Introdução: Há muitas incertezas e desconhecimentos acerca dos efeitos fisiológicos do voo sobre o organismo. Vem sendo observados quadros recorrentes de sinusopatia em pilotos de caça, porém, há escassos relatos publicados na literatura sobre o referido tema. Objetivo: O objetivo desta investigação foi avaliar e documentar os possíveis efeitos fisiológicos do voo sobre os seios paranasais em pilotos de caça da Força Aérea Brasileira, fazendo um estudo prospectivo e longitudinal, correlacionando achados tomográficos dos seios paranasais, bem como, o volume dos seios maxilares e relacionando esses achados com as variáveis de voo. Materiais e métodos: Para tanto, foram mensurados o volume dos seios maxilares, as alterações inflamatórias em todos os seios paranasais, as alterações anatômicas do complexo ostiomeatal, e alterações do septo nasal em pilotos de caça antes e após o programa de treinamento (com duração de 8 meses) utilizando tomografia computadorizada multidectores, considerando os efeitos de altitude de cabine, altitude de voo e horas de voo. Os seios maxilares foram selecionados para a verificação volumétrica por apresentarem características anatômicas mais favoráveis e menos variações anatômicas quando comparados aos demais seios paranasais e, consequentemente, poder evidenciar melhor as alterações fisiológicas provocadas pelo voo. Foram formados dois grupos para comparação e correlação estatística: o grupo dos pilotos de caça (15) e o grupo-controle (41). Ambos os grupos foram formados por jovens de 20 a 30 anos de idade cronológica, gênero masculino, sem comorbidades prévias. Para mensuração da volumetria dos seios maxilares, foi utilizado o software 3D Slicer 4.10, utilizando-se a técnica semiautomática com mensuração da volumetria total dos seios maxilares em centímetros cúbicos (cc). Concomitantemente, foi realizada análise das alterações anatômicas do complexo ostiomeatal, bem como, desvio de septo nasal nas tomografias iniciais e finais, e as principais alterações inflamatórias dos seios paranasais. Os resultados de variáveis quantitativas foram descritos por médias, desvios padrões, medianas, valores mínimos e máximos. Valores de p < 0,05 indicaram significância estatística. Os dados foram analisados com o programa computacional Stata/SE v.14.1. StataCorpLP, USA. Resultados: Observaram-se diferenças volumétricas (aumento) entre a avaliação inicial e final dos seios maxilares em ambos os seios maxilares no grupo dos pilotos quando comparado ao grupo-controle, com resultados estatísticos significativos. Quanto às alterações inflamatórias dos seios paranasais, houve aumento do espessamento mucoso nos seios maxilares e etmoidal anterior no grupo dos pilotos. Não foram observadas alterações do septo nasal neste período e as principais alterações anatômicas no grupo de estudo foram: célula de Agger nasi, desvio de septo nasal, curvatura paradoxal da concha média, alterações do processo uncinado e concha média bolhosa. Conclusões: Para o aumento do volume dos seios maxilares, algumas das hipóteses relacionadas são as mudanças de força G, a expansão do gás de acordo com a Lei de Boyle e a pressão positiva da máscara de oxigênio. Em relação às alterações inflamatórias dos seios paranasais, as hipóteses relacionadas são as mudanças de força G, alterações da pressão atmosférica e redução da umidade da máscara de voo

Introduction: There are many uncertainties and unknowns about the physiological effects of flight on the organism. Recurrent cases of sinus disease have been observed in fighter pilots, however, there are few reports published in the literature about this subject. Objective: The objective of this investigation was to evaluate and document the possible physiological effects of flight on the paranasal sinuses in fighter pilots of the Brazilian Air Force, making a prospective and longitudinal study, correlating tomographic findings of the paranasal sinuses, as well as the volume of the maxillary sinuses and relating these findings to flight variables. Materials and Methods: For this purpose, maxillary sinus volumes, inflammatory changes in all paranasal sinuses, anatomical changes in the ostiomeatal complex and nasal septum changes in fighter pilots were measured before and after the training program (lasting 8 months) using multidetector computed tomography, considering the effects of cabin altitude, flight altitude and flight hours. The maxillary sinuses were selected for volumetric verification for the reason that they present more favorable anatomical characteristics and less anatomical variations when compared to the other paranasal sinuses and, therefore, to be able to better demonstrate the physiological changes caused by the flight. Two groups were formed for comparison and statistical correlation: the fighter pilot group (15) and the control group (41). Both groups were formed by young men of 20-30 years of age, male, without previous comorbidities. To measure the volumetry of the maxillary sinuses, the 3D Slicer 4.10 software was used, using the semi-automatic technique with the measurement of the total volumetry of the maxillary sinuses in cubic centimeters (cc). Concomitantly, an analysis of the anatomical changes of the ostiomeatal complex was performed, as well as deviation of the nasal septum in the initial and final CT scans and the main inflammatory changes in the paranasal sinuses. The results of quantitative variables were described by means, standard deviations, medians, minimum and maximum values. Values of p < 0.05 indicated statistical significance. The data were analyzed with the computer program Stata / SE v.14.1. StataCorpLP, USA. Results: Volumetric differences (increase) were observed between the initial and final evaluation of the maxillary sinuses in both maxillary sinuses in the pilot group when compared to the control group, with significant statistical results. As for inflammatory changes in the paranasal sinuses, there was increased mucous thickening in the maxillary and anterior ethmoidal sinuses in the group of pilots. No changes in the nasal septum were observed in this period and the main anatomical changes in the study group were: Agger nasi cell, deviated nasal septum, paradoxical curvature of the middle concha, changes in the uncinate process and bullous middle concha. Conclusions: For the increase in the volume of the maxillary sinuses, some of the related hypotheses are changes in the G force, the expansion of the gas according to Boyle's law and the positive pressure of the oxygen mask. Regarding inflammatory changes in the paranasal sinuses, the related hypotheses are changes in G-force, changes in atmospheric pressure and reduced humidity of the flight mask
