INTRODUCTION: The non-specific hyperreactivity of rhinitis has been attributed to neurotrophins activating sensory nerves and inflammatory cells. The relationship of these markers with the intensity of symptoms is not well established and few studies have evaluated individuals with idiopathic rhinitis (IR). The aim of this study is to assess whether individuals with allergic rhinitis (AR) have a greater amount of nerve growth factor (NGF), nerve fibers, eosinophils and mast cells than those with IR, whether the intensity of symptoms is associated with such characteristics and describe the distribution of NGF and nerve fibers in the nasal mucosa. MATERIALS AND METHODS: 15 patients with AR and 15 patients with IR with indication for inferior turbinectomy were selected, associated or not with septoplasty. The patients underwent a score of signs and symptoms. After surgery, we quantified eosinophils, mast cells, NGF and nerve fibers in the nasal turbinate. RESULTS: The score of the symptom sign was higher in the AR group, mainly due to the symptoms of sneezing/pruritus, regarding the color of the nasal turbinate and the presence of nasal discharge. NGF was found in cytoplasmic granules of inflammatory cells in the submucosa in greater quantity in the AR group. The number of mast cells was higher in the group with IR and there was no difference regarding eosinophils. The nerve fibers were distributed throughout the tissue, mainly in the subepithelial, glandular and vascular regions and there was no difference between the groups. Greater perivascular innervation was associated with higher signs and symptoms score. CONCLUSIONS: Individuals with AR have a greater amount of NGF in the nasal mucosa when compared to those with IR. Those with IR have a greater number of mast cells. NGF is found in inflammatory cells in the submucosa and nerve fibers is found in the periglandular and perivascular region. Perivascular innervation in the submucosa of the nasal turbinate is directly related to the severity of rhinitis, in both IR and AR
