OBJETIVO: Este estudo comparou a penetração de riboflavina em córneas porcinas utilizando duas soluções diferentes: dextran e hidroxipropilmetilcelulose (HPMC). MÉTODOS: Foi realizada a deturgescência das córneas suínas e depois preparadas em câmara anterior artificial preenchida com solução salina balanceada. A pressão da câmara artificial foi verificada com um tonômetro portátil. As córneas porcinas receberam riboflavina a 0,1%, em solução de dextran a 20%, por 30 minutos (protocolo de Dresden) ou riboflavina a 0,1%, em solução de HPMC a 1%, por 10 minutos. A intensidade de fluorescência captada pela microscopia multifóton foi utilizada para avaliar a concentração da riboflavina a 0,1% na córnea. A espessura da córnea foi avaliada pela paquimetria ultrassônica antes e após o uso de riboflavina a 0,1%. RESULTADOS: Não houve diferença estatisticamente significante entre os grupos quando considerada toda a espessura corneana estudada (p = 0,528). A concentração de riboflavina até 340 ?m foi maior no grupo HPMC (p = 0,146). De 340 ?m a 500 ?m, a concentração foi maior no grupo de dextran (p = 0,248). As medidas paquimétricas no grupo com dextran mostraram diminuição de 12,9% na espessura da córnea após 30 min, enquanto o grupo HPMC apresentou aumento de 9,1% após 10 min. CONCLUSÃO: Riboflavina a 0,1% em solução de HPMC a 1% por 10 minutos apresentou o mesmo nível de segurança e eficácia na saturação da córnea quando comparado ao protocolo de Dresden, logo penetra mais rápido. A paquimetria ultrassônica no crosslinking do colágeno corneano tem importância fundamental no controle da hidratação da córnea devido as alterações na espessura causadas pelos diferentes tipos de solução de riboflavina. Este estudo utilizou métodos diferentes dos anteriores para melhor simular as condições fisiológicas da córnea e também utilizamos solução HPMC 1% que difere dos estuos prévios feitos com HPMC 1,1%

PURPOSE: This study compared riboflavin penetration in porcine corneas using two different solutions: dextran and hydroxypropylmethylcellulose (HPMC). METHODS: Porcine corneas were de-swollen and after prepared in an artificial anterior chamber with balanced saline solution. The pressure was checked with a portable tonometer. Porcine corneas received 0.1% riboflavin in 20% dextran for 30 minutes (Dresden protocol) or 0.1% riboflavin in 1% HPMC for 10 minutes. By multiphoton microscopy, the fluorescence intensity was used to evaluate concentration into the cornea, and ultrasonic pachymetry was performed before and after using riboflavin. RESULTS: There was no statistically significant difference between the groups when considering all the corneal thickness studied (p 0.528). Riboflavin concentration up to 340 ?m was higher in the HPMC group (p 0.146). From 340 ?m to 500 ?m the concentration was higher in the dextran group (p 0.248). Pachymetric measurements in the group with dextran showed a decrease of 12.9% in corneal thickness after 30 min, while the HPMC group showed an increase of 9.1% after 10 min. CONCLUSION: 1% HPMC riboflavin for 10 min had the same level of safety and effectiveness in soaking when compared to the Dresden protocol, so it penetrates the cornea faster during soaking. Ultrasonic pachymetry in corneal collagen crosslinking has fundamental importance in controlling corneal hydration due to changes in thickness caused by different types of riboflavin. This study used different methods than previous studies to better simulate corneal physiological conditions and used 1% HPMC concentration which differs previous studies performed with 1.1% HPMC
