INTRODUÇÃO: A Síndrome de Prader-Willi (SPW) é uma doença genética rara, cuja história natural é a obesidade progressiva. A terapia com hormônio do crescimento (rhGH) desempenha um papel essencial na melhoria da composição corporal e na neutralização dos riscos cardiovasculares. No entanto, o acesso a esse medicamento não é apenas restrito, mas também retardado em muitos países devido ao diagnóstico tardio e / ou despesas com terapia não reembolsadas. OBJETIVOS: Avaliar as alterações antropométricas e metabólicas induzidas pela terapia com rhGH de acordo com a idade dos pacientes no início do tratamento. MÉTODOS: Os pacientes foram divididos arbitrariamente em 3 grupos, da seguinte forma: Grupo 1: crianças que iniciaram o rhGH antes dos 3 anos de idade; Grupo 2: crianças que iniciaram entre 3 e 10 anos; Grupo 3: crianças que começaram após os 10 anos. As variáveis antropométricas e metabólicas foram analisadas em três momentos: na introdução do rhGH, após 1 ano de tratamento e na última consulta relatada. RESULTADOS: A altura melhorou 0,67 DP nos grupos 1 e 0,7 DP no grupo 2. O IMC aumentou no grupo 1, mas no grupo 2 houve uma diminuição no primeiro ano de tratamento, enquanto no grupo 3, o IMC só melhorou na comparação do início e a última nota. A relação circunferência da cintura / altura foi reduzida no grupo 2 no final do primeiro ano de tratamento. CONCLUSÕES: Crianças com PWS que começaram a usar rhGH mais cedo melhoraram o IMC e a estatura. O uso precoce do rhGH como arsenal terapêutico de crianças com SPW demonstrou reduzir o ganho de peso progressivo, modificando um dos pilares da história natural desta doença

INTRODUCTION: Prader-Willi Syndrome (PWS) is a rare genetic disease, whose natural history is progressive obesity. Growth hormone (rhGH) therapy plays an essential role in improving body composition and counteracting cardiovascular risks. However, access to this medication is not only restricted, but also delayed in many countries due to late diagnosis and/or unreimbursed therapy expense. OBJECTIVES: To evaluate anthropometric and metabolic changes induced by the rhGH therapy according to age of patients at the onset of treatment. METHODS: Patients were arbitrarily divided into 3 groups, as follow: Group 1: children who started rhGH before 3 years of age; Group 2: children who started between 3 to 10 years old; Group 3: children who started after 10 years old. Anthropometric and metabolic variables were analyzed at three times: at the introduction of rhGH, after 1 year of treatment and at the last visit reported. RESULTS: Height improved 0.67 SD in groups 1 and 0.7 SD in group 2. BMI increased in group 1, but in group 2 there was a decrease in the first year of treatment, while in group 3, BMI only improved when comparing the beginning and the last note. Waist circumference/height ratio were reduced in group 2 at the end of the first year of treatment. CONCLUSIONS: Children with PWS who start using rhGH earlier improved BMI and stature. The use of rhGH precociously as a therapeutical arsenal of children with PWS proved to reduce progressive weight gain, shifting one of the mainstay of the natural history of this disease
