INTRODUÇÃO: Nos últimos anos, tem havido uma polarização em torno do manejo cervical em pacientes com neoplasia maligna da cavidade oral, sem evidência de comprometimento linfonodal. A discussão baseia-se entre esvaziamento cervical seletivo (ECS) dos níveis I, II e III, biópsia de linfonodo sentinela (BLS) e apenas vigilância. A BLS parece manter os mesmos resultados oncológicos que o ECS com menor morbidade. No entanto, o efeito real da BLS na qualidade de vida dos pacientes permanece desconhecido. OBJETIVOS: Avaliar a qualidade de vida tardia e descrever os resultados oncológicos no uso de biópsia de linfonodo sentinela como parte do tratamento cirúrgico do carcinoma epidermoide T1 / T2N0 da cavidade oral em comparação aos resultados de pacientes submetidos ao ECS dos níveis I, II e III. MÉTODOS: Houve duas fases do estudo com o mesmo grupo de pacientes. A primeira fase foi um estudo prospectivo no qual setenta pacientes foram divididos em dois grupos, sendo 35 submetidos ao ECS e os outros 35 à BLS para descrever os resultados oncológicos. Na segunda fase, após 36 meses de acompanhamento, foram entrevistados 24 pacientes, sendo que 15 deles haviam sido submetidos à BLS e nove ao ECS dos níveis I, II e III, para avaliar a qualidade de vida. RESULTADOS: No grupo ECS, recidiva loco-regional ocorreu em 17,1%, com desenvolvimento de 2,9% de metástase à distância e 8,8% evoluiu com um segundo tumor primário. Na BLS, a recidiva loco-regional foi observada em sete pacientes (20,0%), 5,7% desenvolveram metástases à distância e 5,7% tiveram um segundo tumor primário. Não houve diferença significativa entre os dois grupos quanto a sobrevida global (p=0,521) e sobrevida livre de doença (p=0,753). Em relação à qualidade de vida, a avaliação mostrou piora tardia dos domínios: aparência (p=0,035) e mastigação (p=0,041), bem como diminuição de cerca de 10% da qualidade de vida geral (p=0,025) em pacientes submetidos ao ECS em comparação àqueles submetidos à BLS. CONCLUSÕES: A BLS é um método confiável para o manejo do pescoço clinicamente negativo em pacientes com carcinoma epidermoide da cavidade oral T1 / T2N0 e pacientes submetidos à biópsia de linfonodo sentinela apresentaram melhores resultados tardios de qualidade de vida geral, principalmente quanto à aparência e mastigação quando comparados aos pacientes submetido a esvaziamento cervical seletivo

INTRODUCTION: In recent years, there was a polarization around cervical management in patients with malignant neoplasm of the oral cavity, with no evidence of lymph node involvement. The discussion is based on levels I, II, III selective neck dissection (SND), sentinel lymph node biopsy (SLB) and just surveillance. The SLB seems to maintain the same oncologic results as SND with lower morbidity. However, the actual effect of SLB on patients' late quality of life remains unknown. OBJECTIVES: To evaluate the quality of life and to describe oncological results of sentinel lymph node biopsy as part of the surgical treatment of oral cavity squamous cell carcinoma T1 / T2N0 compared to the results of patients submitted to levels I, II and III SND. METHODS: There were two phases of the study with the same group of patients. The first phase was a prospective study in which seventy patients were divided into two groups, 35 being submitted to SND and the other 35 to SLB to describe the oncological results. In the second phase, after 36 months of follow-up, 24 patients were interviewed, 15 of them have been submitted to SLB and nine to levels I, II, III SND to evaluate quality of life. RESULTS: In the SND group, locoregional recurrence occurred in 17.1%, with a 2.9% development of distant metastasis and 8.8% developed a second primary tumor. In SLB, locoregional recurrence was observed in seven patients (20.0%), 5.7% developed distant metastases, and 5.7% had a second primary tumor. There was no significant difference between the two groups for overall survival (P=.521) and disease-free survival (P=.753). Regarding quality of life, the evaluation showed a late worsening of the domains: appearance (P=.035) and chewing (P=.041), as well as a decrease of about 10% in general quality of life (P=.025) to SND compared to those submitted to SLB. CONCLUSIONS: BLS is a reliable method for clinically negative neck management in patients with squamous cell carcinoma of the oral cavity T1 / T2N0 and patients submitted to sentinel lymph node biopsy had better late results of general quality of life, mainly regarding appearance and chewing when compared to patients submitted to selective neck dissection
