A patogênese da nefropatia do envelhecimento ainda não foi elucidada. A Angiotensina-II (AngII) intrarrenal e a ativação das vias do inflamassoma NF-B e NLRP3 exercem papel patogênico relevante na progressão da doença renal crônica. Procuramos investigar se a monoterapia com Losartan e o tratamento combinado com Losartan e o inibidor de NF-B pirrolidina ditiocarbamato (PDTC) atenuariam a nefropatia do envelhecimento. Ratos Munich-Wistar machos foram distribuídos em quatro grupos: 12M (n=10), ratos com 12 meses de idade; 15M (n=10), ratos de 15 meses de idade; 15MLos (n=10), ratos recebendo Losartan (50 mg/d) e 15MLos+PDTC (n=10), ratos recebendo Losartan e PDTC (15 mg/d). Todos os tratamentos foram administrados por via oral dos 12 aos 15 meses de idade. O grupo 15M exibiu uma leve diminuição da pressão caudal e um aumento acentuado da albuminúria, glomérulos esclerosados, deposição cortical de colágeno-1 e infiltração por miofibroblastos, células AngII-positivas e macrófagos M1 pró-inflamatórios, enquanto a quantidade de macrófagos M2 anti-inflamatórios se mostrou reduzida. Além disso, a abundância renal de TLR4, p65 nuclear e IL-6 estava aumentada, indicando ativação da via NF-B, sem evidência de ativação simultânea da cascata NLRP3. O tratamento com Losartan diminuiu a deposição de colágeno-1 cortical, miofibroblastos e células positivas para AngII e restaurou parcialmente o M2 renal, mas não teve efeito significativo sobre a albuminúria, a glomeruloesclerose ou a ativação de NF-B. O tratamento combinado Losartan+PDTC preveniu todas as anormalidades observadas. O bloqueio da AngII renal associado à inibição da via NF-B pode ser uma alternativa para limitar o declínio da função renal com a idade

The pathogenesis of aging nephropathy is yet to be elucidated. Intrarenal Angiotensin-II (AngII) and activation of the NF-B and NLRP3 inflammasome pathways exert a relevant pathogenic role in the progression of chronic kidney disease (CKD). We sought to investigate whether monotherapy with Losartan and combined treatment with Losartan and the NF-B inhibitor pyrrolidine dithiocarbamate (PDTC) would attenuate experimental aging nephropathy. Male Munich-Wistar rats were distributed among four groups: 12M (n=10), 12-month-old rats; 15M (n=10), 15-month-old rats; 15MLos (n=10), rats receiving Losartan (50 mg/d) and 15MLos+PDTC (n=10), rats receiving Losartan and PDTC (15 mg/d). All treatments were given by mouth from 12 to15 months of age. Group 15M exhibited slightly decreased tail-cuff pressure, and marked increase in albuminuria, sclerotic glomeruli, cortical collagen-1 deposition and infiltration by myofibroblasts, AngII-positive cells and proinflammatory M1 macrophages, whereas the amount of anti-inflammatory M2 macrophages was reduced. In addition, the renal abundance of TLR4, nuclear p65 and IL-6 was increased, indicating activation of the NF-B pathway, without evidence of simultaneous activation of the NLRP3 cascade. Losartan treatment decreased cortical collagen-1 deposition, myofibroblasts and AngII-positive cells, and partially restored renal M2, but had no significant effect on albuminuria, glomerulosclerosis or NF-B activation. Combined Losartan+PDTC prevented all the observed abnormalities. Concomitant blockade of renal AngII and inhibition of the NF-B pathway can be an alternative to limit the decline of renal function with age
