Introdução: na blefaroplastia inferior, uma das etapas mais desafiadoras é a determinação do volume de gordura a ser ressecado. Durante o exame do paciente, a gordura que se projeta anteriormente retrai quando ele está deitado no ato operatório, dando a falsa impressão de menor volume. Assim seria desejável um método que permitisse estimar, no pré-operatório, o volume de bolsas de gordura a ser retirado para que se corrija as bolsas palpebrais de forma adequada. Diversos instrumentos têm sido utilizados para aferir volume no corpo, dentre eles a fotografia 3D, que apresenta como vantagens o fato de não ser invasiva, não gerar irradiação, ser portátil, apresentar relativo baixo custo e possuir software que possibilita fazer simulações do resultado. Objetivo: comparado os resultados entre as simulações obtidas pelo software da câmera 3D Vectra® H1 com os dos pós-operatórios de blefaroplastia inferior após a retirada da quantidade de volume indicado pela simulação. Método: utilizou-se a câmera fotográfica 3D Vectra® H1 (Canfield Imaging Systems, Fairfield, NJ, EUA) no pré-operatório para se estimar o volume de bolsas de gordura de pálpebra inferior a ser retirado. Realizou-se a blefaroplastia inferior pela ressecção do volume estimado pela simulação. Foram obtidas fotografias 3D nos retornos do pós-operatório para avaliar a variação volumétrica na pálpebra inferior após o procedimento e compará-la com o volume estimado no pré-operatório. O resultado estético das fotografias da simulação e do pósoperatório foi analisado pelo questionário objetivo de classificação de resultado em cirurgias estéticas. Os resultados do pós-operatório foram correlacionados estatisticamente com os da simulação. Resultados: dados demográficos da amostra foram apresentados e análises estatísticas da correlação entre os resultados da simulação e do pós-operatório foram realizadas. A correlação se mostrou fraca entre os resultados de simulação e os do pós-operatório. Discussão: verificou-se diferença sistemática entre as medidas da simulação e das fotografias pós-operatórias, observando-se um volume aferido da pálpebra inferior maior nos pós-operatórios em comparação ao volume simulado. Os resultados estéticos avaliados pelo questionário aplicado se mostraram melhores na simulação do que no pós-operatório real. Conclusão: a baixa correlação entre a simulação e os resultados pós-operatório sugerem que a acurácia do simulador da câmera 3D Vectra® H1 é insuficiente para simulações fidedignas nas blefaroplastias inferiores pelo método proposto no presente estudo

Introduction: The amount of fat to be removed from the lower eyelid is challenging because the fat that is shown in excess with the patient standing or sitting retracts into the orbital cavity with the patient lying in the surgery. Therefore, it would be desirable to estimate in the preoperative period the volume of fat removed necessary to correct the eyelid bags, avoiding excess removal and obtaining an adequate result. Several instruments have been used to measure body volume, among them 3D photography has advantages such as the fact that it is not invasive, there is no radiation, it is portable, it has a relatively low cost and it has software that allows simulations of the result. Objective: To analyze the correlation between the simulations of results obtained by the 3D Vectra® H1 camera software and the postoperative of lower blepharoplasty, removing the amount of volume obtained by the simulation. Method: The 3D Vectra® H1 camera (Canfield Imaging Systems, Fairfield, NJ, USA) was used preoperatively to estimate the volume of lower eyelid fat to be removed. Lower blepharoplasty was performed, removing the volume estimated by the simulation. 3D photographs were taken on postoperative returns to assess the reduced volume in the lower eyelid after the procedure and compare it with the estimated volume in the preoperative period. The aesthetic result of the simulation and the postoperative period was evaluated by the objective questionnaire for classification of results in cosmetic surgeries. Results: Demographic data from the sample were presented and statistical analyzes of the correlation between the simulation and the postoperative results were performed. The correlation was weak for the simulation method and the results in the postoperative period. Discussion: There was a systematic difference between the measurements of the simulation and the post-operative photographs, observing a higher volume of the lower eyelid in the postoperative period compared to the simulated volume. The correlation found between the simulation and the postoperative results proved to be weak. The aesthetic results evaluated by the questionnaire applied were better in the simulation than in the postoperative period. Conclusion: The low level of agreement observed suggests the accuracy of the tested software is insufficient for reliable simulations
