INTRODUÇÃO: A esteatose hepática é uma doença metabólica caracterizada por acúmulo de gordura nos hepatócitos. Dentre suas causas, destacam-se o alcoolismo crônico, a obesidade, o diabetes mellitus do tipo II e a síndrome metabólica. O interesse pela detecção e quantificação da esteatose hepática tem crescido nos últimos anos devido à elevação da sua prevalência e aos avanços da técnica cirúrgica hepática. A quantificação da infiltração gordurosa hepática é importante para o acompanhamento do tratamento de pacientes com esteatose alcoólica e não alcoólica e para a avaliação pré-operatória de transplantes hepáticos com doadores vivos e de hepatectomias parciais. O diagnóstico pode ser feito por diversos estudos de imagem, como a ultrassonografia (US), a tomografia computadorizada (TC) e a ressonância magnética (RM), mas o método consagrado para a sua quantificação é a biópsia hepática. Caso a TC se provasse um método confiável para a realização dessa quantificação, poderia ser utilizada como alternativa à biópsia, minimizando custos e eliminando os riscos inerentes a esse procedimento. OBJETIVO: O objetivo deste estudo é avaliar a confiabilidade da tomografia computadorizada na quantificação da esteatose hepática, comparando seus resultados com os achados histopatológicos. MÉTODOS: Foi realizado um estudo transversal prospectivo e retrospectivo com 64 pacientes submetidos à biópsia hepática e TC do abdome em intervalo de até seis meses. A esteatose hepática foi estratificada em graus 0 (ausente), 1 (de 1% a 33%), 2 (de 34% a 66%) e 3 (superior a 66%) na análise histopatológica. A partir da avaliação dos exames de TC, foram obtidos três índices de atenuação hepática para cada paciente: a razão da atenuação hepática para a esplênica, a diferença da atenuação hepática da esplênica e a atenuação hepática livre do componente relacionado ao sangue, definidos como IF/B, IF-B e IPH. Esses índices foram correlacionados com o grau de esteatose hepática evidenciado na análise histopatológica, por meio do coeficiente de correlação de Spearman, do teste t- Student, da curva de estatísticas operacionais (ROC), do teste de análise de variância (ANOVA), do teste de Bonferroni e do modelo de regressão ordinal. RESULTADOS: A análise estatística dos dados obtidos no presente estudo evidenciou uma relação moderada significativa e negativa entre os índices de esteatose calculados com os dados dos exames de TC e os graus de infiltração gordurosa hepática observados na análise histopatológica. Foi possível, ainda, determinar valores de corte dos índices que permitissem o diagnóstico de esteatose hepática graus 2 ou 3 (de 33% ou superior) com sensibilidade e especificidade adequadas. Entretanto, o desempenho dos índices não foi satisfatório para a discriminação de cada grau de infiltração gordurosa hepática. CONCLUSÔES: A TC é um método que permite a quantificação da esteatose hepática, desde que abaixo (graus 0 ou 1) ou acima de 33% (graus 2 ou 3), mas não é adequada para a discriminação de cada grau isoladamente

INTRODUCTION: Hepatic steatosis is a metabolic disease characterized by accumulated fat in hepatocytes. Among its causes, chronic alcoholism, obesity, type II diabetes mellitus and syndrome metabolic. The interest in detecting and quantifying hepatic steatosis has grown in the past years due to higher prevalence and advances in hepatic surgical technical. Quantifying hepatic fat infiltration is important to follow up treatment of patients with alcoholic and nonalcoholic steatosis and for preoperative assessment of living donor hepatic transplants and of partial hepatectomies. The diagnosis can be made by diverse imaging studies, such as ultrasonography (US), computed tomography (CT) and magnetic resonance (MRI), but the most used method for quantifying is hepatic biopsy. If CT proved to be a reliable method for quantification, it could be used as an alternative to biopsy, minimizing costs and eliminating the risks that are inherent to this procedure. OBJECTIVE: The purpose of this study is to assess reliability of computed tomography in quantifying hepatic steatosis, comparing its results with histopathological findings. METHODS: A prospective and retrospective cross-section study was carried out with 64 patients submitted to hepatic biopsy and abdominal CT in intervals of up to six months. Hepatic steatosis was stratified as grade 0 (absent), 1 (1 to 33%), 2 (34 to 66%) and 3 (over 66%) upon pathological analysis. Based on CT evaluation, three liver attenuation indices were obtained for each patient: the liver to spleen attenuation ratio; the difference between hepatic and splenic attenuation and the blood-free hepatic parenchymal attenuation, defined as CTL/S, CTL-S and CTLP. These indices were correlated to the grade of hepatic steatosis shown on pathology by the Spearman correlation coefficient, Students t test, receiver operating characteristic (ROC) curve, analysis of variance (ANOVA), Bonferroni test and the ordinal regression model. RESULTS: The statistical analysis of data gathered in this study showed a significant moderate and negative relation between the steatosis indices, calculated with CT examination data and the hepatic fat infiltration grades observed upon pathological analysis. It was also possible to determine the cutoff values for the indices that enabled diagnosis of hepatic steatosis grades 2 or 3 (33% or over) with appropriate sensibility and specificity. However, the performance of the indices was not satisfactory to discriminate each grade of hepatic fat infiltration. CONCLUSIONS: CT is a method that allows quantifying hepatic steatosis if below (grades 0 or 1) or above 33% (grades 2 or 3), but it is not adequate to discriminate each grade separately
