A gestação é reconhecidamente um período de alta vulnerabilidade para o sistema nervoso central. Exposições de estresse ambiental (fumo, estresse psicossocial, tóxicos, etc.) têm sido associadas a comprometimento do crescimento à alterações e na trajetória do neurodesenvolvimento. Desse modo, intervenções na gestação têm reportado resultados promissores. Mecanismos epigenéticos, como a metilação do DNA (DNAm), estão associados tanto a exposição aos estressores gestacionais como a desfechos neonatais e vulnerabilidade para transtornos mentais. Nesse sentido, é fundamental considerar o órgão interlocutor entre mãe-feto e ambiente, a placenta. A complexidade e as especificidades celulares da placenta humana são pouco conhecidas por falta de modelos de cultura, dessa maneira, essa tese conta com 3 objetivos principais: 1) Avaliar se alterações de metilação do DNA poderiam mediar a associação entre exposição a estresse intra-útero e desfechos neonatais de crescimento, 2) Avaliar se alterações no padrão de metilação do DNA poderiam mediar a associação entre uma intervenção gestacional precoce e desfechos do neurodesenvolvimento e 3) Avaliar modelo de cultura de células-tronco de pluripotência induzidas (do inglês, induced pluripotent stem cell, iPS) para diferenciação em trofoblastos para compreensão das vias relacionadas ao início do desenvolvimento placentário. Microarranjos da Illumina Methylation BeadChip 450k, foram usados nas amostras de sangue do cordão umbilical (SCU) e analisados pelos pacotes Minfi e ChAMP. O relógio epigenético (RE) estimou a idade gestacional (IG) a partir da metilação em sítios específicos (Knight e col., 2016), e, as diferenças entre o RE e IG (? idade), foram obtidas para avaliar uma possível aceleração ou desaceleração frente a IG. Para responder ao objetivo 1, avaliamos dados de estresse gestacional, DNAm e as medidas antropométricas ao nascimento (inseridas no WHO Anthro Survey Analyzer), coletadas no estudo transversal ROC (n=83). Essas medidas foram interpretadas como proxy de qualidade de vida fetal. Observamos que a aceleração do RE (RE < IG) associou a menores índices de crescimento. A exposição a diferentes fatores de risco deu origem a um escore de exposição em escala crescente. Separando as mães em grupo de maior e menor exposição a estressores, mães com os maiores escores de estresse tinham crianças menores, contudo, essa associação não foi mediada pelo RE. Para responder ao objetivo 2, avaliamos dados de uma subamostra com coleta do SCU (n=29) de um Ensaio Clínico Randomizado, que propõe uma intervenção precoce na gestação, por visitas domiciliares de enfermeiras a gestantes de alta vulnerabilidade social. As medidas de neurodesenvolvimento foram obtidas aos 12 meses de vida. Tanto sítios específicos, quanto a região diferencialmente metilada do PF4 mediaram a associação entre a intervenção no período gestacional e o domínio cognitivo das crianças aos 12 meses. Para responder ao objetivo 3, devido às limitações de se obter células placentárias humanas no início da sua diferenciação, desenvolvemos um modelo de cultura de iPS não-dependente de BMP4 que se assemelha com células trofoblásticas, especialmente o citotrofoblasto. Concluímos que, as situações estressoras estão associadas a padrões de DNAm, e que podem ser interpretados como mediadores ou marcadores de diferentes exposições ambientais. Ademais, acreditamos que algumas lacunas podem ser preenchidas na integração das exposições ambientais, DNAm e mecanismos placentários com o novo modelo de cultura de iPS desenvolvido, especialmente associados aos comprometimentos da diferenciação de células trofoblásticas

Pregnancy is known to be a period of high vulnerability for the central nervous system development. Exposure to environmental stress (i.e, smoking, psychosocial, toxic stress) have been linked to impairment of neurodevelopment trajectory and growth. Thus, interventions in pregnancy have reported promising results. Epigenetic mechanisms, such as DNA methylation (DNAm), are associated with exposure to gestational stressors, neonatal outcomes and vulnerability to mental disorders. The hypothesis to be tested in this thesis is that changes in DNAm mediate the relationship between gestational environmental exposure and growth/neurodevelopmental outcomes. Furthermore, it is essential to consider the very beginning of the placenta differentiation in order to understand the organ that is the first to receive stress signals and assure the crosstalk between mother and fetus. The complexity and single-cell specificities of the early human placenta are not fully clarified due to the lack of reliable trophoblast culture models. Thus, this thesis has 3 main goals: 1) Verify whether changes in DNAm could mediate the association of intrauterine stress exposure and neonatal growth outcomes, 2) Assess if changes in the DNAm could mediate the association of an early gestational intervention and neurodevelopmental outcomes and, 3) Evaluate a new model of induced pluripotent stem cell culture (iPS) for differentiation into trophoblasts in order to clarify the pathways related to the onset of placental development. Genome-wide DNAm from cord blood samples was assessed using the Illumina BeadChip 450K array and analyzed by the Minfi and ChAMP packages. The epigenetic clock (EC) estimated gestational age (GA) from methylation at specific sites (Knight et al., 2016), and the differences between RE and GA (? age) were obtained to assess possible acceleration or deceleration compared to GA. To answer objective 1, in the cross-sectional ROC study (n = 83), we assessed data on gestational stress, DNAm and anthropometric measurements at birth (WHO Anthro Survey Analyzer software), which were interpreted as a proxy for intrauterine environment. We observed that the slowdown in the ER (RE < IG) was associated with higher growth rates. Exposure to different risk factors gave rise to an increasing exposure score. Separating mothers into groups of higher and lower exposure to stressors, mothers with the highest stress scores had smaller newborns, however, this association was not mediated by the EC. To answer objective 2, we evaluated data from a subsample with cord blood collection (n = 29) from a Randomized Clinical Trial, which proposes an early intervention in pregnancy, through home visits by nurses to pregnant women with high social vulnerability. Neurodevelopment measures were obtained at 12 months of age. Both differentially methylated sites and the region of PF4 mediated the association between the intervention and the cognitive domain at 12 months. To answer objective 3, due to the gaps in obtaining human placental cells at the very beginning of their differentiation, we developed a non-BMP4-dependent iPS culture model that resembles trophoblastic cells, especially, the cytotrophoblast. We conclude that stressful situations are associated with DNAm variation, and that it can be interpreted as mediators or markers of different environmental exposures. In addition, we believe some gaps can be filled in the integration of environmental exposures, DNAm and placental mechanisms with the new iPS culture model developed, especially associated with the impairment of differentiation of trophoblastic cells
