INTRODUÇÃO: a anatomia da artéria carótida interna e suas relações entre estruturas neurais, ósseas, vasculares e durais é extremamente complexa e de grande importância para o tratamento neurocirúrgico das lesões envolvendo esta região, principalmente os aneurismas. Apesar da extensa bibliografia relacionada ao cavo carotídeo medial existem poucos estudos descrevendo a anatomia cirúrgica da região lateral. OBJETIVOS: descrever a anatomia do cavo carotídeo lateral e do recesso carotídeo lateral e correlacionar os achados anatômicos com os exames radiológicos, enfatizando a sua significância clínica. MÉTODOS: estudo descritivo e observacional em que foram estudados 32 lados de cabeça de cadáver preparados previamente com injeção de silicone colorido nos vasos e infusão em formol para a realização do estudo anatômico, além da análise de todas as angiotomografias e angiografias realizadas no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo nos pacientes operados neste serviço com aneurismas da artéria carótida interna, entre os anos de 2012 e 2016 para a análise radiológica. Foram feitas medidas de frequência, de tendência central e dispersão. RESULTADOS: foram analisados 32 hemicrânios de cabeça de cadáver com idade média de 67 anos, sendo 60% do sexo feminino. O cavo carotídeo lateral e o recesso carotídeo lateral foram encontrados em 3 lados das peças anatômicas, sendo que foram concomitantes em 2 lados. O tamanho médio do recesso carotídeo lateral foi de 4mm de comprimento por 2mm de largura. Foram realizados 12884 procedimentos neurocirúrgicos no período de janeiro de 2012 a dezembro de 2016, sendo 489 destes procedimentos microcirurgias para tratamento de aneurismas intracranianos e destas, 117 eram para tratamento de aneurismas carotídeos. Neste trabalho 58 casos preencheram os critérios de inclusão e exclusão. A idade média dos pacientes foi de 54,5 anos (± 11,3DP) e 87,9% era do sexo feminino. O cavo carotídeo lateral foi encontrado em 3 pacientes, o recesso carotídeo lateral do lado esquerdo em 55,2% dos casos e o do lado direito em 51,7%. Não houve correlação entre a presença do recesso carotídeo lateral e complicações intraoperatórias. CONCLUSÃO: o cavo carotídeo lateral e o recesso carotídeo lateral são estruturas anatômicas que estão presentes tanto nas dissecções de espécimes cadavéricos quanto nos exames de angiotomografia dos pacientes analisados

INTRODUCTION: the internal carotid artery anatomy and its relationships between neural, bone, vascular and dural structures is extremely complex and of great importance for the neurosurgical treatment of lesions involving this region, especially aneurysms. Despite the extensive bibliography related to the medial carotid cave, there are few studies describing the surgical anatomy of the lateral region. OBJECTIVES: to describe the anatomy of the lateral carotid cave and lateral carotid recess and to correlate the anatomical findings with radiological exams, emphasizing their clinical significance. METHODS: descriptive and observational study in which 32 hemicraniums of cadaver's head were studied, previously prepared with injection of colored silicone in the vessels and infusion in formalin to perform the anatomical study, in addition to the analysis of all computer tomography angiography and cerebral angiographies performed at Hospital das Clínicas from the Faculty of Medicine of the University of São Paulo in patients operated in this service with aneurysms of the internal carotid artery, between 2012 and 2016 for radiological analysis. Frequency, central tendency and dispersion measurements were made. Results: 32 cadaver hemicraniums with a mean age of 67 years were analyzed, 60% of which were female. The lateral carotid cave and the lateral carotid recess were found on 3 sides of the anatomical pieces and were concomitant on 2 sides. The average size of the lateral carotid recess was 4mm long by 2mm wide. A total of 12884 neurosurgical procedures were performed from January 2012 to December 2016, 489 of which were microsurgical procedures for the treatment of intracranial aneurysms and of these, 117 were for the treatment of carotid aneurysms. In this study, 58 cases met the inclusion and exclusion criteria. The mean age of patients was 54.5 years (±11.3SD) and 87.9% were female. The lateral carotid cave was found in 3 patients, the lateral carotid recess on the left side in 55.2% of the cases and the right side in 51.7%. There was no correlation between the presence of the lateral carotid recess and intraoperative complications. CONCLUSION: the lateral carotid cave and the lateral carotid recess are anatomical structures that are present both in dissections of cadaveric specimens and in the CTAs of the analyzed patients
