INTRODUÇÃO: A miocardiopatia do cirrótico está associada a um aumento da espessura da parede do ventrículo esquerdo, aumento de câmaras cardíacas e a um significante prejuízo na resposta ao volume. Existem poucos relatos na literatura sobre a avaliação da função das crianças com doença hepática e miocardiopatia submetidos a transplante hepático. Não há estudos na população pediátrica a respeito das modificações cardíacas após o transplante hepático. A ecocardiografia com speckle tracking permite a análise da dinâmica de contração ventricular, possibilitando a detecção precoce de disfunção miocárdica. OBJETIVO: Avaliar a função ventricular dos pacientes com atresia de vias biliares antes e após o transplante hepático utilizando novas técnicas ecocardiográficas como speckle tracking bidimensional. MÉTODOS: Estudo observacional, analítico, transversal com grupo controle saudável de voluntários. Foram recrutados pacientes de zero a 18 anos candidatos a transplante hepático e pacientes após seis meses do transplante hepático realizado por atresia de vias biliares no período de janeiro de 1997 a agosto de 2015 no Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Os pacientes foram submetidos a estudo ecocardiográfico convencional completo. Após essa etapa, foi realizada a captura de imagens para análise do speckle tracking bidimensional. Foi coletado também amostra de sangue para dosagem de Troponina T e dosagem de peptídeo natriurético atrial (BNP). RESULTADOS: A fração de ejeção obtida pelo método de Simpson foi significantemente maior no grupo pré transplante hepático (p<0,001), bem como o tamanho do átrio esquerdo (p < 0,001) e ventrículo esquerdo (p=0,039). O índice de massa ventricular esquerda foi significantemente maior no grupo pré-transplante hepático (p < 0,001), à custa de aumento da espessura do septo e da parede posterior. O volume do átrio esquerdo foi maior no grupo pós-transplante (p=0,008) assim como o índice de massa ventricular esquerda (p=0,035). O tempo de relaxamento isovolumétrico do ventrículo esquerdo foi significantemente mais prolongado nos pacientes pré-transplante hepático (p=0,021). Houve uma correlação inversa e significativa entre BNP e o Strain longitudinal global no grupo pós-transplante (p=0,038 e r=0,427). Foi detectada redução significativa do strain global longitudinal do ventrículo esquerdo antes (p=0,01) e após o transplante hepático (p=0,019). INTRODUCTION: Cirrhotic cardiomyopathy is associated with an increase in the thickness of the left ventricular wall, an increase in cardiac chambers and a significant impairment in response to volume. There are few reports in the literature on the assessment of the function of children with liver disease and cardiomyopathy who underwent liver transplantation. There are no studies in the pediatric population regarding cardiac changes after liver transplantation. Speckle tracking echocardiography allows the analysis of the dynamics of ventricular contraction, enabling the early detection of myocardial dysfunction. OBJECTIVE: To evaluate the ventricular function of patients with biliary atresia before and after liver transplantation using new echocardiographic techniques such as two-dimensional speckle tracking. METHODS: Observational, analytical, cross-sectional study with a healthy control group of volunteers. Patients from zero to 18 years of age who were candidates for liver transplantation and patients after six months of liver transplantation performed for biliary atresia between January 1997 and August 2015 were recruited at Instituto da Criança, Hospital das Clinicas, Faculdade de Medicina da Universidade from Sao Paulo. The patients underwent a complete conventional echocardiographic study. After this stage, images were captured for analysis of two-dimensional speckle tracking. Blood samples were also collected for Troponin T measurement and atrial natriuretic peptide (BNP) measurement. RESULTS: The ejection fraction obtained by the Simpson method was significantly higher in the pre-liver transplant group (p < 0.001), as well as the size of the left atrium (p < 0.001) and left ventricle (p = 0.039). The left ventricular mass index was significantly higher in the liver pre-transplant group (p < 0.001), at the expense of increasing the thickness of the septum and the posterior wall. The volume of the left atrium was greater in the post-transplant group (p = 0.008) as well as the left ventricular mass index (p = 0.035). The left ventricular isovolumetric relaxation time was significantly longer in patients before liver transplantation (p = 0.021). There was an inverse and significant correlation between BNP and the global longitudinal Strain in the post-transplant group (p = 0.038 and r = 0.427). A significant reduction in the global longitudinal strain of the left ventricle was detected before (p = 0.01) and after liver transplantation (p = 0.019). CONCLUSION: Left ventricular subclinical systolic dysfunction was evidenced using the two-dimensional speckle tracking technique before and after pediatric liver transplantation
