Existem poucos estudos sobre injúria renal aguda (IRA) em pacientes com febre amarela (FA). O objetivo deste estudo foi identificar os fatores de risco para IRA e óbito nesses pacientes. Para tanto, foram avaliados 95 pacientes adultos gravemente enfermos com a forma silvestre da FA, confirmada pela reação em cadeia da polymerase (PCR) e/ou sorologia, no Brasil. Os desfechos foram IRA (conforme definido pelos critérios Kidney Disease Improving Global Outcomes) e morte intra-hospitalar. Dos 95 pacientes, 73 (76,8%) apresentaram IRA e 59 no total (62,1%) morreram. Setenta pacientes (73,7%) necessitaram de diálise. Ajustando para idade, sexo e SAPS 3 (Simplified Acute Physiology Score 3), foi observado que a fração de excreção de sódio elevada e a necessidade de diálise foram fatores de risco independentes para mortalidade hospitalar e que a proteinúria se correlacionou com a mortalidade quando na presença de IRA. Além disso, entre os pacientes gravemente enfermos com FA, a IRA pode ser atribuída principalmente ao choque, à insuficiência hepática, podendo também estar associada aos efeitos diretos do vírus da FA no tecido renal. Estes achados indicam que, em tais pacientes, a incidência de IRA é alta e que a IRA está associada a um aumento significativo da mortalidade. Este estudo fornece informações para melhor compreensão da patogênese da IRA na FA e pode auxiliar na tomada de decisões sobre o cuidado dos pacientes

There have been few studies investigating acute kidney injury (AKI) in patients with yellow fever (YF). The objective of this study was to identify the risk factors for AKI and death in such patients. To that end, we evaluated 95 consecutive critically ill adult patients with the sylvatic form of YF, as confirmed by reverse transcriptionpolymerase chain reaction or serology test, in Brazil. The outcome measures were AKI (as defined by Kidney Disease Improving Global Outcomes criteria) and inhospital death. Of the 95 patients, 73 (76.8%) had AKI and 59 (62.1%) died from it. Seventy patients (73.7%) required dialysis because of AKI. After adjusting for age, sex, and the Simplified Acute Physiology Score 3, we found that elevated fractional excretion of sodium and requiring dialysis were independent risk factors for inhospital mortality and that proteinuria correlated with AKI-associated mortality. We also found that, among critically ill patients with YF, AKI is mainly attributable to shock, liver failure, and the effects of YF viruses on kidney tissue. Our findings indicate that, in such patients, the incidence of AKI is high and that AKI is associated with significant mortality, as well as providing useful information to improve understanding of the pathogenesis of AKI in YF and informing decisions regarding the care of the affected patients
