A depressão mista é uma condição peculiar em termos de curso clínico e resposta ao tratamento. A estimulação magnética transcraniana repetitiva (rTMS) está bem estabelecida no tratamento da depressão não-mista, e a modalidade de estimulação theta-burst (TBS) vem substituindo os protocolos convencionais devido à não inferioridade em termos de eficácia e ao tempo de aplicação reduzido. No entanto, a TBS ainda não foi adequadamente estudada em estados mistos. Realizamos um ensaio clínico randomizado, duplo-cego, controlado por estimulação simulada, com 6 semanas de duração, de TBS bilateral aplicada sobre o córtex pré-frontal dorsolateral (CPFDL) direito e esquerdo, respectivamente. Adultos com transtorno bipolar e transtorno depressivo maior que experimentavam um episódio depressivo maior com características mistas foram elegíveis desde que não tivessem se beneficiado de um tratamento de primeira ou segunda linha prévio para depressão unipolar ou bipolar recomendado pela Rede Canadense de Tratamentos de Humor e Ansiedade (CANMAT). Dos 100 pacientes incluídos, 90 compuseram a amostra por intenção de tratamento modificada (mITT), isto é, pacientes que completaram pelo menos uma semana de intervenção. Não houve diferenças significativas entre os grupos na pontuação da escala de depressão de Montgomery-Asberg (MADRS)(diferença média dos quadrados mínimos entre os grupos na semana 3, 0,06 [IC de 95%, 3,39 a 3,51; P = 0,97] em favor da TBS simulada). As taxas de resposta e remissão avaliadas pela MADRS também não foram estatisticamente diferentes entre os grupos ativo e simulado (35,7% vs. 43,7% e 28,5% vs. 37,5% respectivamente na semana 6, ps > 0,51). Nenhuma outra análise desde o início do estudo até as semanas 3 ou 6 de tratamento revelou interação significativa de tempo x grupo ou diferenças médias entre os grupos na amostra mITT. A TBS bilateral aplicada no CPFDL não é eficaz como um tratamento adjuvante da depressão mista unipolar ou bipolar. REGISTRO: NCT04123301 (www.clinicaltrials.gov magnética)

Mixed depression is probably different in terms of clinical course and response to treatment. Repetitive transcranial magnetic stimulation (rTMS) is well established in non-mixed depression, and theta-burst stimulation (TBS) protocol is replacing conventional protocols because of noninferiority and reduced delivery time. However, TBS has not been adequately studied in mixed states. This study was a double-blind, 6-week, sham-controlled and randomized clinical trial of bilateral TBS targeting the right and left dorsolateral prefrontal cortex, respectively. Adults with bipolar and major depressive disorder experiencing an acute mixed depression were eligible if they had not benefited from a first- or second-line treatment for acute unipolar or bipolar depression recommended by the Canadian Network for Mood and Anxiety Treatments. Out of 100 patients included, 90 composed modified intention-to-treat sample, which were patients that completed at least one week of the intervention. There were no significant differences in Montgomery-Asberg depression rating scale score changes (least squares mean difference between groups at week 3, 0.06 [95% CI, 3.39 to 3.51; P = .97] in favor of sham TBS). Response and remission rates per MADRS were also not statistically different among active and sham groups (35.7% vs. 43.7%, and 28.5% vs. 37.5% respectively at week 6, ps > 0.51). No other analyses from baseline to weeks 3 or 6 revealed significant time x group interaction or mean differences among groups in the mITT sample. Bilateral TBS targeting the DLPFC is not efficacious as an add-on treatment of acute bipolar and unipolar mixed depression. REGISTRATION: NCT04123301 (www.clinicaltrials.gov)
