As infecções profundas da ferida esternal (DSWI) após a cirurgia de revascularização do miocárdio (CRM) afetam a morbidade, o tempo de internação, além dos custos hospitalares. O objetivo deste estudo foi desenvolver um modelo preditivo para DSWI até 30 dias após CRM (DEWINSCORE) e avaliar o impacto no custo médio de uma internação com e sem o evento infecioso. O DEWINSCORE foi desenvolvido de um coorte prospetivo, multicêntrico de adultos submetidos à cirurgia de revascularização do miocárdio isolada como procedimento primário (Estudo REPLICCAR II). Foi realizada uma regressão logística multivariada de LASSO, validada interna e externamente, comparando discriminação, calibração in-the-large (CL), melhoria de reclassificação (NRI) e melhoria de discriminação integrada (IDI), avaliadas entre o novo modelo DEWINSCORE e o padrão ouro para DSWI após cirurgia cardíaca, o STS PredDeep. A avaliação de microcusteio foi realizada em amostra pareada e risco, ajustada por escore de propensão. O DEWINSCORE atingiu AUROC de 0,81 (IC 95% 0,770,86) na validação interna. Nos dados de validação externa, a AUROC foi de 0,83 (IC 95% 0,720,95). Em comparação com o modelo STS PredDeep, o modelo teve um NRI de 29% e uma melhora de predição de 6,5% (IDI). No entanto, tanto o STS PredDeep quanto o DEWINSCORE tiveram baixa calibração na amostra de validação externa. Ainda, a variação de custo médio demonstrou um aumento de 180% para o custo de uma infecção profunda e de 291% para uma mediastinite. Desenvolvemos um modelo de risco robusto que pode ser aplicado como ferramenta de melhoria da qualidade para orientar os profissionais de saúde a atuarem de forma proativa na gestão de custo, qualidade e segurança, além de prevenção de DSWI após cirurgia de revascularização do miocárdio

Deep sternal wound infections (DSWI) after coronary artery bypass grafting (CABG) frequently affect morbidity, length of stay and hospital costs. We develop a predictive model for 30-day DSWI after CABG (DEWINSCORE) and evaluate the economic impact on costs related to DSWI using propensity match and risk adjusted. The DEWINSCORE model was created through a multicenter cohort of adults undergoing CABG surgery (REPLICCAR II Study) database, using least absolute shrinkage and selection operator (LASSO) logistic regression, internally and externally validated comparing discrimination, calibration in-the-large (CL), net reclassification improvement (NRI) and integrated discrimination improvement (IDI), trained on, between the new model and a gold standard for DSWI after cardiac surgery, the Society of Thoracic Surgeons (STS) PredDeep. The DEWINSCORE model achieved c-index of 0.81 (95% CI 0.770.86) for internal validation. In the validation data, c-index = 0.83 (95% CI 0.720.95). Compared to the STS PredDeep model, predictions improved by 6.5% (IDI). However, both STS PredDeep and DEWINSCORE had poor calibration. Still, compared to control the cost variation increased 180% for a DSWI and 291% for Mediastinits. We have developed a robust risk model that can be applied as a cost management and quality improvement tool to guide healthcare professionals to proactively act in prevention of DSWI after CABG surgery
