foram incluídos. Os resultados indicam que IPN no câncer de boca tem uma incidência de 28% (IC 95% 24%- 31%); sobrevida em 5 anos com RR 0,67 (0,59- 0,75); sobrevida livre de doença em 5 anos RR 0,71 (0,68-0,75); recidiva locorregional RR 2,09 (1,86-2,35). IPN no câncer de laringe apresenta sobrevida em 2 anos com RR 0,71 (0,59- 0,86); sobrevida livre de doença RR 0,51 (0,14-1,95); e recidiva locorregional RR 1,71 (1,25-2,35). Conclusão: A IPN é um fator prognóstico negativo no câncer de boca e de laringe

Objectives: Many studies have examined the prognostic significance of perineural invasion (PNI) in oral cancer and larynx cancer; however, the results are inconclusive. We therefore conducted a meta-analysis aiming to clarify the prognostic value of PNI in oral and larynx cancer patients. Methods: A literature search was conducted in the MedLine via Pubmed, Scielo, Lilacs, Cochrane and Websco databases. RevMan 5.4 was used for statistical analysis. Results: A total of 19 studies of larynx cancer and 56 studies of oral cancer were finally included. The results indicated that PNI in oral cancer has an incidence of 28% (IC 95% 24%- 31%); 5 yeas OS impact has a RR 0,67 (0,59- 0,75); 5 years DFS RR 0,71 (0,68-0,75); locoregional recurence RR 2,09 (1,86-2,35). PNI in larynx cancer has an 2 yeas OS RR 0,71 (0,59- 0,86); 2 years DFS RR 0,51 (0,14-1,95); locoregional recurence RR 1,71 (1,25-2,35). Conclusion: PNI is a negative independent prognostic factor in patients with oral and larynx cancer
