A hipertensão intra-abdominal (HIA) e a síndrome compartimental abdominal (SCA) têm sido objeto de pesquisas crescentes na última década porque afetam a morbimortalidade em pacientes criticamente enfermos. O objetivo deste estudo foi determinar a incidência e os fatores de risco para SCA em uma unidade de terapia intensiva pediátrica onco-hematológica e analisar a evolução dos pacientes. Uma coorte prospectiva foi realizada de maio de 2015 a outubro de 2017. Duzentos e três pacientes com SCA foram admitidos, e 54 preencheram os critérios de inclusão para medição da Pressão Intra-abdominal (PIA). A PIA foi medida pela técnica indireta intra-bexiga com sistema fechado (AbViser AutoValve®, Wolfle Tory Medical Inc.) em pacientes com indicação clínica de cateterismo vesical de demora. Foram utilizadas as definições da Sociedade Mundial de Síndrome Compartimental Abdominal. Os dados foram digitados em banco de dados Excel (Microsoft Corp.) e analisados com o programa Statistical Package for the Social Sciences, versão 25 (IBM Corp.). A idade mediana dos pacientes foi 5,79 meses, e a mediana do PRISM IV foi 7,1. As incidências de HIA e SCA foram de 54,7% e 27,7%, respectivamente. A presença de massa abdominal foi fator de risco para desenvolvimento de HIA e a ressuscitação com fluidos foi fator de risco estatisticamente significante para SCA na análise univariada. As taxas de mortalidade foram 46,6% nos grupos ACS e 17,9% nos não ACS (p <0,05). Este é o primeiro estudo sobre HIA/SCA em crianças gravemente enfermas com câncer. A incidência e mortalidade foram elevadas, justificando a indicação da medida da pressão intra-abdominal em crianças com fatores de risco para SCA

Intra-abdominal hypertension (HIA) and abdominal compartment syndrome (ACS) have been the subject of increasing research for the past decade because they affect morbidity and mortality in critically ill patients. This study aimed to determine the incidence and risk factors for HIA/ACS in an onco-hematological pediatric intensive care unit and to analyze patient outcomes. This prospective cohort study was performed from May 2015 to October 2017. Two hundred three patients with ACS were admitted, and 54 fulfilled the inclusion criteria for IAP measurement. Intra-abdominal pressure was measured using the intra-bladder indirect technique with a closed system (AbViser AutoValve®, Wolfle Tory Medical Inc.) in patients with a clinical indication for indwelling bladder catheterization. World Society for Abdominal Compartment Syndrome definitions were used. Data were inputted in an Excel database (Microsoft Corp.) and analyzed with the Statistical Package for the Social Sciences, version 25 (IBM Corp.). Patients median age was 5.79 months, and the median PRISM IV score was 7.1. The incidence of HIA and SCA were 57,4% and 27.7%, respectively. Fluid resuscitation was a significant risk factor for ACS in univariate analysis. The mortality rates were 46.6% in the ACS groups and 17.9% in the non-ACS group (p < 0.05). This is the first study about ACS in critically ill children with cancer. The incidence and mortality were high, justifying the indication of intra-abdominal pressure measurement in children with risk factors for ACS
