INTRODUÇÃO: A insuficiência hepática aguda (IHA) é uma síndrome rara e grave. Define-se IHA na evidência de lesão hepática atual; ausência de doença hepática crônica conhecida; coagulopatia não corrigível pela administração de vitamina K; RNI > 1,5 na presença de qualquer grau de encefalopatia ou RNI > 2 na ausência de encefalalopatia clínica. A avaliação prognóstica precoce e precisa de pacientes com IHA é difícil, mas criticamente importante, pois o tratamento padrão para os pacientes que não melhoram é o transplante hepático (TH). Muitos modelos prognósticos diferentes foram desenvolvidos para auxiliar na decisão de transplantar um paciente com IHA ou não; no entanto, nenhum deles é preciso o suficiente para prever o resultado. Nossa hipótese é que existem fatores prognósticos específicos de pacientes brasileiros com IHA que podem ajudar na tomada de decisão. O objetivo deste estudo foi buscar fatores de risco e modelos prognósticos que subsidiem a decisão clínica de indicação de TH para pacientes com IHA. MÉTODOS: Estudo retroprospectivo em duas fases para analisar fatores e modelos prognósticos em pacientes com IHA. Fase 1 (piloto) retrospectiva incluiu 80 pacientes do ano 2004 até 2017. Fase 2 retroprospectiva incluiu 120 pacientes do ano 2000 até 2019. Critérios de inclusão: pacientes com diagnóstico de IHA no Instituto da Criança e Adolescente do Hospital das Clínicas da FMUSP desde o ano 2000 até 2019. Critérios de exclusão: pacientes com condições hepáticas crônicas, acima de 18 anos de idade e com dados incompletos de prontuário. Realizamos uma análise univariada dos dados laboratoriais (amônia, bilirrubinas, TGO, TGP, glicemia, lactato, albumina, ureia, creatinina, RNI, encefalopatia) associados ao desfecho dos pacientes com IHA. Baseado nos resultados da análise univariada, foram construídos modelos prognósticos através de análise logística multivariada. RESULTADOS: Os fatores de risco foram analisados por análise univariada de 120 pacientes e foram estatisticamente significativos para a presença de encefalopatia (p=0,001), RNI (p=0,001), TGP (p=0,029) e bilirrubina total (BT) (P=0,011). Modelos prognósticos foram construídos por meio de regressão logística multivariada, que resultou em 2 modelos: modelo 1 (RNI/TGP) e modelo 2 (RNI/BT). Ambos os modelos apresentaram alta sensibilidade (97,9%/96,9%), bom valor preditivo positivo (89,5%/90,4%) e acurácia (88,4%/88,5%), respectivamente. A característica de operação do receptor foi calculada para ambos os modelos e a área sob a curva foi de 0,87 para o modelo 1 e 0,88 para o modelo 2. O teste de Hosmer-Lemeshow não rejeitou a hipótese nula - que o modelo 1 é bom. CONCLUSÃO: Definimos fatores de risco e construímos um modelo prognóstico baseado nos valores de RNI e TGP para crianças com IHA lastreado em dados clínicos de pacientes do ICr-USP. Este modelo poderá ser uma ferramenta auxiliar na tomada de decisão para o TH e deverá ser validada prospectivamente.

INTRODUCTION: Acute liver failure (ALF) is a rare and severe syndrome. ALF is defined in the evidence of liver damage; absence of known chronic liver disease; coagulopathy that cannot be corrected by vitamin K administration; INR> 1.5 in the presence of any degree of encephalopathy or INR> 2 in the absence of clinical encephalopathy. Early and accurate prognostic assessment of patients with ALF is difficult, but critically important, as the standard treatment for patients who do not improve is liver transplantation (LT). Many different prognostic models have been developed to assist in the decision to transplant a patient with ALF or not; however, none of them are accurate enough to predict the outcome. We hypothesize is that there are specific prognostic factors for Brazilian patients with ALF that can help in decisionmaking. This study aimed to search for risk factors and prognostic models that support the clinical decision to indicate LT for patients with ALF. METHODS: It was a two-phase retroprospective study to analyze prognostic factors and models in patients with ALF. Phase 1 (pilot), retrospective, included 80 patients from 2004 to 2017. Phase 2, retroprospective, included 120 patients from 2000 to 2019. Inclusion criteria: patients diagnosed with ALF at the Instituto da Criança e do Adolescente do Hospital das Clínicas of FMUSP from 2000 to 2019. Exclusion criteria: patients with chronic liver conditions, over 18 years of age, and with incomplete medical record data. We performed a univariate analysis of laboratory data (ammonia, bilirubin, ASL, ALT, glucose, lactate, albumin, urea, creatinine, INR, encephalopathy) associated with the outcome of patients with ALF. Based on the results of the univariate analysis, prognostic models were constructed through multivariate logistic analysis. RESULTS: The risk factors were analyzed by univariate analysis of 120 patients and were statistically significant for the presence of encephalopathy (p = 0.001), RNI (p = 0.001), ALT (p = 0.029) and total bilirubin (P = 0.011) . Prognostic models were built using multivariate logistic regression, which resulted in 2 models: model 1 (INR/ALT) and model 2 (total INR/Total Bilirubin). Both models showed high sensitivity (97.9%/ 96.9%), good positive predictive value (89.5%/90.4%) and accuracy (88.4%/88.5%), respectively. The receiver-operating characteristic was calculated for both models and the area under the curve was 0.87 for model 1 and 0.88 for model 2. The Hosmer-Lemeshow test did not reject the null hypothesis for model 1 - that it is good. CONCLUSION: We defined risk factors and built a prognostic model based on the values of INR and ALT for children with ALF based on clinical data from ICr-USP patients. This model can be an auxiliary tool in decisionmaking for LT and should be validated prospectively.
